Operator finished the cut and was tramming to the next location when a piece of draw rock fell striking the operator on the left arm causing a laceration that required 5 sutures.

While setting the feeder down onto 2 crib-blocks, the top crib-block slid out and fell onto the left foot causing a bruise and a fractured toe.

While installing a cable bolt, the roof bolt plate came in contact with their forearm causing a laceration that required 4 staples

While building a stopping employee pulled a solid block toward them and felt a pop in left bicep. EE was diagnosed with a ruptured bicep tendon. EE has worked every scheduled day since the incident on 7/31 but is having surgery on 11/21/2025 to repair the tendon and will be off work from the day of the surgery until fully recovered.

A non-injury roof fall occurred on the No. 3 Section; No. 2 Entry approximately 10 feet outby the last open break, at spad 8233. Fall measured approximately 80 feet long x 20 feet wide x 6 feet high.

The employee got dust particles in left eye while striking a bearing plate covered in dirt with a hammer. The dust caused a scratch on the surface of eye that was treated with prescription eye drops. The employee returned to work on next scheduled shift and has not missed any days.

While working in the intake, the air current blew the miners hard hat off. While turning head to catch the hat they hit their head on the mine roof causing a laceration that was treated with 7 staples.

While installing a roof bolt a piece of rock 1ft x 1ft x 1/4inch fell from the top striking the employee on the left forearm resulting in a laceration that required 9 stitches.

While installing roof bolts the employee reached back to get a bolt out of EE's tray and tripped and fell catching self with EE's left hand and fractured EE's wrist.

Employee was pulling a string off a roller and caught a finger between the roller and the roller hanger causing a laceration that required 7 stitches.

Employee was lapping up belt, when employee slipped and fell, catching self with left hand, resulting in a fractured wrist.

While putting a crib block under the continuous miner, the corner rolled striking the employee pushing their face into the side of the continuous miner causing swelling around the left eye. There was no treatment provided at initial doctor visit but they returned to the doctor on Monday 7/21/25 and was treated with prescription eye drops. We were notified of treatment on 7/28/25.

While inserting a roof bolt into the drilled hole in the top, a piece of loose rock aprox. 16""x8""x1/2"" fell from the top hitting the injured on the right forearm causing a small laceration requiring 3 stitches.

While hanging discharge line, the injured miner bent down under the water line. While bent over the eye on the bolt plate holding the line broke, dropping the water line and hitting the injured miner on the back before the line was caught by another chain that was supporting the water line.

While loading bolts in a scoop bucket, employee fell on left shoulder and felt a pop in the shoulder.

A non-injury roof fall was discovered on 1B Belt at No. 4 Break in the Track Entry. Fall measured approximately 30 feet long x 20 feet wide x 7 feet high.

A non-injury roof fall was discovered on the No. 1B Belt; Track Entry; No. 3 Break, near spad 3235. Fall measured approximately 70 feet long x 19 feet wide x 8 feet high.

While standing one step from the top of a 6' step ladder EE was placing a block in the row approximately 9' high. While reaching to place the block EE's ladder tipped over causing EE to fall to the mine floor and hit EE's back causing back pain.

While removing drill steel from hole a piece of rock measuring approximately 2 feet wide x 3 feet long x 6 inches thick fell and hit canopy and rolled down the ATRS striking employee on the right arm resulting in a 6 inch laceration. Required 7 stitches.

While repairing a hydraulic hose, the wrench slipped causing employee's elbow to strike a bundle of roof bolt plates, resulted in a laceration requiring 6 stitches.

A non -injury roof fall occurred on the No. 3D Belt at No. 17 break just inby spad 5313. The roof fall measured approximately 15 feet long x 30 feet wide x 5-6 feet high.

Miner tripped and fell into the continuous mining machine striking face and mouth against the machine. Resulted in a fractured tooth which was repaired.

Miner was diagnosed with pneumoconiosis.

Employee was involved in horseplay which caused injury to the lower back.

While operating roof bolting machine, miner struck a roof bolt plate lying in the supply tray with left forearm. Resulted in a laceration requiring 5 stitches.

While bolting in the No. 5 Entry, on the No. 3 Section, rock fell from mine roof pinning the miner's leg against the frame of the DDR bolter. Resulted in fractures to the lower leg.

Employee was prepping to shear the fingers of a finger splice. EE started to shear the fingers with an oscillating tool and slipped with the shears causing the shears to strike right pinky finger.

Miner has been diagnosed with pnuemoconiosis.

Employee was setting a Heitzman jack when a piece of roof rock fell measuring 3 feet x 2 feet x 8 inches thick. The rock hit employee's right foot.

In the process of dismantling a scaffold an 8' piece of lightweight aluminum scaffold support beam fell a distance of about 5' striking the employee of the right shoulder.

Rib rolled out beside of track. Miner was riding personnel carrier with feet on edge of frame. Rib roll caught left foot between rock and frame resulting in a fracture to several metatarsal bones in left foot.

Miner has been diagnosed with pnuemoconiosis.

While attempting to remove plywood contractor employee was hit in the head with a small steel beam that was lying on the plywood.

While using a set of chain ratchets to position the tailpiece during a belt move, the chain ratchet released from the anchor point striking the injured in the mouth dislodging front upper tooth.

A non-injury roof fall was discovered on the No. 4 Belt between 2 and 3 cross cut in the belt entry located approximately 36 feet outby spad No. 3824. Fall measured approximately 15 feet long x 12 feet wide x 4 feet high.

Non-injury roof fall occurred on the No. 3 Section in the No. 5 Entry 1 break outby spad No. 4283. Fall measured approximately 30 feet long x 18 feet wide x 8 feet high.

