An employee experienced a medical emergency underground, and the cause was not immediately known. As a courtesy, MSHA's emergency line was notified. Following a thorough investigation by MSHA, the State of Illinois, mine and contractor management, along with a diagnosis from a licensed healthcare professional it was determined the event was not caused by a hazardous mine condition.
I.E. stated EE was helping carry a material door, the door twisted in EE's hands causing a pop in EE's wrist. X-ray shown a fracture of EE's left radius.
Employee turned in report on 8/6/2025 complaining of a sore back and neck. Employee worked until they had a surgery on 2/10/2026 and was taken off work.
A non-injury accident occurred in the form of a hot spot; smoke was observed with no embers or flames seen. Hot spot was soaked with water until cool.

I.E. stated EE was cutting a pump hose when EE's knife slipped cutting the top of EE's left hand. I.E. received 6 stitches with a full work release.
IE was using a hammer to drive out a frozen pin and hit their thumb causing a fracture
I.E. stated they were removing straps from trailer and stepped on a glut causing knee to pop, employee is off waiting on MRI.
I.E. stated they stepped out of shuttle car twisting ankle on rock. X-ray showed no breaks or broken bones.
I.E. stated EE was using a track jack to raise monorail employee's hand slipped off handle letting it strike EE in the face and head.
I.E. stated that they had a rock slide from between the screen wire and the roof striking employee on the left side of their nose. X-ray shows a fracture and the employee received 5 stitches on their nose. Employee has a full release.
I.E. stated they were riding in a mantrip as a passenger, the driver hit a hole in the travel way causing the E.E. to hit their head on the canopy. I.E. has worked until 3/23/2026 until doctor took employee off work.
I.E. stated they were pulling on a 6-foot bolt that was being installed into the roof when they felt a pop in right arm. Employee went to E.R. with a full release. Employee went to ortho. doctor and was taken off work (light duty) on 11/5/2025.
I.E. stated EE was picking up a bottom roller when another employee dropped another roller down smashing EE's hand between the two. Employee received 18 stitches on EE's right hand.
EE stated they were removing a 12' cable bolt from tray when the end hung on a glue box coming back striking employee in mouth breaking a tooth.
A non-injury accident in the form of an unintentional roof fall located in entry 9 XC 94-95 of the North Mains 2.
I.E. stated a rock fell from in between 2 shiels hitting employee on EE's right middle finger. Employee transported self to ER and received 4 stitches and X-Ray showed a fracture.
I.E. stated the Bobcat that they were driving overheated, employee took radiator off machine. Hot anti-freeze burned their inner arm.
The IE was using a prybar and it slipped out causing the IE to fall and land on EE's elbow. The IE continued to work until 10/1/2025 when EE received surgery.
while operating a BELL 30T ARTICULATING haul truck, operator was exiting dump area with empty load and traveled at an angle to the muddy ruts on the dump. operator states that left front wheel went into rut, and jarred EE. later in the shift EE complained about soreness, refused medical attn at that time. finished EE's shift. The next day EE went to the E.R.
Secondary escapeway mine hoist had a faulty contactor, all production was stopped until hoist was back up and running.
An unintentional roof fall occurred on 8/15/2025 at 3:35 am located at A slope sump in the slope runaround.
An unintentional roof fall occurred in the HG 13 intake air course entry 2, crosscut 7, in the weekly examiners route. Exact time in unknown due to last exam was done on 7/29/2025. Fall was discovered at 9:30 am on 7/31/2025.
IE was stepping down from a feed box to screen and lost EE's balance and fell on EE's right elbow. The doctor put EE in a sling and ordered EE off work
Was working on D6T dozer that was down, was climbing down on the left side, holding handrail with right hand and right foot on the push arm, left foot slipped off push arm step, and hit right ribs on the tracks.
I.E. stated EE was helping recover longwall mono-rail hoses, as the crew was pulling the hose the miner strap slipped due to wet and muddy hose. the end of the hose hit I.E. in the side of the foot. X-ray showed a fracture of EE's Metatarsal. Employee was wearing steel toed metatarsal boots.
I.E. stated EE was using a bottom roller bracket as a handle to pull a bottom roller into a crosscut, as EE was pulling the bracket came of causing EE to lose balance and fall landing on EE's left wrist. X-ray shows a fracture.
I.E. stated they were stepping off of the platform backwards, their left foot was on the ground and their right foot hung on the 5-inch lip on the edge of the platform. I.E. fell backwards hitting their back on the foot of the ATRS. X-rays show a fracture in their L2.
