Miner was using a chain fall to position the rock box for the 3 East transfer. He felt a pain in his left shoulder while pulling down on the chain fall trying to maneuver the rock box into its proper location. Given time off for shoulder pain. Was reaching overhead to pull down. No problems with mining methods, ppe, training or compliance issues.

Employee was performing belt work along a section belt entry when the "J" hooks holding the waterline up to the roof channels, failed causing the waterline to fall onto the employee's shoulder knocking EE to the floor. Employee recently began missing work to have surgery on rotator cuff due to this past injury. Employee didn't violate any regulations and was properly trained.

Injured picked up winch hook to move from behind bucket ram into the scoop bucket. While positioning the hook in the bucket he pinched his gloved middle finger between the winch hook and the bucket, receiving a fracture. Occurred in track heading of 1 East, 27 block. No problems with training, ppe, mining machines or compliance issues.

While bolting in the 2E beltline and dragging Z-Drill employee felt pain in right shoulder resulting in possible rotator cuff injury. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies.

The mine's Safety Department received notification on December 8, 2015 that this retired former employee was granted an award for permanent impairment from noise-induced hearing loss. The incident was caused by the employee's failure to wear the appropriate PPE - hearing protection throughout his career. The employee retired on 12-30-2014.

Miner was turning water pressure back on at 2 block 2 east at the end of the belt spur (after installing booster pump). His hands slipped off handle and fell over backwards hitting the back of his head on the edge of a supply car causing a laceration that required staples. No problems with mining methods, ppe, training or compliance issues.

While de-trashing 24W transfer area he picked up a bottom roller that was leaning against rib, the shaft caught on the rib, pulled out like a plunger and then smashed down pinching his right middle finger between shaft and roller shell. While carrying a pump, the employee slipped and stumbled and afterwards complained of pain in his right lower abdomen. The mine's Safety Department received notification on Dec 18, 2015 that this is a hernia and the employee will undergo surgery on Dec 29, 2015. The incident occurred because the employee was not aware of the bottom conditions.

While lifting a shuttle car anchor, the ee overextended his left shoulder. The ee first began missing work on 12-11-2015 & is periodically attending physical therapy. The incident occurred because the ee failed to get assistance when lifting over his head. The incident did not involve the mining system nor any PPE. The ee was properly trained and has a good safety attitude.

Employee was walking back to belt entry after taking equipment out to the track, he stepped on a piece of belting laying on the floor and twisted his ankle. Cause of this injury was employee awareness to the task at hand. Compliance with rules/regs. not a factor, miner proficiency could be a factor, PPE was not a factor, mining equipment/systems not a factor.

Trolley pole of 126 jeep came off the wire while traveling Main West haulage. Injured leaned over to put pole back on wire and felt pain in lower back. Continued to work normal job until 11/28/15, when he missed 3 days due to the injury. No problems with mining methods, training, ppe, or compliance issues.

Using come along, slate bar and hammers to remove a section of handrail off the panline (near headgate in 24W). Injured was standing in front of the handrail prying it off and it landed on his left foot causing a contusion. No problems with ppe, training, mining machines or compliance issues. Body placement was very poor.

Injured replaced a piece of grating on slope catwalk at 2780, then walked down catwalk and a separate piece of grating failed and he fell striking his right shoulder on the concrete floor below. Started missing work much 11/23/15 for persistent shoulder pain. No problems with mining methods, training, ppe or compliance issues.

Walking bleeders from 24W to 17W and felt pain in lower back, going down both legs, when he got to bleeder fan. Miner has history of lower back issues (herniated lumbar disc) aggravated by walking bent over and hitting his head on the roof a few times. No problems with training, mining methods, PPE or compliance issues.

Miracle Elevator doors gapped open enough to activate door switch, which locked brakes and jolted over speed switch, activating it as well (discovered SOS has weak set spring, replacement ordered). 4 miners entrapped approx. 2 hours about 10 feet off bottom. No problems with training, ppe, mining methods or compliance issues.

Using a pneumatic "tugger" winch to pull drill rods to lay down position while tripping out of the hole. Then putting winch line in stow & was trying to roll winch line on neatly. While doing had his hand in improper place so when the winch line was pulled tight his finger was in between bracket mounted on and winch line. His finger then "smashed" between the cable and bracket.

While carrying a 2" air pump in the bleeder walkway near 21W tap he stepped on a section of slick line and fell backwards, landing on his back. The location where he tripped was dry. He is experiencing localized pain in the upper lumbar and has been referred to a specialist. No problems with training, PPE, mining machines or compliance issues.

After handling a miner cable, the employee felt pain in his back. The incident occurred due to the ee's improper lifting and handling of the cable. The individual was welding with a DC Welder in a tight area when the left side of the neck came in contact with the stinger and received an electrical shock. This did not involve compliance issues, no equipment involved. Personal safety equipment being utilized. This did not involve job proficiency, or attitude. Individual went off work for surgery 1/21/2016.

