INJURED WAS OPERATING A MOBILE BRIDGE CONVEYOR WHEN A COAL RIB POPPED OUT AND COAL STRUCK HIM ON THE SIDE OF HIS FACE AND HEAD.
INJURED STATED THAT HE WAS SHOVELING SOME LOOSE MATERIAL THAT HAD FELL FROM THE RIB WHEN ANOTHER SMALL PIECE FELL FROM THE RIB AND STRUCK HIM ON THE SIDE AND TOP AREA OF THE LEFT FOOT. INJURIE D'S X RAYS FOUND NO BROKEN BONES. THE MINING HEIGHT WAS AROUND 5 FEET.
