There were no witnesses to the event. Employee was found with right arm caught by the hold up roller of the remote discharge of the #1 conveyor. When found by crew members, employee was not responsive. Employee was transported to the surface when employee was pronounced deceased.

EE was in the process of moving the flat car to the spur track at 8 Head. While moving the flat car and motor into the spur, the flat car rolled, contacting EE's lower leg between the flat car and the motor.

EE was attempting to cut a clip out of a jumper to gain better access to pull the splice pin, the knife slipped out of the belt, puncturing the EE on the right leg approximately 4" above the knee, resulting in one suture to close the puncture wound.

A fall occurred on 08/10/2025 approx at 12:10 a.m. in the No. 4 entry near spads 17094 and 17085, 20 to 30 feet. This fall was a continuation of a fall that occurred on July 28, 2025, at approx 12:45 pm in the No.4 entry near spad No. 17107. This area has been under a K-Order since July 28 with multiple modifications.

EE pulled the roof bolter up to install the next row of bolts. After setting the T-Bar the EE went to step forward off the platform when they slipped and fell forward. Their left foot rolled back while the rest of their body fell forward.

EE was changing the scoop batteries. While the EE was disconnecting the charger cable from the scoop, the EE accidentally pulled the battery lid down on EE's hand, causing a laceration.

EE was turning S/C into the right side dump. EE misjudged the turn and hit the corner of a coal rib with the canopy, which resulted in a piece of coal dislodging and coming in contact with the EE, striking the EE on the right ankle, resulting in a fracture.

EE was walking out-by down the track entry from the No.3 section. EE was walking in a bent position. The EE came in contact with a roof bolt causing a laceration to left ear which required 8 sutures to close.

EE was working on knocking a pin in the C/M cutter boom when a fragment of the hammer hit EE in the right forearm. The EE continues to work in EE's regular position. This incident became reportable on 10/11/2024 due to the removal of the fragment and the receiving of sutures to close the wound.

EE was stepping off of the Roof Bolter down to the boom controls. EE stepped onto the uneven surface of the mine floor rolling their right ankle. The EE continued to work for several shifts. The injury became reportable due to EE being placed on restricted duty on June 6, 2024.

EE was watching the No. 155 Roof Bolter being backed out of the No.2 Entry. The R/B operator bumped the reel. The cable came towards the EE. The EE jumped out of the way to avoid the cable. The EE landed awkwardly on their right foot causing the EE to roll ankle which resulted in a fracture.

EE was putting a scoop on charge while raising the battery lids. EE felt a pop in their back. The employee continued to work full duty until April 3, 2024. On April 3, 2024, EE was moved to restricted duty at which point the incident became reportable.

EE was cutting the end of a belt pin. EE rested one handle on the belt rail and used both hands on the other handle. When EE applied force, the cutters slipped and the cutting end struck EE in the mouth chipping EE's tooth.

EE was working on 3B Head Drive attempting to take the motor apart from the reducer to rotate it up. During this process, the motor came out pulling the EE's arm in a downward motion. The EE complained of their right shoulder hurting. The EE continued to work in their regular job role until April 21, 2024. The EE started missing work on April 22, 2024.

EE was using a pull along duster. The auger on the duster quit turning. The EE inspected the auger and seen something was fouling the auger preventing it from turning. EE attempted to remove the object with hand. As the EE was attempting this the Auger started back up. The EE received lacerations on the right hand which required sutures.

EE was prepping to skive a conveyor belt. EE was making the initial undercut to remove the strip of rubber from the end of the belt. EE was cutting towards left arm when the knife blade exited the belt rubber, resulting in the knife blade cutting the EE on the backside of the forearm.

EE was attempting to remove a 4" waterline coupling to replace it with a fire valve. EE was striking the coupling with a hammer, the hammer bounced back striking the EE in the forehead causing a laceration that required 5 sutures.

EE was pulling a combination head drive with a battery powered scoop. As EE pulled the belt head into the intersection the head drive got fouled on the corner of the rib and the 3/8" chain that was being used broke and struck the EE in the front tooth breaking the EE's tooth.

EE was seen at a medical center with negative findings. EE was given a full release to return to work. EE has not reported back to work.

EE was in the process of changing the tail roller on C/M. While striking the connecting link of the chain with a hammer a piece of the connecting link chipped off and struck the employee in the left shin, embedding in the skin. EE was taken to a medical facility. The material was removed and the incision was closed with two sutures.

At approximately 5:45a.m. a roof fall occurred on No.3 section 006-0 MMU. The roof fall took place in the No.9 entry LOCC intersection approximately 120' in-by survey station 11783. The fall extended from the solid right rib in No.9 into the 8 right crosscut about 15'. The fall measured approximately 35' long X 19' wide X 5 1/2' high.

EE was moving the CO line and monitor during a pull back. EE was walking around a scoop. EE hit a flyboard which knocked them back. During the fall the EE fell back on right leg causing injury to the right upper leg. This incident became reportable on 2/22/19 due to the EE having surgery to repair a Quadricep Tendon Tear.

EE was holding a new knee pad attempting to cut part of the knee pad off. EE was sitting down during this process and the knife slipped which resulted in the EE cutting self causing a laceration which required sutures to close the wound.

EE was spotting roof bolt and adjusted roof plate with hand causing the plate to smash EE's index finger against damaged plate and roof. EE is scheduled for surgery to repair EE's left index finger tip and is currently on light duty and will remain on light duty during the healing process.

EE was sanding on a ride prepping it for paint. EE felt like EE had dust in EE's eye. EE went and had EE's eye checked and found out that EE had an infection and a laceration. EE was put on an antibiotic Eye drop.

EE was working on a rock duster machine. The auger had become locked up. EE attempted to free the auger with a screwdriver but the screwdriver kept slipping. The EE thinking the machine was off tried to turn the motor by hand. The motor started and pulled EE left middle finger between the belt and motor pulley. The EE has returned on modified duty.

EE was hand loading roof-bolt plates into scoop bucket. EE finished loading the plates and went back to the section. EE was exiting the scoop and felt pain in the lower lumbar region. This incident did not become reportable until 01/08/2018 when EE was placed on restricted duty.

EE was in the process of hanging roof bolter cable. As EE started hanging the cable, the cable reel was engaged. The cable struck EE in the legs knocking EE to the ground. This was not reportable until EE had an MRI on 12/12/2017 and was diagnosed with a fracture of the medial tibial plateau and a meniscus tear.

A non-injury roof fall occurred on the #3 section at approximately 22:00 on 10/9/17. The fall occurred in the neutral #3 entry in the intersection of Spad 8709 nine cross cuts outby the feeder line. The fall measured approximately 20' x 20' just above anchorage of 48". There were no persons affected and the area has been timbered, cribbed and dangered off.
