Employee had just installed a new drain valve and was attempting to drain 5-1 mud block. After rodding out the drain nozzle, the drain valve came loose and he was sprayed with hot caustic slurry.

Employee was clearing suction line to 45-7 sump pump by beating on it with an 8# sledge. After several minutes employee turned to get an air lance when the suction line cleared and blew liquor and hydrate onto the employee. His hard hat and goggles were knocked off alowing caustic liquor to contact his face and left eye.

Employee was using hot condensate hose down sacle from the tail pulley area of 48 ball mill. The hose was equipped with a hose end valve. The employee closed the hose end valve and the hose ruptured spraying the employee's right wrist and right forearm.

EE WAS TRYING TO CLEAR PLUG (SCALE) IN PIPE ON 3-2 DISCHARGE LUKIE. PLUG BROKE LOOSE. CAUSTIC GOT INTO EMPLOYEE LEFT FOREARM/WRIST CAUSING A SECOND DEGREE CHEMICAL BURN
