EE was working on shaker screen in which he had implemented LO/TO procedures. He finished his work and then removed his lock and flipped the braker switch back on. An Arc Flash occurred when braker switch was activated causing 2nd degree burns to his face, lips, chin and neck.

ARKHOLA · Absorption of radiations, caustics, toxic and noxious substances

EMPLOYEE WAS TRYING TO START THE DREDGE ENGINE. THE SOLENOID ON THE STARTER STUCK CAUSING A SHORT WHICH CAUSED THE BATTERY TO EXPLODE. THIS GOT A SMALL AMOUNT OF ACID INTO THE EMPLOYEE'S EYES WHICH WERE QUICKLY FLUSHED.

Employee was changing fuse in electrical box at palnt, when he went to put in new fuse it arced causing 1st and 2nd degree burns to his hands and arms.
