While traveling underground toward section, employees noticed top breaking and dripping and could not get stopped in time to avoid roof fall. Equip. operator stopped ride & started backing up when fall occurred, striking employee. #2 Main Intake 18 Bl., inby spad #34.

Accident type, without injuries

Roof fall on #2 belt intake in the #2 heading at 16 break. Fall is above anchorage and approximately 4.5' high and 20' long in the crosscut, extending 6' into the intersection. Area was originally mined in 1974 and bolted with 4' conventional bolts. Travelway and air course were not affected by fall.

Struck by flying object

Employee was throwing block into scoop bucket. At the same time, another employee was throwing a block into the scoop bucket and hit the injured employee with a block on right hand.

Roof fall occurred on old 6B-Belt @ 5 BLK. Fall was discovered by weekly mine examiner and MSHA inspector. Area was previously bolted with 6' resin bolts on a 4'x 4' pattern and two (2) 16' cable bolts spaced every other row. Dimensions of fall are approximately 16'W x 18'L x 20'W.

Accident type, without injuries

Roof fall occured on 13-Belt @ 43 Blk. Fall was discovered by weekly examiner and MSHA inspector. Area was previously bolted with 8' tension bolts on 4x4 pattern and two (2) 12' tension bolts spaced every other row. Roof to floor support was previously set at fall area on 6-9-15 due to floor hoofing. Area also borders a large protection zone block for an abandon gas well.

Caught in, under or between a moving and a stationary object

While rounding a turn to get to the continuous miner, a shuttle car struck the rib, causing the bumper of the car to pin the worker between the shuttle car and the rib.

Struck by falling object

While working on 14 Belt head drive, a concrete block, which was secured to the roof by plaster, let loose, striking him on his lower right arm.

Struck by rolling or sliding object

While roof bolting, a piece of coal from the rib rolled out and struck him on the back of his left leg.

While roof bolting, a piece of rock fell out of the top and struck him in the face and eye lid, causing a laceration to both his face and eye lid.

While helping the bolter operator, he got his left foot caught by the boom of the bolter, causing dislocation of his left big toe.

Roof Fall @ 8 BLK on Submains section MMU-007-0 in 4-5 to 6-7 entries in the face. Approximate size of fall is 100' in length x 16-17' in width, and 6'-12' high. Area was bolted with 6', 3/4"" tension rebar bolts. Test holes, a minimum of 8', were drilled every 20'. The cover above the fall is approximately 590'.

A roof fall occurred on the West Sub-Mains Section (#5 Heading, #44 Block in intersection). The fall area was approximately 12'-14' high and 20'-25' long. Prior to the fall, the area was bolted with 8'-12 combination bolts and spotting 20' combination bolts in the intersection. Going around gas well block and had over 900' of cover.

A roof fall was located on the #2 South Section (#18 Block, #4 Heading). The area of the fall was measured at approximated 16' wide x 20' long x 25' high. The area was bolted with 6' tension and 12' cable bolts. A shuttle car was under the fall area.

The employee was running the miner when a piece of the brow fell out, striking him on the right leg.

The employee was dragging a piece of curtain and stepped in a hole causing a fracture to his right lower leg.

A roof fall occurred on the Sub-Mains section (#13 Block, #23 Belt, #2 Entry). The fall area was approximately 16' wide and was bolted with 6' torque tension bolts and 12' cable bolts. Cribs were set at the fall location. No equipment, ventilation, or persons were affected.

Caught in, under or between two or more moving objects

While riding on a mantrip, the vehicle passed between another mantrip and the rib. The employee pinched his right arm between the two mantrips, resulting in multiple fractures to the arm.

A fall was discovered in the Sub-Mains Section (#4 Head, #13 Block) on the #23 Beltline. The area of the fall measured approximately 20' x 17' x 17' and was bolted with 6' torque tension bolts and 12' cable bolts. No people, ventilation, or equipment was affected.

A fall occurred at #58 Block on the #6 Belt in the #4 Heading. The fall area measured 20' high x 14' wide x 18' long. The area was bolted with 6' tension bolts and 18' cable bolts. It was later rebolted with the same type of bolts. No personnel, ventilation, or equipment was affected.

While pushing a roof bolt up to the roof, the employee thought he had the bolt head in the bolter chuck. However, the bolt was not in the chuck, which allowed it to slip, striking the employee in his right forearm.