I.E. stated they were dragging a tube from P.C. area to 2 left when they felt a pull in left shoulder. I.E. took themselves to the hospital for C.T. scan, waiting on results.
IE was working on a conveyor belt, lost EE's footing, and fell. The IE felt pain in EE's back and was taken off work for the pain in EE's back
I.E. stated EE was setting up to weld and cut on miner head, Employee set oxygen bottle up and when picking up acetylene bottle the oxygen fell smashing EE's left thumb between the 2 bottles. X-ray showed displaced fracture.
The IE had been welding and cutting on belt structure. IE is not sure if metal got in eye or it was dust due to the high winds that day. The IE ended up going to the doctor to get the material removed, after attempts to wash it out were not successful. The IE received eye drops after the doctor removed the materials from IE's eye
I.E. stated they were on the clean-up car when a piece of the coal rib slid out from behind the rib wire pinning their leg against a shuttle car. X-rays showed their Fibula was broken.
I.E. stated EE was getting a bag of rock dust off the roof bolter and slipped hitting EE's ribs and shoulder on the machine.
I.E. stated I.E. was driving a Bobcat, when I.E. went over uneven ground it jarred I.E. hitting I.E.'s ribs on the lap bar. I.E. went to hospital for X-Ray with no broken bones.
IE was using a razor knife to prep a cable for a splice and while cutting the outer jacket, the knife slipped and cut IE's left ring finger resulting in a cut that required 5 stitches
A non-injury accident occurred when a roof fall occurred on the travel way of B-slope run-a-round.
I. E. stated that they were standing next to the rib and a o=piece rolled out striking employee in the left ankle. Employee went to hospital x-ray showed nothing broke they followed op with Ortho. doctor that took them off work due to muscle injury. Employee was issued crutches
I.E. stated their loose-fitting sleeve was caught between roof bolt and chuck pulling the sleeve off the shirt. Employee had full release with no restrictions until 3/26/2025 they were complaining of back pain due to incident and doctor took them off work.
I.E. stated they were on the floor sliding out from under a mantrip in the shop, I.E. stated they twisted as they were getting up on their feet and felt a pain in lower back.
The IE was washing the floor in the prep plant and walked under a conveyor belt. A bottom roller bracket failed at that time and the bottom roller fell, striking the IE.

I.E. stated EE was washing out the radiator of a Bobcat and the radiator cap blew off spraying EE's left arm with anti-freeze causing 2nd degree burns.
I.E. stated EE was taking belt structure apart when EE smashed EE's finger between top roller frame and belt rail. I.E. received 4 stitches in EE's left ring finger with follow up with Orthopedic doctor.
The IE stepped out of the mine's guard shack and slipped on ice that formed on the wooden steps. As the IE fell, they hit their head and shoulder on the step. The IE continued to work until they went for shoulder surgery on 6/9/2025.
I.E. stated I.E. was helping change out belt on main north 3 and slipped and fell. When trying to catch self I.E. cut left hand on an unknown object on the mine floor.
I.E. stated they were hanging chains for a belt move when I.E. felt a strain in their neck. Employee went to Hospital where it was determined to be a strained muscle, employee was prescribed a muscle relaxer and returned to work, no lost time.
I.E. stated EE was changing a top roller on A-slope when the roller slid smashing EE's left ring finger. I.E. went to franklin hospital where EE received 3 stitches and x-ray showed tip of finger was fractured.
I.E. stated they were helping take a steel plate off the bed of a pick-up truck, when the employee lifted, they felt a pull in their back.
I.E. stated they was helping roof bolters put up rib wire after the top was bolted, after roof bolter tacked up rib wire i.e. turned their back to the rib and a rock fell striking them in the right shoulder. I.E. went to the hospital for X-ray, no broken bones or dislocations. Doctor released employee on restricted duty.
I.E. stated they were filling dust tank with hose hooked from surface, I.E. stated the bull hose came loose hitting them in the mouth. I.E. went to the Hospital and received stitches on their lip and lost 3 teeth.
I.E. stated they were cutting top LW chain on the LW head drive after the chain had broken, all chain tensioners were released, and slack was in chain. When I.E. cut a piece of chain that was on the head drive the piece had stored energy and hit I.E. on their right forearm. I.E. went to the hospital and received 4 stitches.
