THIS WAS NOT A MINE RELATED ACCIDENT. THIS SHOULD BE A CODE 10. THIS IS NOT OUR EMPLOYEE. THE SUBCONTRACTOR ENTERED OUR PROPERTY WITHOUT PROVIDING NOTIFICATION TO THE MINE OPERATOR. IN THE PRO CESS OF CUTTING TREES IN OUR FLOOD CONTROL CHANNEL, A FELLED TREE CROSSED A 12,000 VOLT POWERLINE. THE SUBCONTRACTOR ATTEMPTED TO REMOVE LIVE WIRE FROM THE FELLED TREE AND WAS FATALLY INJURED

EMPL PULLED FUSED IN SWITCH BOX SO BELT WOULDNT RUN WHILE THEY EMPTIED BUNKERS OF CONTAMINATED MATERIAL.BLEW 1 FUSE AS REPLACED THEM HAD PULLED HANDLE LOCKED OUT AND REPLACED 2 FUSES AND AS RE PLACED THE 3RD,THE BOX BLEW UP.LOOKED PROPERLY DISENGAGED.