Unplanned fall discovered reportable of brow of southern approach to 2 belt trench at the drive. Fall was above anchorage and 4' high, 8' foot long, 16' wide. Investigation discovered brow separated in area of damp, deteriorated clay material. Fell to trench excavation height. No problems with training, PPE, mining machines or compliance issues. Area mined in 1965.

Employee working on clogged trash pump at 11 tail (Guzzla SV-60 pump). He was back flushing the pump and mud blew out the suction line and got into his eyes. He was wearing glasses. He missed no work, but did receive medical attention in the form of medicated eye drops. No problems with training, PPE, mining methods or compliance issues.

A roof fall (66'L X 9'H X 18'W) was found on the old 6-North Track outby spad location 7810. This did obstruct the walkway but did not involve any other compliance issues. No equipment was involved and no other 50.20 criteria applies. The fall approaches will be supported and walkway rerouted.

Winching a car of belt with a belt winder on 7 belt. He pulled off rope to get more slack and got his foot caught in some hydraulic hoses. When he energized the winder, the hoses moved and tripped him. Diagnosed with fracture to distal end of fibula. No problems with training, mining methods, PPE or compliance issues.

During a pre-operational inspection, the hoist operator discovered that a crack in the load line drum caliper brake assembly. The hoist was immediately taken out of service and MSHA was notified.

While sitting in a vehicle, it was struck by another vehicle causing the ee to fall to the floor. The incident was caused by a careless operator of the 2nd vehicle who did not observe the 1st. The injured ee was wearing the appropriate PPE but has a poor attitude. The operator was trained but received retraining in the safe operation.

The Metz elevator experienced a door fault error and engaged the safety brakes, entrapping 2 miners for 40 minutes approximately 175 feet from the top. The safety fault was reset and the elevator tested numerous times but would not reproduce the fault. No problems with training, PPE, mining methods or compliance issues.

Replacing thermocouple on the thermal dryer, but the hole was too small. He used a 1" pipe and 3 lb. hammer to widen opening. He missed the pipe and struck his gloved left hand with the hammer, opening a wound in the soft tissue between the thumb and index finger. Required 7 sutures. No problems with training, PPE, mining methods or compliance issues.

A roof fall occurred above the anchorage point at survey station 57+35on the mainline near the river portal. The fall was 12 feet long and 10 feet high and the width of the entry. The fall occurred due to the age of the area, weather it didn't involve any compliance issues it did impede travel, no equipment was involved no other 50.20 criteria applies.

Bolting (and screening) in 1East #1 entry, 17-18 block, in an area with exposed shale. The ATRS was set and he was starting to drill first hole when a rock (18"x24"x5") broke loose outby the drill canopy, swung around and struck him in the lower back. Started missing work on 10/2/15 for contusions. No problems with training, PPE, mining machines or compliance issues.

Employee was bolting off of the continuous miner with the rock deflector up & the rib door extended when a rock (46"x35"x6")fell from in front of the last strap hitting the rock deflector then hitting the rib protector. The rock then came into the bolter station striking EE in the back. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved.

Employee was injured on 9/20/2015 and worked until 10/20/2015 when doctor recommended change in treatment and pulled him off work. While riding in BT24, ride picked the switch and derailed causing employee to strike his left elbow and shoulder against the side of BT24. It did not involve compliance issues, job proficiency or attitude. All personal safety items were being used.

A roof fall was discovered in a non-active area of 6 North #4 belt at 285 block intersection. The roof fall was estimated at 16 ft in width and approx. 30 to 40 ft in length. The roof fall is approx. 10 ft above the coal seam. The area was initially bolted with 8 ft. 2 piece combination bolts. No injuries. No equipment damaged. No impared ventilation.

Individual was dismounting 24 track mobile not concentrating, caught his foot on the vehicle frame or safety chain causing him to fall (sprain / strain) to the left foot. Fixing a leak in the slick line at 23W #1 around 6 block when the coupler failed, spraying the miner with glue compound being used to "rock lock" the tailgate. Miner missed no work, but was prescribed antibiotic eye drops. No problems with mining methods, training, PPE or compliance issues.

It was discovered the Sugar Run elevator was down for equipment damage after the MonPower substation at Harvey Run lost a phase causing damage to the elevator generator (4 resistor banks were burnt up and #5 line starter had damage to the tips). Repairs made by Schindler. No problems with mining methods, training, PPE or compliance issues.

Unplanned roof fall determined reportable on 24W longwall tailgate. Fall not above anchorage, but restricted travel. The travelway was impassable from 86+39 to the longwall face. 2-3' of dark, moist clay/shale was discovered above the wild coal in this location during the investigation. No problems with mining machines, training, PPE or compliance issues.
