The employee was attempting to move a piece of rock off of the tail drive on 16E Longwall, when a rock measuring approximately 10"x5"x5" fell from between shields #121 and #122 striking the tip EE's right middle finger between the rock EE was attempting to move and the rock that fell. Resulting in a laceration and fracture to the tip of EE's finger.

The employee stated that EE was on the operator side of the right bolter on #2 section. As EE was backing out of 6R, EE spotted a piece of draw rock. The rock looked as if it went back into the rib, so they chose to bolt it to the top rather than try and pry it down with a slate bar. When EE pulled the starter out, the rock fell and broke on the mast, striking EE's left forearm.

A piece of draw rock fell, striking the EE on EE's right 5th (pinky) finger, causing a laceration which required sutures to close.

The injured was loading high voltage cable in #3 entry on 15 East Brk #15. As EE was lifting a loop of cable onto the car, a piece of rock fell from the mine roof, striking EE on the right wrist area, causing a laceration. The Laceration was closed with medical glue.

Injured was in the process of spinning the drill steel out of the hole, when a piece of rock came loose from the top and struck EE on the left pinky finger causing a laceration/contusion.

The injured was drilling a hole to install a roof bolt on the 3 South Head Gate Panel, when a piece of rock measuring approximately 6" W X 5-1/2" L X 1/4" fell from the top and struck the injured's drill pot and a piece broke off and bounced up striking the injured just above the bridge of the nose. Causing a laceration that took one suture to close.

EE was at the Power Center and heard the breaker knock on the right-side roof bolter. EE stated that they went to check the cable. EE states that they saw a piece of draw rock on the cable. EE states that they bent over to remove the rock off of the cable when another piece of draw rock fell striking them on the back of the right hand causing a fracture to one of their fingers.

EE was bolting in the No. 7 right. EE put the steel against the top to start drilling. A piece of draw rock fell out around the bolt hole and hit the EE on the left hand causing a laceration 7" long. The rock scraped down the left arm and hand causing a laceration. The EE received 10 sutures to close.

EE was walking back to their S/C from break No.8. The S/C was parked in 5 Right. EE was on the out-by side of the S/C when their left foot was struck by a piece of draw rock.

The employee was in the process of drilling a test hole and as EE was adding EE's pusher steel a piece of rock fell from the mine roof and struck EE on the left forearm causing a laceration that took 3 sutures to close

The injured was bolting in the #6 Heading on South Mains Section. The Injured was installing a rib bolt in the 3rd Row of the heading and had EE's left hand on EE's tray when a rock measuring 1-1/2' W X 2' L X 1/4" thick struck EE on EE's left ring finger.

EE states that EE was in the process of removing the drill steel. As EE reached to pull the drill steel from the rib a piece of kettle bottom became dislodged striking the EE's hand against the drill pot causing a laceration and fracture.

The injured was setting bits on the tail side of the shearer, when a rock fell from jack 14-16 striking EE on the back leaving a laceration on the left lower side of EE's back approximately 10 inches long requiring sutures to close the wound.

EE was bolting in No.8 entry. A piece of rock came down the drill steel. The rock was spinning when it slid down the drill steel striking the EE in the upper right bicep area causing a laceration.

Injured was bolting in the #1 entry on 1 South. They pulled up to start the third row of bolts in the cut. EE sounded the roof with EE's steel and placed the steel in the drill pot. A rock measuring 10"x10"x2" fell causing a laceration to the right forearm, requiring 3 sutures to close.

Employee had just finished drilling the hole for the second bolt in the last row. While reaching to get the steel out of the hole, a piece of rock measuring 16 inches long, by 10 inches wide, by 1 quarter inch thick struck EE on the arm, just above the glove, causing a laceration approximately 3 inches long, requiring 2 Sutures to close

The employee was bolting on the South Mains section. EE was drilling a rib bolt in EE's 3rd row, when a piece of rock measuring 2' wide by 3' long by 6" thick, fell from the entry appx 9 to 9.5 ft high in between the center t-pad and outer t-pad bouncing off the T-bar and coming down striking EE down EE's right leg and across EE's foot, causing a fracture to EE's right foot

The injured was drilling the roof and was in the process of connecting the pusher steel when a piece of rock measuring 20" X 20" X 0"-1" fell striking the employee on the left wrist causing a laceration that required 9 sutures to close.

The injured was drilling the top when a piece of rock measuring 8" x 6" x 1" and one 11"x 6"x1" fell from the top. One rock knocked the injured's hat off, the other one hit the injured on top of the head, causing a cut that required 9 sutures to close.

The injured set the bolter up to spot a bolt, about 25 feet outby the working face. As EE started drilling, a piece of rock measuring 12" x 5" x 3/4" thick, fell from the roof striking EE on the face and cutting EE above the lip and took 5 sutures to close.

EE was bolting top and was on EE's second row of bolts, The EE was in the process of spot bolting the corner, when the corner broke out striking EE on EE's left side.

The employee was walking thru the center of a walk thru bolter when a piece of rock measuring 65" L X 24" W x 1-10" T fell approximately 12 inches between bolts striking the employee in the top middle back at a glancing blow.

The injured had just finished bolting on the next to last row in 3L crosscut when they started up on the pinner to advance to the next row, a piece of rock measuring 20" x 14" x 4" fell from the top striking them on top of the hard hat causing a laceration to forehead. The laceration required 4 sutures to close.

