Lower War Eagle Coal

EE was attempting to cut a clip out of a jumper to gain better access to pull the splice pin, the knife slipped out of the belt, puncturing the EE on the right leg approximately 4" above the knee, resulting in one suture to close the puncture wound.

A fall occurred on September 10, 2025, in-by survey station No. 11515 in the No.22 pillar block. The fall was approximately 8' thick.

A fall occurred on 08/10/2025 approx at 12:10 a.m. in the No. 4 entry near spads 17094 and 17085, 20 to 30 feet. This fall was a continuation of a fall that occurred on July 28, 2025, at approx 12:45 pm in the No.4 entry near spad No. 17107. This area has been under a K-Order since July 28 with multiple modifications.

EE pulled the roof bolter up to install the next row of bolts. After setting the T-Bar the EE went to step forward off the platform when they slipped and fell forward. Their left foot rolled back while the rest of their body fell forward.

EE was changing the scoop batteries. While the EE was disconnecting the charger cable from the scoop, the EE accidentally pulled the battery lid down on EE's hand, causing a laceration.

EE was turning S/C into the right side dump. EE misjudged the turn and hit the corner of a coal rib with the canopy, which resulted in a piece of coal dislodging and coming in contact with the EE, striking the EE on the right ankle, resulting in a fracture.

EE was walking out-by down the track entry from the No.3 section. EE was walking in a bent position. The EE came in contact with a roof bolt causing a laceration to left ear which required 8 sutures to close.

EE lost footing and slipped causing the EE to fall forward and striking their mouth causing a laceration that had to be closed with sutures.

EE was working on knocking a pin in the C/M cutter boom when a fragment of the hammer hit EE in the right forearm. The EE continues to work in EE's regular position. This incident became reportable on 10/11/2024 due to the removal of the fragment and the receiving of sutures to close the wound.

EE was dismounting a scoop and twisted right knee. This incident became reportable because the EE had surgery on October 4, 2024. The EE continues to work in EE's regular position with no restrictions.

EE was helping load the belt into the scoop bucket. As they were lapping the belt, the scoop rolled forward and the employee's foot accidentally got caught between the scoop bucket and the mine floor causing injury.

EE was stepping off of the Roof Bolter down to the boom controls. EE stepped onto the uneven surface of the mine floor rolling their right ankle. The EE continued to work for several shifts. The injury became reportable due to EE being placed on restricted duty on June 6, 2024.

EE was watching the No. 155 Roof Bolter being backed out of the No.2 Entry. The R/B operator bumped the reel. The cable came towards the EE. The EE jumped out of the way to avoid the cable. The EE landed awkwardly on their right foot causing the EE to roll ankle which resulted in a fracture.

EE was putting a scoop on charge while raising the battery lids. EE felt a pop in their back. The employee continued to work full duty until April 3, 2024. On April 3, 2024, EE was moved to restricted duty at which point the incident became reportable.

EE was changing the pot on the 80 C/M. The plate that holds the pot slipped and smashed the EE's finger causing an avulsion that required 5 sutures to close.

EE was cutting the end of a belt pin. EE rested one handle on the belt rail and used both hands on the other handle. When EE applied force, the cutters slipped and the cutting end struck EE in the mouth chipping EE's tooth.

EE was walking to the parking lot. EE slipped and fell hurting their left knee. EE started missing work on January 19, 2024.

EE was walking back to their S/C from break No.8. The S/C was parked in 5 Right. EE was on the out-by side of the S/C when their left foot was struck by a piece of draw rock.

EE was working on 3B Head Drive attempting to take the motor apart from the reducer to rotate it up. During this process, the motor came out pulling the EE's arm in a downward motion. The EE complained of their right shoulder hurting. The EE continued to work in their regular job role until April 21, 2024. The EE started missing work on April 22, 2024.

Employee was holding an airlock door open when the scoop came through and accidentally slid off the rail causing the scoop bucket to hit the door. The door swung while the employee was holding the handle of the door causing injury to EE's hand.

