Trained foreman was standing near #4 shield on the longwall face in 12 East supervising employees in clean up work. He was between the face and panline. The roof was roof bolted. A rib fell off the face hitting his knee. The roof control and protective equipment were compliant.

A trained longwall shearer operator was drilling the coal bottom floor in the 12 East longwall. The victim was struck by the rib falling and striking him in the knee. The roof control and protective equipment were compliant.

A trained employee was shoveling on the longwall face to clear coal bottom away. He had his back to the face. The roof from the shield tip to the face was roof bolted. The victim was struck on the left shoulder by stone and rock. The roof control and protective equipment were compliant.

He was bent over shoveling on the 7B Longwall Headgate area when a rock dislodged from the rib or roof and struck him in the back. He remained working without absence until medical evidence indicated a fracture of the L-4 Vertebrae. He began to miss work due to his injuries on 10-1-13.

He was walking to the miner when a piece of material became dislodged from the rib and struck him in the right ring finger. The result was two fractures and a laceration requiring two sutures to the right ring finger.

He was rib bolting on the right side of the 8B miner. A piece of stone rolled out of the rib and struck him on the front of his right knee causing a contusion. He began missing work on 4/25/13.

He was working near the rib next to the stage loader on the 7B Longwall section. The bottom part of the rib rolled out and struck him on the back of both legs. He complained of pain in his hips. He began missing work on 4/17/13 as a result of his accident.

Employee was struck in head with a piece of stone from the rib. Employee received a cut to his head requiring stitches and a hairline fracture to his C-2 vertebrae.

Rib rolled on the 6B longwall headgate area. The rib pushed him into the stage loader causing pain in his lower back. *We dispute the allegations in Citation #8051196 and submit this report solely to terminate the citation.

Employee was rib pinning on the right side of the miner in the section. He was drilling the first hole for his rib pin on the corner. The rib rolled out striking him on the right shoulder and behind the right ear.

While working on the tailgate ranging arm a piece of material fell out of the face causing the employee to fall striking his forehead on the shearer resulting in a laceration requiring sutures.

Employee was peeling a piece of rib with left hand when miner operator bumped the miner. His left index finger got caught between rib protector and rib.

EE was rib pinning, rib fell out and struck right middle finger against rib pinner.

Employee was hit by stone/material (cool) - that fell from rib.

A piece of stone approx. 1ft x 1ft 6 to 8in thick fell from the nb striking the EE in the neck/upper back area causing a fracture to the posterior spinal process of C6 and abrasions to the neck.

1 LEFT - 5 SOUTH LONGWALL AT PARTS CAR. RIB FELL AND PUSHED MESH AND POST AGAINST EE. HE HIT HIS MOUTH ON METAL COVER.

NEAR THE END OF THE SHIFT, EE WAS PUTTING HIS TOOLS AWAY BESIDE THE PARTS CAR. STONE AND COAL FELL FROM THE MINE RIB, HITTING HIS LEGS AGAINST THE METAL RAILING AT THE BOTTOM OF THE PARTS CAR.

2 LEFT - 5 SOUTH # 2 ENTRY 4+72. CREW HAD STOPPED MINING AT THE END OF THE SHIFT. MAN HAD DRUG UP ROCKDUST HOSE TO THE LEFT SIDE OF THE MINER TO BEGIN ROCKDUSTING. RIB FELL OUT KNOCKING HIM DOWN WITH ROCK AND COAL HITTING.

MAN WAS WALKING BETWEEN MINE RIB AND SHUTTLE CAR TO GET IN SHUTTLE CAR, RIB ROLLED OUT AND HIT HIS RIGHT SIDE WHICH SHOVED HIM AGAINST SHUTTLE CAR AFFECTING HIS LEFT SIDE. LOWER BACK AND SHOUL DERS.

1 LEFT- NORTH HEADGATE SECTION. MAN WAS MOVING OIL CANS SO THAT HE COULD BACK LOADER BESIDE FAN IN CROSSCUT. A LOOSE RIB ROLLED OUT AND HIT MAN ON RT. LEG.

ALONG #10 MAIN LINE BOLT-BETWEEN 29 & 30 X-CUT HIGH AREA, A LARGE ROB HAD FALLEN OUT. MEN HAD BEEN BOLTING AND MESHING IT. 85% OF THE AREA HAD BEEN DONE. EMPLOYEE WAS USING A SLATE BAR TO SCA LP A SMALL AREA WHEN A LARGE AREA FELL OUT HITTING HIM.

ALONG #10 MAIN LINE BELT- BETWEEN 29 AND 30 CROSSCUT. HIGH AREA PREVIOUSLY RIB OF STONE AND COALHAD FALLEN OUT. MEN WERE MESHING AND BOLTING BAD RIB. THEY WERE 85% FINISHED. EMPLOYEE WAS USING A SLATE BAR TO SCALE A SMALL AREA WHEN A LARGER AREA FELL OUT HITTING HIM.

EMPLOYEE HAD MOVED TO FACE SIDE OF LONGWALL FACECONVEYOR WHILE CHAIN AND FLIGHTS WERE BEING MOVED BY MEANS OF HYDRAULIC TENSION UNITS. A SLAB OFCOAL APPROX. 4' X 2' X 8" FELL OUT FROM THE FACE ONTO EMPLOYEE, PUSHING HIM INTO PANLINE. EMPLOYEE WAS SUB-CONTRACED TO LONG AIRDOX FOR REPAIR WORK TO FACE CONVEYOR.
