Stepping off of a 1 foot platform in the Bagging Facility rolled left ankle resulting in a sprained ankle and hairline fracture.

While working to remove the base of the ore feeder, employee was fitting the slidebase into place to remove the feeder base, the slidebase twisted and pinched employee's ring finger on the left hand between the two pieces of metal causing the tip of the finger to become broken and cut.

Employee tripped over his own foot walking up steps over a secure walk way causing him to fall and fracture his left wrist.

Employee was outside talking with the operations superintendent. The employee was wearing glasses when the wind blew material into his eye. He washed it out immediately. He did not go to the hospital until the next day when he realized the material was still in his eye and that his eye was still irritated.

THE E WAS REMOVING A BOTTOM CLAMP RING FROM A PIECE OF EQUIPMENT WITH A RATCHET. THE RATCHET SLIPPED AND STRUCK HIM IN THE MOUTH, CAUSING A FRACTURE TO HIS TWO FRONT TEETH.
