Individual was standing beside the Long Wall Power Center when the high pressure hose came uncoupled striking the right ankle area, individual suffered a fracture to the right fibula, This did not involve any compliance issues, no equipment involved, it didn't involve job skills, training or attitude.

Individual was at the tail drive area of the longwall when a 13mm high pressure hose busted at the connector striking him, in a whipping motion, on the back and right elbow area. Individual received bruises to the entire back and a small laceration and swelling to the right elbow.

While filling a set of rock dust pods, the employee mistakenly unhooked a pressurized line. The pressurized line struck him in his chest and right arm, resulting in a laceration of the right arm, requiring 4 sutures.

EE was cleaning the bunker area with a high pressure water hose, the water hose broke at the coupler and the pressure caused the right hand to twist backwards causing pain and swelling to the right wrist. EE cont. to work until 5/26/11 when he went off work not able to lift with right hand.

EE was repairing a water leak on #69 shield. He removed a keeper pin on a 13mm. High pressure line. The hose blew and the pressure in the line cut his left ring finger and middle. Ring finger has 10 sutures and middle finger has 3 suture.

The injured EE was positioned in the jackline of O panel longwall near jacks 4-5 oberserving the shear cutting out at the head,when hydraulic hose fitting apparently broke,the hydraulic hose struck the employee on the left leg resulting in a fracture of the fibula.Indcident occured on May 11,08 and reported to mine mgmnt on May 15,08

Emp standing 5' away from borehole pipe waiting for shotcrete line with air blowing through to shut off. Line broke at top of borehole - falling - one of the joint pipes hit employee in head/face.
