Showing all 36EE was standing behind the operator while operator was setting the ATRS. The ATRS was almost to the top when a rock fell out hitting the ATRS and sliding down towards the canopy. The rock struck the canopy and bounced off striking the EE on the top of right hand causing a laceration that resulted in 6 stitches. The top was 14' high and the rock was 20"x14"x3" about 20 pounds.

While on the Main West travel way, the examiner was trying to pry down a piece of rock that was resting on a curtain board. Another piece of rock fell striking him across the bridge on the nose causing a small fracture and eight sutures.
While employee was resting his arm on the miner, piece of rock fell hitting his right arm and thumb.
At approximately 10:00pm the employee was stuck by falling debris on top of his head. This caused his teeth to come together abruptly and a crowned tooth was broken out. The remainder of the tooth will need to be removed and replaced in the future.

Employee was struck by a piece of falling coal. The employee's hand was caught between the piece of coal and a tray on his bolting machine. The piece of falling coal amputated the first joint of the employee's left little finger.

He was transported to the hospital and received stitches in his right ring finger and returned to the mine. This happened in the #1 entry on the Headgate Unit. 001 Unit.
Employee was hanging roof bolter cable across the entry of #2 entry of the Tailgate Unit at x-cut #90, a piece of rock fell from the screen wire and struck him on the back of the neck at the hair line and made a laceration down the back of the neck. He was taken to emergency room and received stitches in the back of the neck and returned to work.
