THE E WAS REMOVING A BOTTOM CLAMP RING FROM A PIECE OF EQUIPMENT WITH A RATCHET. THE RATCHET SLIPPED AND STRUCK HIM IN THE MOUTH, CAUSING A FRACTURE TO HIS TWO FRONT TEETH.
WHILE HAMMERING BOLTS DOWN ON STEDMAN A PIECE OF METAL BROKE FREE EMBEDDING ITSELF IN LEFT HAND BETWEEN FORFINGER AND MIDDLE FINGER.
WAS COMING DOWN LADDER FROM TANKS AND STEPPED WRONG TWISTING ANKLE.
EMPLOYEE REPOSITIONED A STEDMAN RING ON THE WELDING TABLE WHEN HE LET IT GO, IT TIPPED AND WAS FALLING OFF THE TABLE WHEN HE ATTEMPTED TO CATCH IT. JOHN'S KNEE DISLOCATED TO THE SIDE AND WENT BACK INTO PLACE.