Employee was diagnosed with pneumoconiosis.

Employee was handling a piece of sheet metal 5 feet x 10 feet x 1/4 inch thick and strained their back.

While nailing curtain to rib, some material hit the injured in the right eye over top of EE's safety glasses.

While installing roof bolts, rock fell from mine roof striking ATRS and then struck the injured on the left shoulder, sliding down injured back and striking right foot.

While placing crib blocks under the continuous mining machine, one of the cribs unexpectedly shifted causing the machine to shift, smashing right middle finger. Resulted in a fracture and laceration requiring stitches.

Employee was utilizing a 1/2 inch chain connected to a service scoop to pull slack out of a feeder conveyor chain. The 1/2 inch chain broke striking employee in the face. Resulted in a fracture, stitches, and a chipped tooth.

While waking, injured stumbled over loose rock laying on the the mine floor and cut palm of hand on sharp piece of rock. Resulted in a laceration requiring 12 stitches.

While operating a scoop, injured ran over a rib roll, jarring back and neck in the operator's compartment of the scoop.

Employee was diagnosed with pneumoconiosis.

Injured was handling a shuttle car trailing cable and felt pain in back.

Employee was handling a worn skirt board from a conveyor tailpiece. Resulted in a laceration requiring 6 stitches.

Non-injury roof fall occurred at spad 3193 in the 1C Belt Entry at break 7. Fall measured approximately 40 feet wide X 20 feet wide X 8 feet high. All entrances to the fall area were timbered and a clean up plan was submitted to MSHA for review.

Hand was on canopy when rock fell from mine roof smashing middle finger on left hand. Resulted in a chipped bone on tip of finger.

While constructing a stopping a block fell from the top row, which was approximately 10 feet high, and smashed right thumb. Resulted in a fracture.

Employee was using a bumper jack to adjust the conveyor tailpiece, when the jack fell over striking the employee on the left hand resulting in a fracture. The operator determined the incident to be reportable on 07/06/2022.

Employee was beating a pie pan back into place against the mine roof and struck wrist on another roof bolt plate causing a laceration resulting in 6 stitches.

Employee was removing rock from top of continuous miner and pinched left ring finger between rock and machine. Resulted in a fracture.

While loading a gear case for a continuous miner onto a flat car underground, the gear case slid off of the scoop bucket causing the scoop to bounce. Employee hit EE's head on the canopy inside the operators deck.

While roof bolting, a small piece of roof rock dislodged near the drill steel and fell smashing employee's left hand against the drill pot of the machine. Resulted in a fractured middle finger.

Employee was operating a rail mounted personnel carrier underground when it collided with an 15 ton locomotive near No. 5 Head.

A roof fall was discovered on the No. 2A Belt Conveyor near the No. 2 Section in the No. 3 Entry. The fall was two breaks inby spad 2457. The fall measured approximately 30 feet long X 20 feet wide X 6 feet high. A plan was submitted to MSHA for clean up and removal of the fall.

After crossing belt to update escapeway map, miner stepped in a hole causing a twisted ankle.

Miner was attempting to reposition a continuous miner head being hauled on the track using multiple jacks when one of the jacks slipped pinching the tip of EE's left ring finger between another jack. Resulted in amputation down to first knuckle. (60 days).

A non-injury roof fall was discovered on the No. 5 Belt Conveyor in the No. 19 cross cut, spad 1337. Fall measured approximately 50-60 feet in length, 20 feet in width and approximately 6 feet high.

While replacing a shield on the section feeder, employee pinched right pinky finger between the shield and frame of machine. Resulted in a fracture and six (6) sutures.

A roof fall was discovered in the track entry adjacent to the No. 3 Conveyor Belt, near the No. 8 cross-cut at spad no. 445. Fall was approximately 20' long x 20' wide x 5.5' high. No injuries reported.

Miner was closing air-lock doors when head became pinched between the doors resulting in a laceration.

While installing guarding on the No. 2B Belt Head, employee slipped and fell on I-beam at base of take up.

Employee was plastering stopping using B-Bond. Was Wearing rubber gloves that fit up to elbows. Several days later employee developed rash due to exposure to B-Bond. Determined to be Occupational Illness on 08/13/2021.

Draw rock fell striking miner on right hand causing a laceration.

Injured miner was working to replace the foot jack on the ATRS system of the Roof Ranger II roof bolting machine when the ATRS system unexpectedly moved. The injured miner's head was caught in the system resulting in head trauma.

Slipped and fell, causing a laceration on left hand. Required 8 stitches.

Employee slipped on piece of conveyor belt outside on the mine yard. Employee continued to work. Was diagnosed on 02/06/2021 with fractured rib.

Employee was at inch tram controls on roof bolter when a piece of roof rock struck employee in the right arm causing a laceration, resulting in multiple sutures.

Rib rock fell striking miner on the back. Resulted in a fractured rib.

Discovered roof fall 1 cross cut inby survey station 603. Length-20 feet; Width 18-20 feet; Height 5-6 feet. No injuries.

While carrying a roll of ventilation curtain on right shoulder, employee's right hand struck a roof pan lacerating right hand. Resulted in seven (7) stitches.

While setting up roof bolting machine in the No. 4 Right crosscut, a rock fell from the mine roof striking the employee on the arm. Resulting in a laceration requiring 3 stitches.

Employee was dropping miner cable when EE's right hand was caught by the hook on the rope hanger, lacerating EE's hand above the palm. Resulted in 10 sutures.

While waiting for scoop to stop in order to unhook pull rope from bucket, the employee slipped off a ledge and was caught between the ledge and scoop bucket.

Miner tripped on gravel and a cable and fell into a piece of tin lacerating face and neck. Received stitches.