The fall occurred on #1 South Panel (A Panel, #6 Block, #7 Heading)--between A3 and A4 inlets. The fall was 18' x 16' x 8'. The mine is monitoring A1, A2, B1, and B2 on a daily basis, and findings are being entered in the weekly book. The area was bolted with 6' tension and 12' cable bolts. The mine is preparing to seal the area.

on 7 north section G2 panel the intersection fell in at 9 brk. The area was bolted with 6' tension bolts and 12' cable bolts. Top was laminated sandstone with a cutter in it noticeable after the fall. fall was aprox 12' and 17' wide. No ventilation affected and no injuries

A roof fall occurred in the intersection at #6 Heading, #69 Block, and #6A Neutral Heading. The fall was reported to MSHA at 11:57am. The area was mined in February 2008. The fall area was approximately 14' high and 16' wide.

The employee was crossing over the tailpiece when he slipped and twisted his left knee. The employee returned to regular work duties the next day. During a follow-up visit to a physician, it was determined that the employee would need surgery to repair a torn ligament in his knee. He began missing work on 4/16/2012.

While employee was on one knee bolting, a piece of coal/rock fell striking his ankle, causing the injury. The employee stated that he examined his work area prior to the start of his task, and he deemed the area safe for work.

Roof fall occurred in intersection #3 HDG on 7N C-panel, area was bolted with 6' tension bolts and 12' cable bolts, area also had been center cribbed in various places, with 550' of cover. Fall was approx 14' high and 16' wide inby spad #9040.

The employee was fixing a hydraulic hose on a mantrip when the wrench slipped. The employee hit his hand on the oil pan, causing a laceration on his right hand. Stitches were required as treatment.

A buggy cable struck the employee in the head, causing his head to strike the shuttle car canopy. The canopy cut his right ear. Stitches were required as treatment.

He was servicing the miner. The miner bits got caught on the line curtain, so he shut the miner off and tried to get the curtain loose. Since he could not free it, he started the miner to drop the head. He placed his left hand on a bit lug. Instead of lowering the head, he mistakenly raised the head, smashing his left hand between the roof and the miner head.

The employee stated that he was leaning over the scoop batteries to let the bolter cable down. The roof bolter operator was tramming the bolter around the turn, and the pad of the ATRS roof support struck him, pinning him between the scoop batteries and the ATRS.

The employee stated that while he was putting the roof bolt into the chuck, a piece of rock fell and struck him on the hand. The accident resulted in a broken left pinkie finger.

A roof fall occurred in #3 entry 5A intake in primary escape way at 39 blk. area was previously bolted with 6' resin bolts and 12' cable bolts and 4' cribs had been set on each side of intersection. Primary escape way was rerouted to #2 HDG and Heinzman bars and jacks were set at 39 BLk #2 HDG on 7/12/11 on 2nd shift.

The employee stated that he was jacking up the tailpiece to put a block under it. The jack slipped out, smashing his thumb. Stitches were required as treatment.

While running the continuous miner, a piece of rib rolled out and struck the miner on the back of his right leg and ankle. The employee began missing work on 20 June 2011 as a result of the injury.

Employee was shoveling belt line when another employee put a rock on the belt and it flew off and hit EE on the forehead.

Area was already bolted with 6' tension bolts and 12' cable bolts every 8' fall occured in the #4 entry at 37 block on the 7 North Submain area. Cribs set on the inby and outby corners and crosscut 4-3 had been center cribbed before the intersection fell in.

Struck against a moving object

The employee stated that he was backing up on a small ride, and the front tire hit a rock. The steering wheel turned quickly and the steering knob struck the back of his left hand.

Was cleaning loose material from around cement pump feed area when the auger caught his glove.

An unintentional roof fall occurred in the old #5 North panel in the #6 entry at Spad #6185. The fall is approximately 38' long by 17' wide and 12' high. The fall occurred at a Pittsburgh pillar break. area was bolted with 6' resin bolts. Mine management called MSHA hotline to report.

The area was bolted on advance with 6' fully grouted resin bolts. Some areas around the fall were cribbed previously. Fall varies from 6' to 7' high by 17' wide by 18' long. Approximately 500' of cover.