I.E. stated right shoulder popping while doing repetitive motion. (roof bolting)
I.E. stated they had just finished changing canoe liners in HG 10 flow through, as I.E. was walking away from the flow through I.E. slipped in the mud causing right hand to fly up becoming wedged between the belt and the bottom roller. I.E. went to a hospital where X-rays showed no broken bones. I.E. is supposed to follow up with Ortho for full release
I.E. stated I.E. was helping take old pick breaker off the top of feeder; while dropping pick breaker down 2 belt rails were placed to protect the control panel and allow the pick breaker to slide down. Something hung on one of the belt rails causing the rail to get stored energy, that rail broke loose hitting i.e. in lower leg. X-ray showed break in lower leg.
Employee was pushing on a come a long handle while loaded with weight when employee strained left shoulder.
Employee was moving components of a vulcanizing press when employee's right middle and index fingers were pinched between the plate and the table. The injury resulted in a fracture and 9 sutures. It was determined on employee's follow up appointment on 6/1/24 that it would be a lost time injury.
I. E. stated I. E. seen the fork truck hit a piece of metal with the rear of fork truck. I. E. ran to try and stop the piece of metal from falling, I. E. could not hold metal up and the piece of metal fell hitting I. E.'s left foot.
I.E. stated that they had a utility knife in right pocket as they were working recovering belt. I.E. bent down to pick up a piece of belt, when they stood up they felt a burning on thigh. I.E. found that the knife in pocket opened and cut their thigh when they stood up. I.E. went to hospital and received stitches.
I.E. Stated EE was strapping down a load of bolting supplies at cage bottom and stepped on a rock and twisted EE's knee.
Employee conducts repetitive motion work as a computer programmer which resulted in carpal tunnel in both hands and wrist. The employee had surgery on 8/28/2024
I.E. stated was walking up a steep hill with a cat head over left shoulder and as I.E. was walking up the hill felt a sharp pain shoot down left shoulder and forearm.
Employee was pulling on a piece of conveyor belting when employee felt pain in right shoulder.
I.E. was putting new discharge hose on air pump, while tightening hose clamp nut driver slipped causing them to cut lower part of thumb on tail of hose clamp. I.E. went to clinic and received 8 stitches after the shift was over.
EE stated they saw the emulsion line had blown and they were running down the travel road to find a shut-off when they rolled an ankle. Employee went to the hospital with no broken bones but was issued a walking boot and crutches.

I.E. stated I.E. was loading the flinger duster on the scoop bucket outby the section when I.E. backed out of the XC I.E. hit the rib with the bucket, when I.E. checked the functions on the duster, I.E. had a leak on one hose. I.E. got out of the scoop to tighten hose, while tightening the hose a small piece of the rib fell out and smashed I.E.'s small finger on scoop bucket.
I.E. stated hit a bump in the haul road while driving the shuttle car to/from the miner causing EE to hit head on the canopy of the shuttle car. I.E. went to the hospital with a muscle strain in the neck.
I.E. stated was helping add belt for a belt move and stepped on a wet piece of belt causing I.E. to slip and fall hitting right ear on vulcanize table. Employee had stitches put in ear at a hospital.
I.E. stated EE was hanging 6"" pipe from a man basket, when the operator lowered the basket it rocked back smashing I.E. fingers between basket and mask.
The employee was in the process of getting out of a track-hoe and the employee's leg buckled causing employee to fall. The employee did land on feet but injured back during the accident. The employee continued to work until surgery on September 18, 2024.
I.E. stated was carrying 16 ft straps to roof bolter when I.E. felt a pop in right shoulder. I.E. has been waiting on a doctor's appointment since injury, I.E. went to doctor on 10/10/2023 and results of MRI was torn rotator cuff.
I.E., stated was putting up an 8ft bolt when I.E. felt a pop in left wrist. I.E. Worked until 9/19/2023 when a doctor on a follow up appointment took I.E. off work for P.T.
I.E. Stated that they were pulling shields when a piece of coal went behind their safety glasses sticking in their eye. I. E. went to the doctor the following day and had the piece of coal removed.
I.E. Stated that while putting up a cable bolt EE felt a pop in EE's shoulder.
Elevated C.O. levels were detected at 11XC and 5XC on the NM #1 belt. It was determined that the belt had been rubbing against the structural frame at the TG 3 transfer area causing smoke and elevated C.O. levels.
I.E. Stated as I.E. was replacing Y strainer cap on fan, a strap with a hook attached to the cap hooked I.E.'s left thumb causing severe pain.
Employee was collaring off for drill out when a rock fell from the shaft rib from approximately 15'and struck them in the back of the head just below their hard hat causing a laceration.
