I.E. stated they were on the clean-up car when a piece of the coal rib slid out from behind the rib wire pinning their leg against a shuttle car. X-rays showed their Fibula was broken.
I. E. stated that they were standing next to the rib and a o=piece rolled out striking employee in the left ankle. Employee went to hospital x-ray showed nothing broke they followed op with Ortho. doctor that took them off work due to muscle injury. Employee was issued crutches
I.E. stated I.E. was loading the flinger duster on the scoop bucket outby the section when I.E. backed out of the XC I.E. hit the rib with the bucket, when I.E. checked the functions on the duster, I.E. had a leak on one hose. I.E. got out of the scoop to tighten hose, while tightening the hose a small piece of the rib fell out and smashed I.E.'s small finger on scoop bucket.
Employee was collaring off for drill out when a rock fell from the shaft rib from approximately 15'and struck them in the back of the head just below their hard hat causing a laceration.

Employee was walking between crib line and rib when some coal and rock from rib rolled and struck him on the left shoulder area. This incident occurred on 10/31/14. The employee has been having issues with the shoulder and he was diagnosed that surgery is needed for a shoulder tear.
EE was driving shuttle car coming around corner when the car cable grabbed the cm cable and pulled it out in front of car. EE got out to move cable out of way when a piece of rib rolled out striking him on shoulder. EE continued to work until he had to have surgery on 10/28/14.
employee was installing the rib bolt and the rib rolled out, striking the employee on the left foot fracturing his left foot.
The employee was walking along the longwall face and the face of coal rolled out on the panline and struck his left foot, he was wearing rubber boots and coal punctured his boot. He went to the hospital and received stitches in his left foot.
Employee was helping the miner operator move into a cut to mine, as he threw the cable over against the right rib out of the way, the rib rolled out and hit him on his right side. He twisted his back, bruised his right shoulder and cracked his ribs. He was taken out of the mines, and went to a hospital for x-rays.
Clean shaft bottom. Rock fell from rib and hit employee on left shoulder causing head contusion with secondary cervical strain
Employee was walking between the rib and the single boom. The rib rolled out on the corner striking employee into the platform of the single boom. Hitting the platform cut his right shoulder and a rock came to rest on right leg.