EE was hoisting a sun gear off a flat car using a chain hoist. The weight of the chain hoist caused the rope to slip. The EE's pinky was caught in the rope causing a laceration and fracture.

EE was cutting the top part of a pair of knee pads when the knife EE was using slipped and struck EE in the left thumb causing a laceration that required sutures.

A roof fall has been found on 5 belt in the #3 entry, spad # 11765. Fall is approx. 40'x20'x4'-5' thick. The fall does not impede travel or ventilation. The fall has been barricaded off.

A roof fall was found at Crosscut No.33 along the No. 7 beltline in-by survey station 14960. The fall measures 50' long X 20' wide X 9' thick. This fall does not impede travel or ventilation. This fall has been barricaded off.

EE was bolting in No.8 entry. A piece of rock came down the drill steel. The rock was spinning when it slid down the drill steel striking the EE in the upper right bicep area causing a laceration.

Employee was cutting the corner of a belt with a knife and the knife slipped and accidentally struck employee's thigh causing injury.

EE was working on the new No.3C head trimming belt. EE was cutting towards themselves, the knife slipped out of the casing cutting the EE on the outside of the R knee. EE received 4 sutures to close the laceration.

EE was using a pull along duster. The auger on the duster quit turning. The EE inspected the auger and seen something was fouling the auger preventing it from turning. EE attempted to remove the object with hand. As the EE was attempting this the Auger started back up. The EE received lacerations on the right hand which required sutures.

EE was bending over to pick up a chain ratchet when EE felt a pain in lower back.

EE was placing rock on curtain and caught a finger between rock and rib causing a laceration that required seven sutures.

EE was bolting top and was on EE's second row of bolts, The EE was in the process of spot bolting the corner, when the corner broke out striking EE on EE's left side.

EE was scaling a piece of draw rock with a slate bar in the No.5 entry on 1 section. A piece of the draw rock struck the EE on back of the right forearm causing a laceration that required 8 sutures to close wound.

EE stepped off of a forklift and felt sharp pain in right knee. EE continued to work in regular job position with no restriction. On July 27, 2021 EE was placed on restricted duty.

EE was prepping to skive a conveyor belt. EE was making the initial undercut to remove the strip of rubber from the end of the belt. EE was cutting towards left arm when the knife blade exited the belt rubber, resulting in the knife blade cutting the EE on the backside of the forearm.

EE was attempting to remove a 4" waterline coupling to replace it with a fire valve. EE was striking the coupling with a hammer, the hammer bounced back striking the EE in the forehead causing a laceration that required 5 sutures.

EE was pulling a combination head drive with a battery powered scoop. As EE pulled the belt head into the intersection the head drive got fouled on the corner of the rib and the 3/8" chain that was being used broke and struck the EE in the front tooth breaking the EE's tooth.

EE was repairing the suction on a R/B. A piece of draw rock dislodged and struck EE in the back. EE was placed on restricted duty.

EE was traveling down right stopping line in scoop roadway gathering up block. EE bent over to pick up block and a piece of rock fell out striking EE in the right middle back area. EE was seen at a medical center with negative findings. EE was given a full release to return to work. EE has not reported back to work.

EE was in the process of changing the tail roller on C/M. While striking the connecting link of the chain with a hammer a piece of the connecting link chipped off and struck the employee in the left shin, embedding in the skin. EE was taken to a medical facility. The material was removed and the incision was closed with two sutures.

EE was bolting top and turned to look under a brow. Something came down and hit EE in eye. Object was able to get behind glasses because of the way the EE had their head turned. This is being reported due to EE being prescribed antibiotic Eye Drops.

EE was operating a scoop in the No.8 heading on the No.3 section. EE hit a hole bouncing the employee around in the deck jarring their back.

At approximately 5:45a.m. a roof fall occurred on No.3 section 006-0 MMU. The roof fall took place in the No.9 entry LOCC intersection approximately 120' in-by survey station 11783. The fall extended from the solid right rib in No.9 into the 8 right crosscut about 15'. The fall measured approximately 35' long X 19' wide X 5 1/2' high.

