Employee was pulling on a piece of k-wall foram that was attached to the top. As EE pulled on it, a rock fell from the top and hit EE in the back of the head, knocking EE's hat off and pushing EE to the ground. Was knocked unconscious for a second.

As EE was removing bolts from the winch, a piece of coal/rock fell out from between the roof channel straps and struck the back of EE's leg forcing EE against the headgate drive. EE has pain and swelling in the right knee and ankle. No rules or regulations were violated.

CM was being serviced and injured employee handed a bucket of bit inserts to another employee and a small piece of coal/rock fell and struck the top of EE's right hand causing a laceration. Employee received 11 stitches to close the cut. Returned to work the next shift.
