Operator finished the cut and was tramming to the next location when a piece of draw rock fell striking the operator on the left arm causing a laceration that required 5 sutures.
While setting the feeder down onto 2 crib-blocks, the top crib-block slid out and fell onto the left foot causing a bruise and a fractured toe.
While installing a cable bolt, the roof bolt plate came in contact with their forearm causing a laceration that required 4 staples
While building a stopping employee pulled a solid block toward them and felt a pop in left bicep. EE was diagnosed with a ruptured bicep tendon. EE has worked every scheduled day since the incident on 7/31 but is having surgery on 11/21/2025 to repair the tendon and will be off work from the day of the surgery until fully recovered.
A non-injury roof fall occurred on the No. 3 Section; No. 2 Entry approximately 10 feet outby the last open break, at spad 8233. Fall measured approximately 80 feet long x 20 feet wide x 6 feet high.
The employee got dust particles in left eye while striking a bearing plate covered in dirt with a hammer. The dust caused a scratch on the surface of eye that was treated with prescription eye drops. The employee returned to work on next scheduled shift and has not missed any days.
While working in the intake, the air current blew the miners hard hat off. While turning head to catch the hat they hit their head on the mine roof causing a laceration that was treated with 7 staples.

There was no treatment provided at initial doctor visit but they returned to the doctor on Monday 7/21/25 and was treated with prescription eye drops. We were notified of treatment on 7/28/25.

While inserting a roof bolt into the drilled hole in the top, a piece of loose rock aprox. 16""x8""x1/2"" fell from the top hitting the injured on the right forearm causing a small laceration requiring 3 stitches.
While hanging discharge line, the injured miner bent down under the water line. While bent over the eye on the bolt plate holding the line broke, dropping the water line and hitting the injured miner on the back before the line was caught by another chain that was supporting the water line.
While loading bolts in a scoop bucket, employee fell on left shoulder and felt a pop in the shoulder.
A non-injury roof fall was discovered on 1B Belt at No. 4 Break in the Track Entry. Fall measured approximately 30 feet long x 20 feet wide x 7 feet high.
A non-injury roof fall was discovered on the No. 1B Belt; Track Entry; No. 3 Break, near spad 3235. Fall measured approximately 70 feet long x 19 feet wide x 8 feet high.
While standing one step from the top of a 6' step ladder EE was placing a block in the row approximately 9' high. While reaching to place the block EE's ladder tipped over causing EE to fall to the mine floor and hit EE's back causing back pain.
While removing drill steel from hole a piece of rock measuring approximately 2 feet wide x 3 feet long x 6 inches thick fell and hit canopy and rolled down the ATRS striking employee on the right arm resulting in a 6 inch laceration. Required 7 stitches.
While repairing a hydraulic hose, the wrench slipped causing employee's elbow to strike a bundle of roof bolt plates, resulted in a laceration requiring 6 stitches.

Employee was prepping to shear the fingers of a finger splice. EE started to shear the fingers with an oscillating tool and slipped with the shears causing the shears to strike right pinky finger.

Employee was utilizing a 1/2 inch chain connected to a service scoop to pull slack out of a feeder conveyor chain. The 1/2 inch chain broke striking employee in the face. Resulted in a fracture, stitches, and a chipped tooth.
While waking, injured stumbled over loose rock laying on the the mine floor and cut palm of hand on sharp piece of rock. Resulted in a laceration requiring 12 stitches.

A roof fall was discovered on the No. 2A Belt Conveyor near the No. 2 Section in the No. 3 Entry. The fall was two breaks inby spad 2457. The fall measured approximately 30 feet long X 20 feet wide X 6 feet high. A plan was submitted to MSHA for clean up and removal of the fall.
After crossing belt to update escapeway map, miner stepped in a hole causing a twisted ankle.
Miner was attempting to reposition a continuous miner head being hauled on the track using multiple jacks when one of the jacks slipped pinching the tip of EE's left ring finger between another jack. Resulted in amputation down to first knuckle. (60 days).
A non-injury roof fall was discovered on the No. 5 Belt Conveyor in the No. 19 cross cut, spad 1337. Fall measured approximately 50-60 feet in length, 20 feet in width and approximately 6 feet high.
While replacing a shield on the section feeder, employee pinched right pinky finger between the shield and frame of machine. Resulted in a fracture and six (6) sutures.
A roof fall was discovered in the track entry adjacent to the No. 3 Conveyor Belt, near the No. 8 cross-cut at spad no. 445. Fall was approximately 20' long x 20' wide x 5.5' high. No injuries reported.

Injured miner was working to replace the foot jack on the ATRS system of the Roof Ranger II roof bolting machine when the ATRS system unexpectedly moved. The injured miner's head was caught in the system resulting in head trauma.

Employee was dropping miner cable when EE's right hand was caught by the hook on the rope hanger, lacerating EE's hand above the palm. Resulted in 10 sutures.
While waiting for scoop to stop in order to unhook pull rope from bucket, the employee slipped off a ledge and was caught between the ledge and scoop bucket.
