The #8 elevator was out of service for a period of time exceeding 30 minutes due to the govenor overspeed switch being ajar. No individuals were on the elevator at the time. The overspeed switch was reset, the elevator examined and placed back into service at 4:15 pm.
EE was taking a 3 wheel battery golf cart to the section when a chain fell from the oper. comp. Without thinking or stopping the ride, He reached out with his L. hand to retrieve the chain, ride went thru a low place in the roadway causing his L. middle finger on his L. hand to be caught between the ride & the mine floor. He broke his finger and 6 stitches were needed to close.
An unintentional roof fall was discovered by a mine examiner making a weekly examination. The roof fall was approximately 8' high x 16' wide x 50' long and occured at the 106 crosscut in the #1 entry of north mains.
The injured tripped over the anchor chains for the belt tailpiece and fell against a belt stand and injured his right shoulder. The injured continued to work till 8/6/09 when he went to the doctor and was diagnosed with a torn rotator cuff. His 1st day off work was 8/7/09
He was repairing bridges and installing stone in tramway, he twisted his left knee, causing a left MCL strain and derangemant.
A roof fall occurred in the #7 entry 250 feet inby spad #18010. the fall was about 20 ft wide, 40 ft long, and 8 foot high. The area was bolted with 5 foot torque tenison bolts. The fall was crib and danger off, and will not be clean up.
Attempting to lift an 80 lb. bag of gunite when employee felt a sharp pain in his lower back.
Employee was placing a bench hammer in a shaft bucket and smashed finger. Laceration required stitches.
While employee was hanging miner cable he felt sharp pain in his lower back.
Injured was using a utility knife to strip wire and the knife slipped. He lacerated his right thumb. The laceration required four sutures to close.
He was removing the miner cable from the cable hangers when a piece of rock fell from the top of a strap and struck him on the left middle finger causing a laceration requiring 4 sutures.
While he was building a stopping he was getting a block off the pallet and one fell off hitting his right thumb. (He returned to work on his next shift 7-20-09. This was a medical report only.)
The #8 elevator was out of service for a period of time exceeding 15 minutes during an electrical storm when two fuses blew. Two individuals were on the elevator at the time. The fuses were replaced & the individuals removed from the elevator @ 12:40 PM. The elevator was placed back into service @ 12:50 PM. No injuries were reported.
He was attempting to throw a set of steps over a hand rail, the steps came back and struck him on the right wrist, causing a laceration that required 8 sutures.
He was using a razor knife to make a hole in the skirt rubber for a bolt, when the knife slipped striking him on the right thumb, causing a laceration requiring 5 sutures.
Injured was operating a 5 ton motor. He states he was jostled in the motor and his foot slipped off the toe board in the operator's compartment and struck a rock. He sustained a fracture to his left ankle.

Injured and co-worker were unloading 8 inch aluminum pipe from a scoop. As the co-worker picked an end of the pipe up injured's left middle finger was caught between the pipe and scoop smashing the finger tip. He sustained a left distal phalanx fracture and began losing time from work on 07/02/09.
Employee was exiting a truck when he slipped on the running board causing a meniscus tear to the right. ***This is a reoccurance***
Employee began missing work due to a work related illness to his right shoulder. Degenerative changes were noted in the acromioclavicular joint of his right shoulder in the impression noted after an MRI was conducted. He was taken off work by a physician.
Washing down sump with a water hose while standing on a platform. Slipped and fell approx 18" striking back on a crib block and handrail.
Employee began missing time for a work related condition when surgery was required to his right knee. MRI findings showed damage due to changes from trauma. He has been employed for over 31 years as a supervisor in underground mines. The condition was noted after he felt pain from walking.
While cutting cable tray with a battery powered sawsall, the individual's left hand slipped past the tool grip, crushing his finger in the mechanism driving the saw blade. Stitches were required.
Injured began losing time when he was diagnosed with a torn left rotator cuff tear and surgery was necessary to repair it. He is an underground mechanic and the repetitive motion necessary to perform tasks for this job contributed to it. He began missing time on 7/27/09.

