Helping two men remove a set of air lock doors. When he removed a jack a piece of draw rock fell hitting and cutting his upper lip.
This mine is in inactive/non-producing status. Barometric Pressure change with water blown from trap in SC #2 seal. Curtain torn down by rock. Safety Dept Personnel and MSHA Inspector found 5% methane. Men were immediately pulled and power turned off. Action plan put in place under the direct supervision of two MSHA Inspectors who were on-site.

Fell crossing belt, landing on tailbone and back.
Picked up a mini master hoseduster and turning it around. Felt pain in lower back shooting thru left leg.
Greasing Dolly Wheels.Started to get up and twisted right knee.

Got off Shuttle Car to check cable. Tripped on cable and fell down, fracturing/breaking left wrist.
Loading rock dust. Something flew under safety glasses and went in left eye causing pain and swelling.

Helping load rock duster in Mac 8 mantrip. Pulled something in upper leg.

Mechanical Failure, elevator shut down, men sent home, reported for repair.
EE was outside loading supplies to take U.G. EE was cutting steel straps on a bundle of roof bolts and cut his right index finger.
EE was struck by a rib roll on the left ankle while tramming the miner on 008 section. No broken bones, pain and swelling only.
EE was putting together water line for the continuous miner with a banding tool. His right hand slipped & his right pinky finger was caught between the water line & the banding tool while the hammer was coming down. He mashed his pinky resulting in an open wound, loss of nail, & bone was broken at the tip of the finger. No lost work days. Returned to normal work activity.
While bolting, a piece of draw rock fell and cut right arm, above elbow. Resulted in 14 stitches.
EE was working on #5 belt scraper. He was pulling out a belt pin, when he slipped and fell across the belt. He scraped and bruised his right rib and his right leg.
EE was struck by a piece of draw rock while bolting. It hit his left arm and shoulder. He suffered from cuts on his arm and soreness in his shoulder. No broken bones.
Employee was carrying an old bull gear that was removed from a head drive. He dropped the gear on his right hand resulting in swelling and bruising.

EE was driving a pin in with a crib block on a piece of equipment. The repetitive motion caused his right wrist and arm to become sore. This resulted in a mild sprain. He visited Pineville ER and was released the same day. No lost work days.
Injure was repairing a Kennedy Stoppings on the belt line. A rib rolled off and struck him on the left shoulder, resulting in bruising and swelling.

Elevator down. No accidents or injuries. Went down at 0330, repaired at 1730.
EE was mucking the #5 belt at the overcast. A piece of draw rock fell and struck him on the right shoulder and the arm. This resulted in pain, bruising and swelling of the right arm.
EE was replacing a tire on the std car on section 008. The tire slipped, resulting in EE straining his back (upper) and his neck. Pain in back and neck.
EE was roof bolting when a bolt strap fell and cut his right forearm. Resulted in 7 stitches.
Employee was rockdusting 008 section at the intake. He accidently threw some rock dust in his eye while using the rockduster. His left eye was swollen and irritated. Seeked medical attention.
Employee was driving roll pins out of a tail roller. A piece of metal chipped off the punch and struck his left wrist above his gloves. Cut on wrist, open wound.
Employee was opening a can of oil. His hand slipped with trying to open the lid. He cut his middle finger on his right hand on the lid. Required 6 stitches. No lost time.
Employee was walking through a vent curtain. He had his right hand on a crib and a piece of rock dislodged and struck his hand. This resulted in a broken bone in his hand. He was not operating any machinery.
Employee was shoveling belts. A blunt object flew up in the air and hit his right elbow, resulting in a bump. Some swelling and pain.
EE popped his left knee while lifting his lunch box, EE complained of pain and swelling.
Employee was performing a belt splice. He bent over to grab the belt with his right hand and had the belt pin in his right hand. He leaned down and stuck the pin into his left eye. The pin penetrated the muscle area in the corner of his left eye and scratched the first layer of his eye. It did not affect his vision. No lost work Days.
Employee was hanging a cable while standing on top of a scoop. When he stepped off the scoop, he jammed up his right leg & hip. Complained of pain in his right leg, hip & back. Pinched nerve in right hip.
EE was removing the canopy on the off side of the roof bolter. He lowered the canopy too far down and it fell. It hit him in the shoulder and the back. He had a small cut on his head and bruises on his upper and lower back. He had a back injury and was transported by helicopter to the University of Tennessee Medical Center.
Rock fall reported to mine management at 5:05 am. The location of the fall is the right corner intake side of the #2 seal. The fall was 10' long, 7' wide and 4' thick. No accidents or injuries to report. The MSHA hotline was called and fall was reported. Three roof bolts where pulled out as of result of fall. This area is currently being cleaned up and bolted.
EE was loading a car tire into a scoop bucket. He sprain a muscle in his lower back. Left side of lower back and pain down left leg.
Employee was pulling broken unit back together when the handle came out of the rope hoist, striking his right thumb resulting in a broken bone and 4 sutures.

While watching his cable, employee put his hand between the boom and the frame of the shuttle car. When he let the boom down, his left hand was in a pinch point. He injured his pointer, index and ring fingers. Open wounds and avulsion of the fingers.
Employee hit his head on the right side directly above his temple. He walked into an open window at the mine office. He cut his head on the corner of the window frame resulting in an open wound about 2"" long. No lost time or restricted duty.
Employee was struck in the right elbow by a bolt wrench while bolting. No open wound or broken bones. Swelling and bruising occurred.
While installing a shaft on the Joy continuous miner on section 009, his right hand got caught in the machine, cutting the tip of his pinky finger. Required surgery.

Roof fall in the intersection of #4 entry 008 section at the last open break along anchorage. Ventilation not affected. No injuries.
There has been a roof fall outby spade #5539. 10-12"" thick and apprx 50'long. The area has been dangered off. It is not abstucting air flow and poses no danger at this time. A clean up plan has been posted at the site off the fall and a copy has been submitted to both state and federal inspectors.
While using a grinder, a foreign object flew into his left eye. He was wearing safety glasses. A minor injury. No lost time.

EE was cutting a piece of belt at the #2 head drive. The knife slipped off the belt and cut his leg. He cut his left leg on the inner calf-3in. cut- received medical attention. no lost time.

Employee was working on a slope. He slipped while holding a metal beam. The bean hit his foot (left) Some swelling and bruising. 1st Aid Applied. Went to hospital.
EE was shoveling #2 belt when a rock from crib rolled off and struck him in the mid-section of his back. Small scratches, no bruising at the time, no bleeding. Complaining of pain in that area. He was asked if he had pain anywhere else and he said no.

He was stretching out pump cable around corner through break when rib rolled off striking worker in head and shoulder, knocking him to the ground.
While dislodging rock in miner conveyer the rock bar glanced off rock and punctured top of foot through boot.
Blind side of car pulling miner cable; caught left foot between car and rib. No break in skin or broken bones; swelling of ankle.