EE was running the C/M on the No.1 section. EE pulled the miner into 4L and was panning up Gob as they pulled up and loading into S/C a piece of rib rolled out striking the EE in the back of neck causing a laceration which required staples/sutures to close the laceration.

EE was walking towards Continuous Miner Operator when a rib rolled out striking EE in lower legs.

EE states while bending over to pick up block for the use of building a stopping EE twisted back.

Removing a rock from the miner while pushing on the rock the rock broke cutting the right arm causing a laceration that required eleven stitches.

EE was moving the CO line and monitor during a pull back. EE was walking around a scoop. EE hit a flyboard which knocked them back. During the fall the EE fell back on right leg causing injury to the right upper leg. This incident became reportable on 2/22/19 due to the EE having surgery to repair a Quadricep Tendon Tear.

EE was holding a new knee pad attempting to cut part of the knee pad off. EE was sitting down during this process and the knife slipped which resulted in the EE cutting self causing a laceration which required sutures to close the wound.

EE was roof bolting in the No. 1 entry. EE stepped on a piece of draw rock. The draw rock cut through the bottom of EE's boot. EE received a laceration on the bottom of EE's foot. EE received 3 stitches to the bottom of EE's foot.

EE was hanging cables throughout the shift. During the shift EE states felt a tightening in the groin area. This incident was not reportable until EE was placed on restricted duty on 11/7/2018.

EE was spotting roof bolt and adjusted roof plate with hand causing the plate to smash EE's index finger against damaged plate and roof. EE is scheduled for surgery to repair EE's left index finger tip and is currently on light duty and will remain on light duty during the healing process.

EE was sanding on a ride prepping it for paint. EE felt like EE had dust in EE's eye. EE went and had EE's eye checked and found out that EE had an infection and a laceration. EE was put on an antibiotic Eye drop.

EE stated that EE was throwing glue from the offside of the RB machine to the operator side. EE states EE felt a pop and pain in EE's right shoulder. EE is currently on restricted duty.

EE was working on a rock duster machine. The auger had become locked up. EE attempted to free the auger with a screwdriver but the screwdriver kept slipping. The EE thinking the machine was off tried to turn the motor by hand. The motor started and pulled EE left middle finger between the belt and motor pulley. The EE has returned on modified duty.

EE was bolting in the No.2 Entry on the No.1 section. EE was installing the fourth row of bolts when a piece of draw rock fell from the top striking the EE on the left forearm which resulted in a laceration requiring stitches.

EE was hand loading roof-bolt plates into scoop bucket. EE finished loading the plates and went back to the section. EE was exiting the scoop and felt pain in the lower lumbar region. This incident did not become reportable until 01/08/2018 when EE was placed on restricted duty.

EE was unloading a retrieval rope by self out of a scoop bucket into the E Sled. EE states EE felt a pull in left shoulder.

Employee was dropping the bolter cable, the operator turned the reel on pulling the cable down striking employee on the left side and pushing employee into the rib hurting right shoulder.

EE was loading plates onto the R/B Machine, during this process EE states felt a sharp pain in the center of abdomen.

EE was kneeled down driving insert in ripper head of miner. A piece of draw rock fell between bolts hitting EE in EE's lower right leg resulting in a fracture to the tibia.

EE was in the process of hanging roof bolter cable. As EE started hanging the cable, the cable reel was engaged. The cable struck EE in the legs knocking EE to the ground. This was not reportable until EE had an MRI on 12/12/2017 and was diagnosed with a fracture of the medial tibial plateau and a meniscus tear.

A non-injury roof fall occurred on the #3 section at approximately 22:00 on 10/9/17. The fall occurred in the neutral #3 entry in the intersection of Spad 8709 nine cross cuts outby the feeder line. The fall measured approximately 20' x 20' just above anchorage of 48". There were no persons affected and the area has been timbered, cribbed and dangered off.

