Employee was walking from the tailgate towards the head-gate when employee was stepping from shield 105 to shield 104 a rock fell from in-between the shields and landed on employee's foot. This caused a laceration between the two small toes on right foot and fractured the small toe.

Injured miner was giving directions to another miner as they were using shield #3 to free pan blockage. Injured miner was standing under shield #4 when a piece of rock fell and hit foot. The wound required stitches to close.
Employee was cutting keepers off the connectors that hold longwall pan section together. As he was cutting the keepers, a piece of coal fell and struck him, this resulted in a fractured leg.

The piece of coal measured approximately 12" to 14" long by 6" to 7" high. Employee was taken to the hospital as a precaution and returned to his next scheduled shift.
While pushing in panline in the tailgate with shield 130 and 129, a piece of rock fell over gob plate and struck employee in knee. Late reporting due to further needed medical evaluation. He has been placed on restricted duty.
A piece of coal struck employee in ankle. Medical was sought, no fracture, told ankle would get better, full release. Ankle not getting better, appointment was scheduled with orthopedic DR., before appointment there was minor re-aggravation. Feb 10, 2011 he attended scheduled appointment, became LTA with soft tissue injury. Late reporting due to medical records gathering.
Employee was in the process of moving ventilation tubing, during this process a piece of cap rock fell and struck the employee in the head. Medical personnel has placed the employee on restricted duty as precautionary due to the possibility of a slight concussion.
