The employee with 5 years experience was turning a track switch by the handle when the switch throw stopped suddenly. He sprained his wrist.
Employee knelt to cut a rubber flap on a mantrip to attach a safety chain. He was using both hands when the knife slipped cutting his finger.
A roof was installing a 6' roof bolt on a full face miner. He was torqueing the bolt when a 2' wrench broke and hit him above the eye.
Employee was stepping off the longwall parts car and stepped a hose - Twisting his left ankle.
Tailgate of the longwall at 54+80 (#1 entry of 12 East) at roof fall was discovered at 2:10 pm. The fall was 40 feet long by 15 feet wide and 12 feet high. The section operated under the approved tailgate blockage plan. The area is now in the gob.
A fully trained maintenance helper had turned a shield off. He was standing on the shield that he believed had fully functioned. He started the shield and it lowered on him. Another employee raised the shield. The employee was trained as a shieldman.
Employee was standing beside a roll of inch and a quarter plastic slick line while it was being unrolled. The employee attempted to put marking tape on the end of the roll when it finished unrolling from a hanging support and the end of the plastic line struck his safety glasses and he received a laceration to his face at the right eyebrow. The employee received sutures.
Employee was backing out of the loader compartment when a piece of stone from in between roof straps struck him on the back.
A fully trained loader operator bent over to pick up the loader cable and felt a pop in his back.
IE was installing a piece of 12 gauge "V" deck panel in the steel sets that had been previously installed. A piece of rock came dislodged and was laying on the "V" deck panel pinching IE's right hand between decking and beam resulting in laceration and fracture to right ring finger
While attempting to abate a citation for mandoor distances at 10-17 between belt and old loaded track a roof fall was discovered between square sets and the stopping the fall was determined to be above anchorage. Support will be added to create access to terminate citation.
A fully trained miner was unloading an air drill used for bolting with the help from bolt partner upon lifting the drill he felt pain in his right shoulder and back he needed to have better body positioning while lifting he was wearing all required PPE
The Cameron elevator was taken out of service for a damaged sheave wheel There was no one entrapped we took it out of service and due to item 11 of immediate reportable events the elevator was reported
On 10/13/15 3 employees were stuck on the elevator at the Grapevine portal from 5:50 to 6:23 pm for a total of 33 minutes due to the door switch, the switch was repaired.
A fully trained miner was checking the suction on the bolter while the drill steel was in place the steel broke and struck him in the mouth the miner had on all required PPE but should not have had steel in place while checking suction
A fully trained miner was mudding a wall in 15 west the b-bond dripped inside of his gloves and irritated his right arm to his wrist the miner was not wearing the proper gloves for this job and was not wearing long sleeves either.
A fully trained miner was walking to the mantrip stepped in a hole at switch throw for a spur and tripped twisting his ankle. The area was well lit dry and level The miner needed to pay better attention to his surroundings and watch where he is walking.
A fully trained miner was adding oil to the miner when a rock fell from between the straps striking him in the neck shoulder and arm The area was properly bolted and he was wearing required PPE but he should have made a work place exam of the area before starting work.
A fully trained miner was changing a broken piece of monorail in the belt line in 13 east while removing the come-along a piece of rock fell from between the straps striking his R. wrist. He was wearing the correct PPE .
A roof fall was discovered at 6:10 on mainline 3 belt between X-cuts 3-01 and 3-02. The roof fall was above anchorage and was due to humidity and weathering of top. The fall measured 10' x 8'x 16'. This area was originally mined in 1968
A fully trained electrician was cutting tape off of a splice in a pager cable the knife slipped and cut his left ring and small finger resulting in 8 sutures. He was not wearing his gloves
A fully trained miner was wearing his metatarsal gloves and was loading 4" waterline onto a supply car. While loading the waterline his right ring finger got caught between the waterline and bulkhead of the car resulting in a fracture and laceration.
