Person operating continuous miner on south section, #7 entry taking B run. Operator was setting 6 rows of bolts outby last row. Rock fell from between rib and 1st row of bolts and hit operator in shoulder, wrist and leg.
EE was backing miner out of xcut 6-5 and had head down watching miner cable when a piece of rock fell from between bolts and hit him in back of head. He was wearing hard hat.
Injured was roof bolting when rock fell from roof hitting him in left elbow and forearm (bruise).
EE was walking to feeder to service and said that a piece of rock fell from the roof in X cut 6-7 and it hit him on the lower back.
Rock fell out between posts. 2" thick, 1' in diameter. Bolts were 4x4 all around. A piece of layered sandstone fell out and hit him in the side on rib cage.
Individual was fixing some loose bolts on the belt line when a rock came down on his foot.
WHILE MAN WAS BOLTING A PIECE OF ROCK FELL AND HIT HIM ON THE SHOULDER.
Bolter (operator's side) had drilled hole for bolt, reached for bolt when a rock approximately 5' x 4' x 6" fell striking bolter on back and left foot.
While employee was on 2 right sect., #6 heading, 9 block a piece of slate 1" thick x 2' wide x 1 1/2' long fell from between 6' resin bolts with 2"T x 6"W X 12"L hole boards, striking employee on his right hand. Employee hasn't called mine to inform us of his injury. Company has tried several times to reach him.
Roof bolting. Small loose rock came loose and fell for appr. 3 feet striking hand.
MAN WAS LOOKING AT SCOOP & HAD HAND ON FINDER & ROCK FELL OUT BETWEEN BOLTS. ROCK APPROXIMATELY 1" THICK, 10" WIDE, 11" LONG, HIT MAN ON THUMB.
WHILE CHANGING BITS ON MINER EMPLOYEE WAS BENT OVER TO PIECE A WEB MEASURING 1"-3" THICK, 16" WIDE 24" LONG FELL OUT AND HIT HIM ON BACK. (ROCK FELL OUT BETWEEN ROOF BOLTS.)
