The employee stated that he had moved over to the center of the entry behind the miner to check his sights. A rib roll occurred inby of the last row of bolts and a piece traveled outby and struck the employee in the ankle.

The employee was struck on the hard hat and shoulder by a piece of rib rock. After the end of the shift the employee suffered symptoms of a concussion.

Location B9HG #3 Entry, 6XC. Personnel were shoveling ribs, a piece of rock (approx. 3x16x18" in 3 pieces) came loose and fell from the roof and rib line. The rock struck the employee on the left foot. Injuries consist of soft tissue tears to the foot, scraps and contusions to the left knee, left neck, shoulder and left hand.