I. E. stated was hanging cable that was wet and had mud on outside making the cable slick. The cable slid into I.E. bending EE's thumb backwards. This injury is questionable and under review, employee has not complained of the injury and went to emergency room 7 1/2 months later. I.E. last day of work was 2/14/2024.
Employee has gone off work with a occupational illness, of Carpal tunnel.
I.E., stated EE was replacing a collapsed tube when I.E., head was pulled up to the tube due to the suction. I. E. went to the hospital after shift and received stitches.
I.E. complained of discomfort in right arm and shoulder. I.E. went off for surgery on 3/15/2024.
Employee was attempting to get an airline to stop leaking by twisting the fitting when the connection came free striking employee above left eye. The injury resulted in a laceration that required glue to close the wound.
I.E. stated while installing cable bolt into roof a T-3 plate slid on the bolt and cut employee on top of wrist. Employee received 4 stitches.
I.E. stated that I.E. had drilled the hole for a 12' cable bolt, and when I.E. reached out to grab the steels from the roof, felt a pop in right shoulder as I.E. was lowering the 3 steels.

IE was using a razor knife to cut urethane screen panels and the knife slipped as EE was using pressure to cut the panel and sliced their middle and right finger on the left hand.
An unplanned roof fall occurred on the North Mains #1 travel road entry #2 between xc 40 and 41. The roof fall was 11 feet wide and 15 feet long. This area of the mine was mined June of 2012.
Employee has tingling, numbness, and pain in both hands and wrist. Dr. has diagnosed as carpal tunnel.
I.E. stated they were checking dust in face fan 37, they had shut off hyd. to the duster raised lid and slipped with right hand getting wedged between agitator and dust hopper. I.E. re-gained footing and pulled hand out. Employee worked until 10/21/2022 then went to doctor for agitation and discomfort in right hand.
Employee is a roof bolter that started having progressive shoulder pain. Employee went to doctor, it was treated with shots. Employee has never missed work.
I.E. stated they had cut the phone line for the R/A when I.E. moved the scoop outby they went to move phone line out of their way sticking their left arm out of cab pinning the arm between cab and coal rib.
I.E. stated rocks fell from roof, first hit in the hard hat knocking it off and then hitting I.E. in the head after hard hat was knocked off
IE was disposing of used filters from the RO plant into a dumpster. IE lost EE's grip on a filter and still tried to throw it up into the dumpster then felt a pop in EE's left side rib cage area. IE was transported to McCleansboro ER. IE was examined and given three days off, then a full return to work release. IE did has returned to work.
I.E. stated mining to Head-gate a rock rolled out of the face landing on the shear drum tossing the rock over the pan line on to left ankle
I.E. Stated that they were lifting a 72"" bottom belt roller to put bracket on belt rail when they felt something pop in the right bicep area.
One of the four wire ropes that are associated with the main hoist failed, causing the hoist to be inoperable.
Witness stated employee was walking to truck when employee passed out falling with no njuries, witness and helper helped victim walk to man cage and went outside with victim. Dr. on site sent victim for blood work. Findings from blood work was that 2 prescription medication dropped blood pressure.
Employee was attempting to sheave hoses through stopping. Bolt broke that was holding 4 1/2 inch sheave allowing hoses to come back and hit I.E. in the left rib cage. During investigation it was found that I.E. was pulling off of permanent roof angle bolt.
IE was roof bolting in the North Mains 5 left entry. IE finished drilling EE's hole and put EE's glue stick in the hole. When IE went to put the bolt in the hole, IE slipped of EE's platform towards the atrs and fell to the ground. The platform was in the elevated position (approximately 5 to 5 1/2 feet) off the ground.
IE was bolting on the right side in the North Mains entry #4. IE was walking through the middle of the DBRB and was going to step up on the bolting platform. IE's foot slipped and struck the bolting platform. IE received a cut to the right leg (shin). IE received 8 sutures to close the cut.
IE was fixing drop down dust hose from the mars system. In the process of cutting the hose. IE's hand slipped and cut EE's right thigh. IE received sutures to close the wound.
I.E. was standing on the catwalk by the plant feed belt head while other employee moved the belt magnet which caused a rock bar to fall in the direction of I.E. The rock bar struck I.E. in the chest and caused an abrasion. I.E. did get a chest x-ray that was negative. I.E. was told to take a few days off, I.E. did miss three scheduled days and is presently back to work.
The IE was assisting another miner move a 100 pound acetylene bottle. The IE dropped the valve end on the bottle when the other miner picked up the opposite end. The bottle landed on the IE's foot. The IE did not receive any broken bones but was ordered to stay off work for 2 days.
