EE was operating an articulating haul truck when EE hit a bump and EE's back began to hurt. EE clocked out and went to EE's personal urgent care facility. EE reported EE's injury to the company July 23, 2024.

Miner walked out of parts bin and slipped on wet concrete pad. Miner experienced pain in hamstring area several days after the fall. Miner did not report injury to a supervisor until 3/8/23.

Mechanic was working on an excavator when the impact gun EE was using suddenly moved and slammed EE's finger into part of the machine.
Plant operator was descending a fixed ladder. Operator assumed they had reached the last ladder rung when they were in fact another step from the bottom. The operator jumped from the ladder and twisted their back.
Cat 775G Haul truck #3-74 dumped material into hopper and began to move away from dumping chute when fire started in engine compartment. Operator immediately exited machine without injury and fire was extinguished by nearby water truck.

Equipment greaser was greasing haul truck when ee attempted to align the grease hose from the grease gun into the grease receptacle. Grease was injected into skin when ee activated the grease gun.

Employee had a seizure while on the clock operating a haul truck. We don't think this is chargeable, but wanted to report it to be on the safe side, just in case. Employee later failed drug test, and was terminated.
Employee was assisting in an oil change. He spilled some oil, then slipped in the oil, falling backwards and catching himself with his right wrist. Wrist is thought to be sprained at this point, but not known for sure.

EMPLOYEES HAD SHUT PLANT OFF, PREPARED FOR THE WEEKEND, AND WERE THROWING A FOOTBALL AROUND. EMPLOYEE FELL BACKWARDS REACHING FOR THE BALL, HITTING HIS HEAD ON THE GROUND. HE WAS TAKEN TO INLAND VALLEY AND LATER TRANSFERRED TO LOMA LINDA HOSPITAL. HE PASSED AWAY AT LOMA LINDA HOSPITAL AT 1:05 PM 12/31/05.

Employee was carrying a strip of plastic, slipped in a muddy spot on decking, and fell to one knee. Employee's left knee hit the ground. Employee has not missed any work, but has now been referred to orthopedic specialist for further evaluation.