The injured was installing a roof bolt in the #2 Entry of 9 East Section. EE had drilled into the hole enough to install EE's pusher onto EE's starter steel and as EE was inserting the steel into the chuck a piece of rock measuring 12" L x7" W x 1.5" thick fell from the mine roof cutting off the tip of EE's right middle finger.

EE was carrying miner bits to the right side of the miner approximately 50' in-by survey station 728 when a piece of draw rock fell striking the EE on the top of the left foot.

EE was traveling down right stopping line in scoop roadway gathering up block. EE bent over to pick up block and a piece of rock fell out striking EE in the right middle back area. EE was seen at a medical center with negative findings. EE was given a full release to return to work. EE has not reported back to work.

EE was bolting in No.5 entry when a piece of draw rock fell between the bolt and rib. The rock bounced off the canopy striking the EE in the lower back and lower left leg resulting in a hair line fracture in the lower left leg. This employee is currently working on restricted duty.

EE was bolting top and turned to look under a brow. Something came down and hit EE in eye. Object was able to get behind glasses because of the way the EE had their head turned. This is being reported due to EE being prescribed antibiotic Eye Drops.

While attempting to raise canopy on drill mask, after tramming up and setting the bolter ATRS to install the last row of bolts, a rock measuring 9"W x 17"L x 0-1" thick fell from the roof, striking the employee on the right hand at the wrist area, between EE's Kevlar arm sleeve and glove. Causing a Laceration which required 4 sutures to close

While in the process of drilling a bolt hole, when a piece of rock fell from the roof ,striking the canopy of the roof bolter and a piece 12"X 10"X 1-0" struck right hand's ring finger causing an avulsion of the of the skin from the very tip of finger. EE finger was caught between the rock and the tray on the roof bolter. No bone damage. Started Missing work on 10/12/18

EE was installing outside bolt and had placed the drill steel against the top. As ee was preparing to begin drilling the hole a piece of the top broke loose and fell down bouncing off the drill pot and struck the employee below right eye causing a laceration. EE was transported outside and to GVMC where ee required 6 stitches to close the laceration.

Injured was getting ready to rock dust # 4 heading; a piece of rock fell from the roof striking ee on the head, knocking ee to the ground. EE walked from # 4 entry to # 2 entry to tell the boss about the accident. They immediately treated ee, and transported ee to the surface.

EE was bolting area for boom hole. While bolting a piece of rock fell striking EE on the top of the foot causing a laceration that required 3 stitches.

The injured was installing a rib bolt in the #2 heading on 28 Right Section when a rock measuring 19" L X 9" W X 6 1/2" Thick fell from the area of the ATRS striking the injured's left hand resulting in an avulsion the tip of the middle finger. The employee was taken to Clinch Valley Medical Center where the top tip of left middle finger was surgically amputated.

Inj was installing bolts in the 2 Left Heading 27 Right. Inj was in the process of drilling the outside bolt hole in next to the last row of bolts that was to be installed in this cut. The hole was drilled approximately 1' when a piece of draw rock fell from the mine roof and struck inj in the left neck/shoulder area resulting in a contusion and arm was placed in a sling.

EE was bolting in the No.2 Entry on the No.1 section. EE was installing the fourth row of bolts when a piece of draw rock fell from the top striking the EE on the left forearm which resulted in a laceration requiring stitches.

A piece of draw rock became dislodged from the mine roof and fell approx. 3' before striking injured in the head and then in the left arm and hand. When the draw rock struck injured in the left hand palm side thumb area EE received a laceration. Inj was transported to Clinch Valley Medical center where EE received 10 sutures.

EE was kneeled down driving insert in ripper head of miner. A piece of draw rock fell between bolts hitting EE in EE's lower right leg resulting in a fracture to the tibia.

EE was running a s/c. The s/c ran out of cable. EE parked the piece of equipment and went to see if anyone else needed help. During this process the EE was struck in the middle of EE's back, hip, and leg.

The injured placed right hand on top of the 2 East Longwall stageloader and a rock measuring 19" long x 6" wide x 0 to 1/2" thick fell and struck the tip of right middle finger causing lacerations which took 5 sutures to close.

EE was rolling up curtain when a piece of draw rock fell striking EE on the Neck and shoulder area.

The injured was starting to drill the last hole in the last row of bolts when a piece of draw rock fell on employee's left hand. The rock measured 16"-21" in diameter and was 0-2.5" thick. The injured was working on 4 East no. 3 heading. The injured fractured employee's left little finger and 2 pins were put in employee's left ring finger.

EE was installing a curtain board to be in compliance with ventilation plan, during this process a piece of draw rock fell from between bolts striking the EE on EE's left foot/ankle.

EE was bolting in return - took slate bar to pull loose rock. The rock fell on canopy & as EE was trying to get rock off of canopy another piece of rock fell out between bolts striking EE on the back.

EE was running a continuous miner on a Pillar section. EE saw the top dripping over the miner and was starting to reposition self and was struck by a piece of draw rock.

Employee was struck by a piece of slate measuring approximately 7"W x 10"L that fell from the mine roof. The mining height in this area is approximately 62".

EE was walking around miner and was in the process of dropping the line curtain and installing a pogo stick for ventilation control purposes when a piece of draw rock fell striking EE on neck and back.

Employee states EE was unhooking chain from scoop when some draw rock fell and hit finger which resulted in a fracture to the finger.

Employee was operating a continuous mining machine when a piece of rock fell from the corner and struck employee on the foot which resulted in a fracture.

Employee was on knees working when a piece of coal fell and hit employee. Employee never sought treatment until his 10/14/15 shift where he was placed on restricted work duty.