The DBRB was being worked on by maintenance. Maintenance had an expanded metal guard lying next to the side of the bolter between the coal rib and ventilation tube. The IE walked by the tube and the expanded metal guard and got pant leg caught on the guard. IE lost balance and cut right leg (calf). IE received 6 sutures.
IE was disconnecting hydraulic hoses on a Sandvik scoop. During this process, IE brushed EE's right arm against the metal bucket causing a puncture cut to EE's right forearm. A piece of metal remained under the skin and treatment followed. The piece of metal (foreign object) was removed from arm on 3/10/2022. No sutures were given to close the cut.
IE was driving in a bit cone on the miner and a piece of (small) metal went through IE's glove and into the right thumb. First aid was given on this day and shift was finished. IE worked until 3/12/2022. On 3/14/2022 IE was taken off work and is scheduled to have metal taken out of thumb.
An excavator operator went to swing the boom of the excavator & as the operator swung the boom the bucket came into contact with an 18"" plastic pipe lying next to the excavator, this caused the pipe to move suddenly striking IE on the left side lower leg. IE was transported to a Hospital and had surgery to lower left leg that was fractured.

IE was in the process of staging material for a concrete stopping to be built. IE was carrying cinder blocks and metal framing to this location. When IE set material down, IE's middle finger on the left hand was smashed between metal framing and a cinder block. IE suffered a fracture and sutures to this finger. IE was taken off work on 12/22/2021
IE was talking to two other co-workers while turning to walk away and caught foot on a transformer slide causing IE to fall. IE landed on right knee, causing an abrasion swell. IE finished the shift and saw own doctor the next day who told IE to take off two weeks with a follow up exam.
The IE was using a lever hoist to pull a piece of casing into a borehole. As tension was being applied a weld broke on the hoisting lug causing it to come lose and strike the IE in the back of the head.
IE was working on the surface standing by a stack of drill rods watching the drill rig, a end-loader drove by the other end of the stack of drill rods and caught one of the drill rods spun the rod and struck IE in the back of the legs. IE x-rays were negative and is still under doctor care and receiving therapy.
IE was assisting with a power move on the North main section pull back from HG-12. IE was pulling cable slack under the belt running to the intake stopping. When IE pulled the cable, IE felt a sharp pain in IE's lower right stomach (Hernia). IE was taken off work on 8/16/2021.
On 08-13-2021, miners were cutting in the HG8 Long Wall number one entry, 25xc to 26xc area, a hot spot was created that could not be extinguished from underground. The mine is presently idle and the area atmosphere is being monitored from the surface and nitrogen is being pumped from the surface as well.
IE was dragging wire mesh across EE's canopy on a DBRB. While dragging wire mesh across the canopy a piece of debris flaked off and went into IE left eye. IE was wearing safety glasses at time of this incident. IE was given prescription eye drops.
I.E. was getting out of the cab of SM02 and caught EE's right foot between the seat and steering lever. I.E. was falling and used EE's right arm to catch self that is when EE felt a pop and immediate pain in EE's right shoulder. IE is waiting to see a specialist. This happen on the surface at the Akin portal, the conditions was dry and still daylight.
IE was in HG-9 working on a double boom roof bolter. IE was driving in a bearing on the pinner. When a piece of metal chipped off and went into the IE right forearm. IE had the piece of metal removed from arm on 9/3/2021 and was taken off work.
IE was throwing crib ties up on a square set for a false roof and lost footing falling into a pile of rib rash. IE's hard hat came off before forehead came into contact with the rib rash. The action resulted in a laceration that required eight staples. IE was working on the mine conveyor belt that was locked out and the top belt was no more than waist high.
At approximately 1:28pm an unplanned roof fall was discovered at the top of Slope B conveyor. Deterioration of the top caused the roof fall. The fall was approximately 8-10' of black shale for the length of 18'. A roof fall clean up and support plan was put in place.
I.E. was removing a damaged top roller from the take-up of the slope A belt, when I.E. cut the damaged roller loose it popped up and caught right index finger in between the roller frame and conveyor belt rail, causing a fracture to the finger. I.E. started missing work on 07/12/2021 after a follow up with an Orthopedic surgeon. The Doctor said the finger is healing good.
On 06/26/2021, an unplanned roof fall occurred on the HG9 travel way (primary escape way)at xc72, entry number 2. The fall was contained to the intersection and about 10 to 12 feet in height. The fall appeared to be black shell rock, and had not been glued.
