Showing all 7Employee was installing a 6' roof-bolt in-by the brow after the brow had been bolted by 9' cable bolts when a piece of rock fell and struck the right side of EE's hard hat causing a laceration on the right side of EE's forehead. EE received 4 sutures.
Employee stated EE had set EE's ATRS and swing out to start bolting and had EE's finger on top of the canopy when a rock 1'x1'x1" fell striking EE's right index finger. EE needed sutures.
Employee stated that while EE was leveling EE's drill pot 2 x-cuts outby the face area a piece of rock fell from a slip after EE drilled 2 holes in the roof striking the top of EE's right hand causing a laceration that resulted in sutures. Employee stated a piece of rock fell and struck right foot. The rock struck about 3 inches back on EE's boot. An xray showed that EE's big toe and small toe had a hairline fracture.
Employee stated was tramming the right side miner into the face of the #8 entry when EE reached to take the strap off and a piece of rock fell and hit EE's elbow. inby station 3+50. Sutures were required.
Attempting to put second steel in roof while holding top steel in roof with left hand, a part of a kettle bottom broke free from roof striking EE in the right hand.
