While putting a crib block under the continuous miner, the corner rolled striking the employee pushing their face into the side of the continuous miner causing swelling around the left eye. There was no treatment provided at initial doctor visit but they returned to the doctor on Monday 7/21/25 and was treated with prescription eye drops. We were notified of treatment on 7/28/25.

Employee was bolting in #7 entry about 3 rows inby corner. While he was swinging pod out to bolt the corner, rib rolled out striking employee's left ankle. (2 bones fractured) Rib was 78" long x 39" wide x 32" thick. Rib was behind curtain. Employee was treated at Raleigh General Hospital.
Employee was pulling cables by a parked shuttle car when a coal rib fell hitting him in his chest causing him to fall against the rub rail on the shuttle car resulting in a deep bruise on his left thigh bone. Employee was transported to Appalachian Regional Hospital where he was admitted for observation.
Employee was bolting in 6 right when a piece of rock approximately 24" x 3" x 2" fell from the rib striking employee on the lower back. Employee was treated by EMT's at the scene and transported by Whitesville Ambulance to CAMC General Division. Employee had xrays taken which was negative but was referred to a spine specialist for further treatment.
Employee was cleaning 5 entry with a scoop. As he was cleaning the right rib piece of line curtain fell. He went to re-hang the line curtain a piece of rock fell out of the rib pinned right foot. The rock was approx 72x32x16. He was transported to CAMC and was diagnosed with 6 fractures in his right foot. He was given instructions to follow up with an orthopedic surgeon.
Employee was building a stopping in #6R across from feeder. A piece of rock broke loose from the rib striking employee on the right shoulder and right hand. Employee was transported to Raleigh General Hospital. Xrays were taken and was told to follow up with a specialist for further evaluation.
Member was in the process of packing up material beside the section power in preparation to pull the power center back for pillaring, when a small pice of coal rib fell on the member's left leg.

He was taken to hospital where X-ray was normal. He was referred to follow up w/specialist and MRI. Contacted Safey Dept 9/4/09. MRI on 9/2 showed ruptured discs/1 bulging disc.
Employee was shoveling at a belt tailpiece when a chunk of coal rolled off the rib and struck his left knee area.
Member was pulling a loose piece of rib when it fell. He put his hand up to keep it from hitting him in the face. A sharp rock cut his hand causing a laceration that required 3 stitches to repair.
While tramming continuous miner into #4 entry the operator stopped miner to take slack rope off of cable, while doing so a loose piece of the rib rolled out and struck the miners right pinky finger causing a laceration that required 3 stitches.
While attempting to start roof bolting, the 15' cut in #5 crosscut to the left, employee noticed that the right rib was loose. As he started to get a slate bar, the rib fell on his right foot, fracturing it. The rib was 18' long and 18" to 5" thick.
EE was attempting to pull slack on the feeder cable to allow the section electrician to connect it to the breaker. A rock Measuring 52" long X 16" wide X 7" thick fell striking him on right ankle. He was then transported to CMAC General where x-rays showed a fractured ankle.
EE WAS HANGING A CURTAIN IN A CROSSCUT TO THE LEFT OF THE BELT ENTRY WHEN A PIECE OF ROCK FELL OUT OF THE RIB, STRIKING HIM ON THE RIGHT KNEE CAUSING A SPRAIN. HE CONTINUED TO WORK UNTIL 9-8-0 3.
EE WAS PULLING A RIB ALONG THE SCOOP ROADWAY IN AN OUTBY AREA WHEN A PIECE OF ROCK MEASURING 2' LONG X 4" THICK FELL, STRIKING HIM ON THE LOWER PAR OF HIS LEFT LEG. THE EXACT LOCATION OF THE A CCIDENT WAS BETWEEN SPAD #366 AND 382.
EE WAS WALKING BY THE TAILPIECE LOCATED AT THE HEAD GATE BOX WHEN A RIB ROLLED OUT STRIKING HIS LEFT SIDE-(NECK, BACK AND LEG), FORCING HIM TO THE MINE FLOOR.
A RIB ROLL OCCURRED WHILE EMPLOYEE WAS SHOVELING BELT.
MEMBER WAS BOLTING THE 2ND CUT OUT OF 2 HEADING WHEN A RIB ROLLED OUT AND HIT HIM ON HIS LEFT SIDE CAUSING HIM TO BREAK HIS LEFT ANKLE AND 3 METATARCEL BONEN IN HIS FOOT, RIB ROLL MEASURED 2'X 2'X10'.