Mine floor, roof and rib conditions were good. IE was picking up a piece of T channel to bolt it to the mine roof and twisted right forearm. IE finished the shift and during next scheduled shift said they were going to have the arm checked. The doctor said to take two days off to rest arm before returning to work.
IE was using Knipex bolt cutters to cut the bands on a bundle of hog panel. When IE cut the metal band it came back and came in contact with EE's right hand index finger. IE received a laceration that required two sutures.
IE was helping maintenance raise the headgate ranging arm into position to pin it into the shear. The AC shield hauler did not have enough lift to raise the ranging arm. Cribs were built on top of hauler forks. When the hauler raised the forks the cribs became unstable and popped out striking IE. IE received 3 sutures under the chin on the right side.
IE moved belt line car 1 crosscut inby. IE grazed opposite operator rib. Got out of car to check positioning. Turned wheels back to get off rib striking operator side rib. Hand was placed between two pieces of metal when rib was struck. Smashing middle and ring finger. IE returned to work and was than taken off work on 4/28/2021
IE pulled 2 tubs of longwall chain onto the pan line using a shield hauler. IE was in the process of pulling the 3rd tub on. A 1/2"" chain got caught in the tail sprocket binding the longwall chain. IE stepped out to see what was stuck. In the process, winch rope came undone, slinging foundry hook. Foundry hook ricocheted off chain tub than hit IE, breaking 2 bones in lower leg.
IE was removing a liner plate from a vessel in the prep plant, during this process IE right index finger came in between the liner plate and vessel frame and the plate snapped back into place smashing IE fingertip. The liner plate was still attached on one end contributing to the incident. IE right index finger had a fracture to the tip of the bone.
A diesel Bobcat Skid-steer was discovered on fire at cross cut 14, entry 2 of HG-6. The fire burned approx. 30 minutes and was extinguished with fire extinguishers and rock dust. Once water was established, it was applied to cool the Bobcat down and to wet the mine floor. All miners, other than those assisting with firefighting, were ordered to evacuate the mine by management.
Contract employee was trimming fingers for a vulcanized splice while using a razor knife, it slipped contacting their finger. Cause of injury was employee awareness and hand placement. Compliance with rules/reg, mining systems/equipment not a factor. Miner proficiency/adequate gloves may be a factor.
IE was cutting a lifeline cone with a utility knife to install on the lifeline. While cutting the lifeline cone, IE's hand slipped and IE cut their left hand (palm) with the utility knife. IE received sutures to close the laceration.
IE was troubleshooting #3 LW shield from #4 shield. While bumping functions a rock was dislodged from between the two shields and struck IE in the left side neck as IE was watching for a blown hose. This resulted in IE receiving 12 stitches to left side neck.
IE was working on HG-8. IE was in entry #1 at crosscut #91. Right before the accident, the IE was standing next to the miner man talking, with back against the rib. IE was struck by two pieces of shale. The first piece hit the hard hat knocking it to the ground, second piece struck the IE on top of head resulting in five staples. Rock measured 8"" long, 4"" wide, 0-3"" thick.
The IE was using a cutting torch to cut a piece of pin holding a jack in place on the tail of a CM. In the process of blowing the cutting material away, a piece of the metal got in EE's eye.
IE was at HG-7 crosscut #48. IE was using a utility knife to cut down a yellow rope to lower the cable. When IE was in the process of cutting down yellow rope, IE slipped and cut left forearm. IE received 5 sutures to the left forearm and returned to work.
A roof fall occurred on the HG9 travel road entry number 2 at 31 cross cut. The fall was approx. 12 feet high, 20 feet wide and 18 foot long.

IE was working from a latter while adjusting a vessel chain when they strained their back. IE continued to work finishing the shift, on 12/23/2020 the pain had increased so they reported it to the company and received medical attention. IE then started missing work for the injury on 12-28-2020.
IE was lifting a 12"" sweeping 90* fitting out of the back of the bed of a pickup truck with the tail gate lid closed. As IE was lifting the pipe bent over, IE's foot slipped off of the back of the bumper causing pain in lower back/leg. IE began receiving chiropractic treatment on lower back on 4/21/2022.
IE was performing maintenance working to a shield on the HG8 longwall. IE was climbing in and out of this shield changing hoses when felt a crack in EE's left knee. IE worked two more hours before reporting it, IE continued to work for a few months before seeking medical attention. Conditions in the area was good and level but wet from washing.