EE was pulling curtain that was covered in mud. EE is complaining of pain in their back and groin area.

EE was picking up pin supplies with scoop. EE put down pressure on the scoop bucket to pick up Roof Bolts. During this process an hydraulic hose burst, spraying the EE in the face and eyes.

EE was running a s/c. The s/c ran out of cable. EE parked the piece of equipment and went to see if anyone else needed help. During this process the EE was struck in the middle of EE's back, hip, and leg.

EE sat down on a Blue Wolf container. The container had Blue Wolf on the top of it. The Blue Wolf soaked through the EE's clothing causing a reaction to the EE's skin.

A non injury roof fall accident occurred @ approximately 2120 hours on 08/04/2017. The roof fall occurred on #3 section in the #5 heading in-by spad #8115 on the left side lift of the MRS which pulled a few of the Rib line bolts. This fall did not impede escape travel ways and did not effect ventilation.

EE was in process of pulling miner cable back. During this process, the rib corner fell out striking the EE and pushing the EE against the pan of the miner.

EE was hauling to the #1 entry. EE was adjusting curtain. EE claims back was hurting. This 7000-1 is being reported due to the ten day time frame. This 7000-1 could possibly be amended or rescinded at a later date due to the lack of information at the time the 7000-1 is being reported. Investigation is still ongoing and medical documentation is being gathered.

EE was bolting on #2 section on the #5 Heading at survey station 8456 on the left curtain side of the bolter. EE was installing second bolt in the third row. During this process a rib rolled striking the EE on the right leg which resulted in a fracture.

EE was walking towards the rear of the continuous miner checking the center line when a piece of rib rolled out striking EE on leg.

EE was trying to get an 8'x4'x 5" rock off of the continuous miner. EE grabbed the rock with hands and was trying to help the continuous miner operator remove the rock. During this process the EE stated EE had hurt EE's back.

Employee switched out with the other roof bolter operator when a piece of drill steel came apart, bounced and hit the drill pot when the steel hit the employee in the mouth which resulted in tooth being chipped (fractured).

EE was loading bolt plates. EE felt a pull in groin area. EE was seen on 04/22/2017 in the ER where there wasn't a diagnosis and nothing found. EE continued to work. This became reportable as of 08/15/2017 when EE had to have surgery for a hernia and was taken off work.

A non-injury fall has occurred in the No.6 entry of the Southeast Submains at Cross-Cut No.11, one break inby Spad NO.7943. This fall measures approximately 5'thick x 25'long x 20'wide and extends into the 6R Cross-Cut. This fall does not impede travel nor does it interrupt ventilation.

EE was working on the Continuous Miner. EE was using a sledge hammer to strike against a pin. EE felt a pop in left shoulder area. This became reportable when EE began to miss work on 03/24/2017.

EE had knelt down in a location where a solid block stopping use to be. Two of the blocks were still hanging from the top. The two blocks became dislodged striking the EE on the left ankle.

EE was picking up a piece of metal and felt something pull in their lower back

Employee tripped on a pile of gunite. EE put hand down to catch self as ee was falling and got cut by a rock.

EE was operating Roof Bolter. EE lost suction on side of the machine. EE was in the process of trying to regain suction. EE requested for the pin partners assistance. EE asked the pin partner to apply up-pressure when EE began to yell about finger being caught. EE received severe laceration that later required amputation of the left pinky.

Employee was awarded occupational illness.

EE was removing guard from miner when one of the other shields fell and caught EE's thumb between guard.

Employee was diagnosed with degenerative disc disease in the form of desiccation. The operator received medical evidence on 11/11/16.

EE was changing out a conveyor motor when the slate bar ee was using slipped striking ee in the mouth causing a laceration to upper lip.

Employee was walking down slope when EE felt a pain in the left knee. Employee continued to work. The operator received notification that the employee was scheduled to have the meniscus repaired on 12/15/16.

Employee was diagnosed with bilateral hearing loss.

