Standing along the right side of the continuous miner preparing to install roof bolts, when a piece of slate fell from the roof and struck his hard hat, and then struck him on the right side of the top upper lip causing a laceration requiring one suture.

Using a hilti gun to hang canvas, a piece of rock fell from the strap and struck him on the left side of his head. He continued to work at his regular duties. He went to a physician and was taken off work on 8/8/08 due to a disc herniation.