A trained loader operator was pulling miner cable slack in the #2 in 15 east he pulled miner cable stopped to move loader cable out of the way he grabbed it he felt a shock Area was dry his gloves were on after investigation 2 pin holes in the cable where he touched he never lost consciousness and walked to the powercenter and complained of numbness in his legs and EMS was called
A fully trained miner was moving up the snub for the shuttle car, the employee bent over to pick up the buggy cable and twisted his lower back. The area was dry and level all PPE was not a factor.
a fully trained miner was putting fire suppression pipe together at the take up . The employee was bent over facing the rib and a piece of stone 6"x2"x1" fell from the rib striking him in the neck and shoulder area. The employee never lost consciousness and PPE was not a factor.
A fully trained miner was repairing a omega block wall while putting a 2"x12" block on top of the wall fibers from the block fell in behind his safety glasses and getting into his left eye.
A fully trained was loading a automatic greaser into a wheel barrow the rear of the wheel barrow tilted back and sat down on top of the employees right foot. Employee was wearing leather metatarsal boots and completed his shift did not report to management until 9/9 2015. Employee did not pre-op the wheel barrow the wheel barrow had a flat tire.
on 9/2 a unplanned roof fall was discovered at 74 xc 8 to 9 entry at 9:10 am fell above anchorage due horizontal stress and humidity measuring 20'x15' and 10 feet high.
A full trained miner had replace the switch at 10 drive and was picking up old piece of 15'-20' rail to load on car set one end of rail on the car with help and the rail bounced off the car landing on the left foot of the miner . He was wearing leather metatarsal boots.
a roof fall was discovered at 14-19 in the 2 to 3 crosscut belt to track entry measuring 20x16 and 10 feet high
A fully trained miner was lifting a 10 ton jack that weighs approximately 25 lbs. out of a box to put on a mantrip and strained his lower back. The area was dry and flat level surface .Employee did not use proper lifting technique proper PPE was not a factor.
EE was cleaning the JSeal pump at end of shift and removed the grating of the hopper. Right hand came in contact with a moving auger. EE suffered traumatic amputation of middle and ring fingers on right hand.
A fully trained employee was riding in a 5 man mantrip leaving 77 wall going inby on 2 track when he traveled past 80 wall he jumped the switch and struck another ride parked in the spur resulting in a strained back PPE was not a factor.
A trained foreman was sitting in a ride in a switch with his lights on when the ride was struck by another ride because the driver said he could not see the ride due to the height of the canopy on his ride. The collision resulted in his back being hurt, and was caused by poor communication and the poor visibility. Lack of PPE was not a factor. Area was well lit.
Fully trained miner was loading belt stands onto a flat car in the 14 east section. The employee caught R middle finger causing a laceration he was wearing the proper gloves and required PPE. The area was dry and level and accident was due to poor hand placement and concentration.
A fully trained miner was rock dusting the belt line in 15 W. while dragging the hose to the belt he stumbled catching himself against the rib with his L hand. The employee stated he woke up and hand was numb. Employee had on required PPE. The employee did not report injury to Management till 8/28/15 and did not start losing time till 8/30/15.
A fully trained miner was clearing out gob from the head gate in 13 east at #4 shield and was removing a piece of metal from the walkway of pan line. While he was bent over a 2'x2'x6" stone came out of the pan while clearing the chain landing on his right foot causing a contusion. He was wearing Rubber metatarsal boots at the time. The pan was not in motion
While bolting with a fletcher twin boom bolter in the #9 entry on the rt side of the mains, at the overcast area a piece of draw rock measuring 1'x1'x6" fell from approximately 10' between the canopy and the ATRS striking the miner on the top of his head. The miner continued to work until around 1:00 pm and then came out of the mine and was transported to Wheeling Hospital
Employee lifted a roll of water stop and felt pain in back.
While setting bits into the insert of the CM a fully trained miner swung his hammer to set the bit and slipped striking his right hand between the bit and the hammer. The miner finished his shift and worked the next 2 shifts also and reported this to management on 8/20/15 and took himself to wheeling hospital on 8/21/2015.
A miner while carrying a Z-drill down the 5n 2 belt. While carrying the drill through the steel sets at 40 wall he twisted and felt a muscle pull in his lower back.