Employee was moving the continuous miner power cable out of the way of the continuous miner. The mine floor was wet and ee slipped and twisted right knee. ***Employee did not start missing days away from work until 10/15/2016.

Employee was unhooking the continuous miner cable from a rope hanger when the continuous miner cable slipped. The caused the "S" hook on the rope hanger to puncture left hand.

EE was walking around miner and was in the process of dropping the line curtain and installing a pogo stick for ventilation control purposes when a piece of draw rock fell striking EE on neck and back.

Employee was walking down the entry and walked into a piece of rock that was hanging down from the mine roof causing a laceration to left ear.

Employee was carrying a 42" top roller. EE slipped on the track and strained a muscle in EE's chest trying to hold on to the roller.

Employee was trying to scale a piece of loose rock from the mine roof. The piece of loose rock was partially held up by a roof bolt plate. The employee strained back while trying to scale down the piece of rock.

Employee was cutting angle brackets when hot slag entered ear. Employee continued to work until 12-16-16 when employee was taken away from work.

Employee was trying to remove roof bolter cable from underneath rock when EE received an apparent electrical shock.

Employee's hand was caught between airlock doors. EE never missed work until June 27, 2016 shift.

Employee slipped on muddy floor conditions and twisted right knee. *** Employee continued working regularly scheduled shifts until 9/28/2016 when EE was taken off work.

Employee was diagnosed with multifocal degenerative changes.

A roof fall was found in #1 entry at 33 Brk. (spad 429) of the North West Mains return during the weekly examination on 4/25/16. The fall was above anchorage and measured 20ft wide, approximately 25 ft. long, and 0 to 4 ft. thick. This fall was found in the non traveled common return air course. All approaches have been supported and dangered off.

Employee was diagnosed with occupational pneumoconiosis. The employer received notification on April 6, 2016.

Employee received burn to the left little finger that required partial amputation of the finger at the first knuckle. Employee was installing ISCR on Company No. 141 Roof Bolter. The incident happened on #3 section on Day shift.

Employee states that they were building a stopping and while wedging the top block into place employee struck left thumb with a hammer that resulted in stitches.

Employee was lifting a timber when they felt pain in lower stomach area. The employee never missed work until the 3/3/16 shift.

Employee was operating a continuous mining machine when a piece of rock fell from the corner and struck employee on the foot which resulted in a fracture.

Employee was preparing to advance the roof bolter forward to the next place when a piece of rib rolled and struck him on the back side.

Employee was waiting on MRS to be moved forward when a piece of rock fell from the mine roof and struck the employees foot.

Employee was walking around the continuous mining machine when a piece of draw rock fell and hit the backside of the employee's arm resulting in a cut which required stitches.

Employee was in the process of changing out a shuttle car wheel unit and as he picked up the fender, he tripped over a shuttle car cable that had been placed in his work area. Therefore, the end of his fingers, between the fender and frame of the shuttle care were smashed.

Employee was loading structure into scoop bucket when his finger was caught between the structure.

Employee was hanging feeder cable when he began to experience pain in his back.

Employee was installing roof bolts and when he stood up, he twisted his knee.

EE was laying track & while installing tie in track rail caught left pinky finger, smashing it.

Employee was attempting to knock a block out of a stopping for a regulator. When he hit the block with his hammer, wet plaster flew into his left eye.

Employee was operating miner when a rib rolled and hit employee.

Employee was pulling shuttle car cat-head up the entry during a power/belt move when he began to experience pain in his lower back area.

Employee was walking when he stepped into a hole and twisted his back. Note: Employee never missed work or sought treatment until his 02/27/15 shift.

Employee was operating motor on track when he bumped his head.

Employee was loosening device on scoop from water car when his hand was caught between objects and he received a cut on his finger that required stitches.

Employees were pulling scoop charger into scoop bucket. Employee was walking around the scoop when he slipped and fell. He was trying to catch himself by grabbing the bucket of the scoop when his hand was caught between the bucket and ram.

Employee was cutting a bag of bulk dust open with a utility knife when the knife slipped and cut his forearm.

