A ROCKFALL OCCURRED IN THE S/R IN THE 302 SUBMAIN WEST IN THE #4 ENTRY AT SPAD 3+50 SOMETIME BETWEEN THE AFTERNOON SHIFT ON 9/12/20 & 9/13/20 WHEN THE PRE-SHIFT EXAMINERS CAME IN AT 8:00 PM TO EXAM THE MINES FOR THE 3RD SHIFT WHICH STARTED AT 11:00 PM. THE FALL WAS 10X19X60.

Employee stated EE was bolting the #10 entry. The top and bottom pinner steels were stuck together and when EE pulled them apart pulling towards EE's face the pinner steel hit EE in the chin requiring 4 sutures.

Employee was installing a 6' roof-bolt in-by the brow after the brow had been bolted by 9' cable bolts when a piece of rock fell and struck the right side of EE's hard hat causing a laceration on the right side of EE's forehead. EE received 4 sutures.

Employee was rolling up the curtain in the #11 entry when they stepped in a hole and twisted left knee which caused them to fall. The Hospital determined that they had broken the left leg right below the left knee.

Employee stated while building a brattice EE stepped on a piece of block. When EE started to fall EE tried to catch self and twisted EE's lower back. Employee stated EE had a back injury previously.

Employee stated EE had set EE's ATRS and swing out to start bolting and had EE's finger on top of the canopy when a rock 1'x1'x1" fell striking EE's right index finger. EE needed sutures.

Employee stated that while EE was leveling EE's drill pot 2 x-cuts outby the face area a piece of rock fell from a slip after EE drilled 2 holes in the roof striking the top of EE's right hand causing a laceration that resulted in sutures. The rock also dislocated 4 fingers on EE's right hand. EE was wearing gloves at the time of the accident. #1 Unit - Entry #11 - Spad 1+50

Employee stated that the wet down hose was pushed into the feeder and became wrapped around the pit breaker. The power was locked and tagged out and employee was manually rotating the pit breaker unwinding the hose when right hand got caught between the frame and a pit breaker bit. The employee received sutures in right hand.

Employee stated EE was holding the steel and pushing it up with the pot when the steel bent striking EE's left index finger knuckle. EE had removed EE's hand from the steel after the pot came in contact with the steel.

The employee was attempting to hook up a diesel fuel trailer to a diesel ride. While lining up the trailer hitch to the bumper of the ride the wooden prop that the trailer was resting on slipped, mashing EE's middle left finger between the hitch and the bumper causing a fracture and sutures

the employee was behind the bolter reaching for a piece of tie wire with right hand, had left hand on top of the operators deck canopy the bolter slightly moved, it hit a high spot on the bottom, causing it to raise up mashing left index finger between the canopy and the top causing a fracture.

Operator reeled up bolter cable while tramming, EE was standing on the wrong side the cable when the cable was torn down from the roof striking EE in the jaw. EE was shoved against the rib causing EE to strike head causing a laceration to head resulting in 3 staples.

Employee stated EE was rock dusting the belt on the 2nd West when the hose EE was holding became caught and jerked EE off the trailer. When EE was pulled off the trailer by the tightening of the hose, EE's right foot went under the trailer and the wheel hit EE's lower leg just below EE's knee.

The employee was raising the drill pot to the roof EE was attempting to put the drill steel in the pot, the steel got caught on the chuck, when it went into the chuck it had too much up pressure the steel caught EE's glove and struck EE's hand. The employee did not start missing work until 8-3-2019.

Employee was bleeding air out of a 6" waterline at the fire valve on #3 Unit. The waterline had just been installed at the new unit. The employee was opening and shutting the fire outlet valve when the last section came apart at the last joint which was 20' away. The belt was chained to the roof but lunged forward striking the employee in the nose. This resulted in a broken nose.

The contract employee had finished loading supplies on a man-trip when the employee bent down to lift lunch box. When employee lifted lunch box employee felt a pain in their back. The following were not a factor: rules, regulations, mining systems or equipment, training, protective clothing or devices. Attitude may have been a factor. Did not start to lose time until 3/2/2019.

An unplanned roof fall was discovered in the 1st North Mains intake at spad # 31+00. Ventilation was not disrupted. The primary escapeway has been re-routed and the area cribbed and dangered off. Area was previously supported at time of mining, with fully grouted 6' bolts and supplemental supports consisting of 9' cable bolts.
