The #8 elevator was out of service for a period of time exceeding 30 minutes due to the govenor overspeed switch being ajar. No individuals were on the elevator at the time. The overspeed switch was reset, the elevator examined and placed back into service at 4:15 pm.

EE was taking a 3 wheel battery golf cart to the section when a chain fell from the oper. comp. Without thinking or stopping the ride, He reached out with his L. hand to retrieve the chain, ride went thru a low place in the roadway causing his L. middle finger on his L. hand to be caught between the ride & the mine floor. He broke his finger and 6 stitches were needed to close.

An unintentional roof fall was discovered by a mine examiner making a weekly examination. The roof fall was approximately 8' high x 16' wide x 50' long and occured at the 106 crosscut in the #1 entry of north mains.

The injured tripped over the anchor chains for the belt tailpiece and fell against a belt stand and injured his right shoulder. The injured continued to work till 8/6/09 when he went to the doctor and was diagnosed with a torn rotator cuff. His 1st day off work was 8/7/09

A roof fall occurred in the #7 entry 250 feet inby spad #18010. the fall was about 20 ft wide, 40 ft long, and 8 foot high. The area was bolted with 5 foot torque tenison bolts. The fall was crib and danger off, and will not be clean up.

The #8 elevator was out of service for a period of time exceeding 15 minutes during an electrical storm when two fuses blew. Two individuals were on the elevator at the time. The fuses were replaced & the individuals removed from the elevator @ 12:40 PM. The elevator was placed back into service @ 12:50 PM. No injuries were reported.

Injured and co-worker were unloading 8 inch aluminum pipe from a scoop. As the co-worker picked an end of the pipe up injured's left middle finger was caught between the pipe and scoop smashing the finger tip. He sustained a left distal phalanx fracture and began losing time from work on 07/02/09.

Employee began missing time for a work related condition when surgery was required to his right knee. MRI findings showed damage due to changes from trauma. He has been employed for over 31 years as a supervisor in underground mines. The condition was noted after he felt pain from walking.

The #8 elevator failed to leave the top landing. The elevator displayed contactor fault. Upon investigation a faulty coil on the M contactor was found. The coil on the contactor was replaced and the elevator placed back into service @ 9:15 AM. No individuals were on the elevator at the time.

While using a floor mounted grinder to smooth up a cut on a thin piece of metal the object and the employees right hand were pulled into the wire brush wheel. The table and guards were all in place and properly adjusted. The employee sustained an amputation on the tip of the little finger and a laceration on the ring finger which required 12 stitches to close.

He continued to work at his regular duties until 6/16/09 when he was removed from work for a medical miniscus tear of the right knee. (EE started losing time again on 1/14/10 due to increased pain and pending surgery.)

Employee began missing work on 5/1/09 for what was originally thought to be a personal injury. After further review of tests & medical evidence it was concluded the condition was caused by repeated trauma sustained from work related activities. He has worked various jobs in the mine for over 30 years and suffered a clavicle strain that required surgery.