Four men were hanging miner, loader, bolter cables and 1 1/2" hose, when he felt pain in left shoulder. He continued to work at his regular duties, but also continued to have pain. He eventually was taken off work for left shoulder surgery on 11/13/2009.
Walking on toes of longwall face shields when foot slipped and fell between toes and relay bar. He suffered a fractured fibula.
He stepped from a wooden plank along the stage loader onto a crib that was placed under #1 shield and slipped, causing him to twist his right ankle, creating a right ankle sprain.
Employee was assisting with threading a drill steel and got his hand wrapped up in the steel.
He was doing permissability on the disconnects on the tight side of the stageloader. The rib rolled out and he tripped and fell striking his head against a tailpiece anchor abraision and cervical strain.
The elevator at the Camp Creek portal was down for more than 30 minutes due to a bad mid-shaft switch. The switch was replaced. Persons could travel the slope by slope car or walk.
The #8 elevator failed to leave the top landing. The elevator displayed contactor fault. Upon investigation a faulty coil on the M contactor was found. The coil on the contactor was replaced and the elevator placed back into service @ 9:15 AM. No individuals were on the elevator at the time.
While using a floor mounted grinder to smooth up a cut on a thin piece of metal the object and the employees right hand were pulled into the wire brush wheel. The table and guards were all in place and properly adjusted. The employee sustained an amputation on the tip of the little finger and a laceration on the ring finger which required 12 stitches to close.
The elevator serving #6 Portal was unavailable due to a malfunction in the door lock. The door lock was repaired and the elevator was put back in service at 1:30am on 6/2/2009.
EE was sitting on a stopped scoop when the operator inadvertantly moved the bucket down his left hand was caught between the frame of the scoop and the ram causing a crushing injury to the left hand and amputation of left middle finger.
While the employee was putting belt structure in on a belt move, he felt something pull in his right groin area.** Employee started missing work on 6/3/09.
He was tying a rope to the loader cable to pull it up when his foot became stuck in the mud. When he attempted to free his foot he caused a strain to the left buttock hamstring.
He was getting into a pick up truck when his foot slipped on the rocker panel causing him to twist his right knee. He continued to work at his regular duties until 6/16/09 when he was removed from work for a medical miniscus tear of the right knee. (EE started losing time again on 1/14/10 due to increased pain and pending surgery.)
During a routine inspection the governor for the #6 elevator was found to be defective. The elevtor was down from 12:55 p.m. to replace the governor.
He was using a hammer and punch to drive out a pin on a cat pad of loading machine when he struck his right index finger with the hammer causing a 2.5 centimeter laceration requiring one continous suture.
Conditions unknown. Minor right eye irritation.
EE was hanging miner cable while on his knees, when the Fairchild 35-C scoop backed around the corner and ran upon his left metatarsal boot.
Tried to throw a rope with weight on the end of it down to a poly pipe in a coal recovery pit.
Injured was struck on the left knee when a jack used to bend rails slipped off the rail being bent and struck him.
Carrying supplies to the seal, when EE hit his head on a roof bolt. EE said he re-injured an old injury from when he worked at Bailey Mine for Consol Energy.
The slope hoist was taken out of service due to two kinks in the rope. 260 feet of rope was removed and the rope reattached to the slope car. The car was returned to service at 5:58 pm. No escapeways were affected and no individuals were on the car at the time.
Employee began missing work on 5/1/09 for what was originally thought to be a personal injury. After further review of tests & medical evidence it was concluded the condition was caused by repeated trauma sustained from work related activities. He has worked various jobs in the mine for over 30 years and suffered a clavicle strain that required surgery.
Injured had walked the escapeway from the section when he began to experience pain in his right knee and sought medical attention the next day. When he was seen on 5/4/09 it was discovered he had degeneration to his knee. He began missing time on 5/4/09.
Crossing overcast thru 32x32 door & into return entry with 250' 3" poly line. Poly wedged up, freed up, wedged up again causing injury/pain to employee hip from pulling the line.
Lifting a piece of 80# rail that had a guard rail attached to it with a co-worker. He strained his lower back. Injured was off work from 4/30/09 until 5/6/09.