Accident happened on HG8 in entry 3 while roof bolting. IE had just started the screen wiring process. IE set the controls on a double boom bolter and stepped onto EE's platform. When IE stepped on the platform. IE felt a pop in EE's knee.
Employee was spreading 60"" structure for a belt move, went to throw a top roller off the trailer and felt a pop in lower back. Employee tried to finish the shift but decided to get back checked out by a medical facility.
Injured person was roof bolting when the drill steel clogged causing EE to lower the drill box to see if EE had suction at the chuck. When EE put a hand on the chuck the drill steel fell from the roof striking and fracturing left ring finger.
In the Prep Plant, clean dry floor and well lighted. *EE was changing springs with a chain fall. The chain fall came loose and smashed hand against the screen, cutting finger and thumb.*
Employee was driving company truck #69 heading to bottom. Driver and IE encountered dust for three crosscuts. Driver slowed down and went around examiner. Traveled half a crosscut and rear ended water car.

IE was taking a pump out of the company pick-up truck. When IE lifted the pump out of truck. IE right hand (middle finger) was in the way and was struck between pump and pick-up truck.
Pulling drive shaft from under a truck and securing it EE injured/stained right shoulder. EE continued to work with the injury until a doctor determined EE needed corrective surgery.
A crib block fell off the top of a bobcat and struck the employee on the foot and ankle.
Employee was using come A long to help put left drum on miner, when the hook slipped off striking employee in EE's left elbow.
An unplanned roof fall occurred in entry #2, xc5 HG8 set up rooms.
A rock fell while setting the ATRS, lowering the ATRS to get the rock off the screen more rock fell and a piece slid down the ATRS boom into the walk way.
Employee was standing on a rock while taking an air reading when EE lost EE's footing and fell to the ground on EE's wrist. Employee worked to 6/15/2020 then was put on light duty with restrictions
IE was picking up a 60"" bottom roller and felt a pop in IE's shoulder. IE continued working and alerted management 3 hours after incident. IE continued working the next 4 months. IE was taken off work on 9/24/2020.
Hook came off chain during belt move while tightening up come-A-long and belt fell and struck EE in neck. EE worked up until 5/11/2020 then went off for this injury.
IE picked up a bottom roller at the Main North 2 Drive and felt a sharp pain in right shoulder. IE has worked with no restrictions until 11/4/2020. At that time the IE was taken off work for right shoulder surgery.
Employee was stacking structure to banded, when it slid and pinched EE's middle finger.

There was an electrical failure on our secondary hoist. The proximity switch was malfunctioning, causing the hoist to raise and lower at a slower rate of speed. We replaced the proximity switch, then ""recycled"" the cage.
Employee was hanging cable in front of the feeder on HG7. After hanging the cable EE slipped and fell striking EE's back and left ribs. Employee went for medical treatment the next day, x-ray showed EE had received 4 fractured ribs.
Employee was using a track jack to raise the top belt to change a middle roller. As the belt raised it became harder to push the jack down. Employee felt pain in right shoulder. Employee worked until 2/27/20 at that time doctor gave a cortisone and took off of work until the following week.
Employee was in the set-up face on HG7 loading cable bolts on the bolter when they felt a 'pop' in the left shoulder. Incident happened on 1-9-20 and was reported to the Safety Dept. on 1-13-20. Employee went for medical treatment on 1-13-20

A roof pot out occurred at cross cut 120 in the number 1 entry of tailgate 2. The fallen roof material impeded passage through the area.
Employee was cleaning up trash. EE picked up a 50 gallon empty glue drum, EE accidently hit the valve on the end of the drum and releasing a small amount of pressure spraying glue on EE's face and running down into EE's eyes. Employee went for medical treatment that evening.
Employee was guiding a shield hauler in to pick up a re-lay bar on a shield. Employee told the hauler operator to ""tilt down"". After the hauler tilted down, the employee misplaced right foot, and it became mashed under the left fork of the hauler, fracturing right Big toe.
Employee was installing chain hangers in #1 entry on HG7. When EE raised EE's boom on the right side of the bolter, a 5ft steel became caught on the boom fan. When it dis-lodged it struck the employee on the right elbow causing a laceration requiring 5 stitches.
Employee was at the MN#1 drive, helping let out the take-up. EE was on the travel road side of the take-up, and had hand on the rail. When the movable roller started to roll, it caught EE's left ring finger between the roller and rail causing a laceration.
Employee backed miner out of cut, employee was throwing cable over and felt a pop in right shoulder. Employee went for medical attention the day of the incident with nothing found. Employee went to the Doctor the next day and could not return to work because of pain.