Employee was bolting top when a piece of rock broke loose and hit his forearm requiring stitches.

Employee was lifting cribs when he felt pain in his shoulder.

Employee was loading a timber in the scoop bucket when he smashed his finger between the timber and the side of the scoop bucket.

EE had just loaded a shuttle car w/the continuous miner when a rib rolled & fell onto his leg. On this date, he was transported to a Medical Center ER & was told there was no fracture. On 4-21-14, he kept experiencing pain & soreness & went to a different Medical Center where a 2nd X-ray was performed & was told he had a hairline fracture of tibia. His 1st day missed was 4-22-14.

Employee was cutting a piece of the suction hose off on the bottom of the drill pot when the knife slipped cutting his left arm requiring stitches.

He was helping to move the rail bender from one rail to the next when moved the bender down to clamp it in place his finger was between the rail and bender causing the accident.

EE was traveling up the main line # 1 belt when he stopped to open a air lock door. The hinge broke as he was opening the door, causing the door to fall and knock him backwards.

Employee alleges that he was walking through flypads and was struck by a shuttle car. Cliffs Logan County Coal disputes that EE sustained an accident, as that term is defined in 30 CFR 50.2(h), or that he sustained an occupational injury as that term is defined in 30 CFR 50.2(e) or an occupational illness as that term is defined in 30 CFR 50.2(g).

While filling up the pod duster, employee was laying the hose back at the fill tank and felt pain in his back.

While installing roof bolts, employee had his right hand on the drill steel and chuck, pinching his hand causing a cut that required stitches.

Employee was installing belt hangers when his drill steel became stuck. The employee tried to spin the steel out, while placing his hand on the drill steel, his hand got caught, wrapping his arm around the steel causing a compound fracture to his right forearm.

While making a splice on a buggy cable, employee was holding the knocker in his left hand. He then swung to hit the knocker and missed it, hitting his left thumb. He received several stitches and a broken thumb.

Employee was hanging miner cable when he felt a sharp pain in his back and hip.

Employee was helping on a belt power move. When he attempted to hang a miner cable, he felt a pop and pain in his lower back. He stopped working at that time.

EE was working on section. The floor was wet. He was going to hang the shuttle car trailing cable and felt a tingling sensation but there were no cuts or markings on hi skin. He was taken to the ER via ambulance. A EKG was performed on site by the amb. crew and he was kept overnight at the hospital for observation purposes only.

Employee was hanging miner cable and waterline when he felt a sharp pain in his shoulder. The employee never missed work until his 7/29/13 shift.

Employee was pulling belt and stacking it along the rib so a scoop could pick it up. He finished the shift and went to the doctor the next day.

Employee was cutting a zip tie with a utility knife when it slipped and cut his left hand requiring stitches.

Employee was pulling on miner cable when he felt pain in his shoulder. Employee never missed work until his 7/11/13 shift.

Employee was working on mains belt cleaning up draw rock when he lifted a rock and felt pain in his groin area. Employee started missing work again 7/15/13, for this surgery

Employee was rolling up a spool of Leaky Feeder Cable on a cart in #4 entry, 1st Right Intake when the cart flipped and he tried to catch it as it was falling and felt pain in his back.

EE was using B Bond to plaster a stopping. While doing so he got some of the B Bond on his arm. He brushed it off and continued to work until 5-31-13. He went to the hospital for treatment and started missing work on 6-1-13.

Employee was down on knees cleaning out brake sander when a piece of draw rock fell form the top and struck employee.

Employee was operating a shuttle car when he ran over a hole and began to experience pain in his back form the jarring.

Employee was hanging curtain when he smashed his thumb between flyboard and hammer.

EE was hanging roof bolter cable when he backed into a bent roof bolt plate and cut his ear which required stitches.

Employee was bending bolts in #7 entry when he felt pain in his back and leg.

Employee working on air pump. He reached into the pump to check for mud. He accidentally hit air valve, turning pump on. Has 3 broken fingers, cuts, & lacerations. Unknown hand.