IE was handling refuse conveyor belt structure and IE felt like IE tore something in IE's elbow and shoulder. The IE begin missing work on 12/10/2019.
Employee was dragging a vent tube to the face, and twisted left knee on some loose coal. Employee went for medical treatment and was released to return to work. On 9-23-2019 employee returned to the doctor and had to have fluid removed from left knee, and became a lost time.
At aprox 7:30 employee started de-gasification and flames shot out of the pump.
Employee was setting bits on the shear with a bit wrench and 3 pound sledge hammer. A small piece of metal flew off the bit striking EE in the left arm. Employee went for a x-ray the next day.
Employee loaded a bundle of hogpanel on the roofbolter. Employee used a set of 18 inch bolt cutters to cut the bands holding the hogpanel together, and felt a ""pop' in left shoulder. Employee went to employee's doctor on 9/17/19 and the doctor is keeping employee off for obsevation.
Cooling package on a 279c Skid Steer fell down and pinned employee's right hand between the latch striker and the latch assembly. The gas strut was defective and came off its housing when it was bumped. Doctor determined employee had a fracture in the shaft of the #1 metacarpal on right hand.
There was an electrical failure on our secondary hoist. The main motor failed. The hoist was down from 9-8-2019 to 9/10/2019.
Employee was unloading 4ft bolts from the scoop. When EE grabbed a handful of bolts, a metal band was in the scoop bucket and flew up striking employees right arm causing a laceration requiring 5 stitches.
EE was pulling a curtain off of the mesh when a nail holding the curtain to the mesh came out striking EE in the mouth breaking the tooth.
Employee was splicing roofbolter cable, EE trimmed the cable back aprox. 10 inches and was holding the excess insulation with right hand, when EE was cutting off the excess insulation the knife slipped, cutting EE's right hand requiring 1 stitch.
There was an electrical failure on our secondary hoist. The contactor in the main panel failed. The hoist was down for aprox. 1 hour.
An unplanned roof fall occurred in HG-8 crosscut 66 in entry 2, 18'Wide x 20' long, and 18'high
A 3rd shift diesel mechanic was installing an exhaust cover on a Getman tractor. When they raised the cover to put it on the tractor, they felt a pop in the right shoulder. Incident happened on 7-19-19 and employee worked until 9-4-19.
Cutting smashed suction hose and was cutting towards myself and cut upper right leg above knee.
Employee was a contractor for 4 months and hired on 6-19-19. On 6-20-19 employee was building a 10ft crib on HG5 recovery. As EE walked away, the crib fell striking EE on the left shoulder and thumb. Employee worked until 6-24-19 and went to the doctor and was referred for a CT scan on 6-27-19, then referred for a MRI on 7-3-19 and diagnosed to need therapy for 3 weeks.
Employee knocked a stopping at 102 x-cut on HG6. After knocking the stopping, employee was scaling some block that was hung on the roof and a 4 inch block fell 8.5 feet striking employee on right foot fracturing the middle toe.
Building a block wall noticed lack of grip, numbness, pain, and tingling. Got worse with repetitive motion.
employee was on the North side of the cage bottom, the cage was on the surface, the employee opened the North side gate and walked 4.5 feet, (assuming the cage was on the Bottom) and fell 19.5 feet to the grading at the bottom of the shaft, landing on EE's right side. Employee received fractures to EE's back and left leg.
A rock slid off the top of the miner hitting me in the hand.
I was watering roads when I noticed a flame, I immediately set the fire suppression off and started using extinguisher and tried putting fire out, then was taken out.

Roof fall occurred on HG-7 belt line at crosscut 114 1 entry
IE was using a chain fall, chain hoist to lift a 4'x 8' sheet of 1/2 metal. The IE sprained or strained the middle left finger while ratcheting the chain fall. The IE continued unrestricted work until December 16, 2019.
Continuous overhead movement of arm, reaching for roof bolts and glue from repetitive motion
Shoveled along south mains belt line got EE's glove caught in roller and pinched EE's finger between belt and roller.
Employee was cutting a zip tie off a huck gun hose, slipped off cutting EE's left thumb receiving 6 stitches.
While changing a coupler on a 4 inch Aqua line, some emulsion sprayed into right eye.
A contractor employee was walking and pulling a rope. While pulling the rope EE tripped over a hose fracturing left ankle.
Cutting in the face of 1, sumping in felt something struck EE at the bottom of EE's left eye causing 4 stitches.