The area where the roof fall occurred is a bleeder travelway for the #3 North Panel inby the 3N, #6 Evaluation Point and proceeded up the entry towards Spad #5685. The area was bolted on advance with 6' fully grouted resin bolts. Some areas around the fall were cribbed previously. Fall varies from 6'-7' high x 17' wide x 18' long, approximately 500' of cover.

The fire boss found the roof fall on the South Mains B Section. The fall was approximately 80' x 16' x 13'. The cover was approximately 230' in the area of the fall, and the top was laminated shale and sandstone. Area was bolted with 6' fully grouted resin bolts with 12' cable bolts installed sporadically in the area. The fall occurred on an idle shift.

The employee stated that while traveling on a small ride, the tire hit a rock that was lying on the bottom. When the ride's tire hit the rock, it caused the tires to turn to the right. As the steering wheel quickly turned to the right, it flew out of the employee's hand. The steering knob hit him on the back of the hand and injured his right hand.

A Pittsburgh second mining break was within 100 feet of the roof fall location. Adverse conditions were encountered when the cross cut from #5 to #6 Entry was turned on the South A Section at #12 Break. There was 380' of cover present, block size was 60 X 60 feet.

Contact with electrical current

Crew was completing a power move, employee was hanging the feeder cable when he received an electrical shock to his right hand.

The injured was washing out the reducer on the feeder with a water hose and he pinched the end of his finger between the hose and the frame of the machine.

Over-exertion in lifting objects

Employee was stacking crib block, he was lifting and turning without moving his feet and sprained his left knee.

Geologic conditions consisting of grey shale top mixed with stack rock. Cover of 330', area was bolted with 6' Fully Grouted Resin Bolts with hole boards and 12' non-tension cable bolts as supplement. Fall was approximately 16' by 16' and 10' in height. Notified MSHA hotline.

While handling a miner cable, the employee felt pain in his lower back. Please not: This employee did not begin missing work for this injury until 3/11/2009.

X-cut 2 - 1 at 7 brk s m section #4 panel fall is approx 7' - 9' high, 17' wide & approx 45' long. Cover 390' area bolted with 6' resin bolts & hole boards. A slip at 8' was visible after fall occurred. X-cut will not be cleaned up. Called State & Federal hotlines.

EE bent a roof bolt during installation. The head of the bolt stuck in the chuck of the bolter. The EE was working his drill head up and down to try and free the roof bolt. He accidentally hit the rotation lever, causing bolt to spin and strike his arm.

3 North section #3 HDG. 11-12 blk. while employee was hanging line curtain at face a rock fell, stirking employee on his left foot. He was on his knee at the time of accident.

#3 Hdg., 6 to 7 x-cut. Mains North. The injured was working beside a roof bolter when a rock fell out between bolts in #3 Hdg 6 to 7 x-cut. The rock struck the individual in the back of the head, knocking him face first into the machine. The area was bolted with 6' resin bolts and employee was getting ready to install a 12' cable bolt.

On 3-n sect. #4 inter, 4 BL. 100' inby spad 5138 Fell in approx. 15-20' high 16'w x 20'L top was gray shale laminated. Water was evident in top after fall. Cover of 270' area bolted with 6' resin bolts and 12' cable bolts. x3-4 was cribbed off also. Fall will be cleaned up and heizman canopy installed called MSHA hot line.

The bolter wrench was stuck in the head. Injured was using channel locks to remove the stuck wrench. While jerking on the channel locks, injured twisted his wrist. He continued to bolt for the remainder of the shift. (Bolting helper-side Fletcher RRII). He went to the hospital later for treatment after wrist began swelling and pain increased.

Injured was operating fletcher twin boom roof bolter. Drill steels dropped out of roof and hit EE in middle finger of right hand.

While removing a wheel unit from shuttle car, employee had a breaker bar on a wrench. He stood on the breaker bar which slipped off the wrench and struck him on the leg.

Injured was turning to walk away from roof bolter when he got his foot wedged between two rocks and twisted his right inside leg near knee.

On 2-South #4 hdg - 7 bl fall approx 6'-7' high x 17' w x 40'-60' in length. Cover approx 250' area bolted with 5' resin bolts. The hdg was mined between a full, partial pillared block & solid block of Pittsburgh coal causing roof condition problems in seam. Affected area has been rebolted with 10' & 12' cable bolts & 6' 3/4"" resin bolts & boards. Hdg width narrowed down also.

While EE was installing a cable bit. His drill steel got stuck in the roof while trying to remove drill empty, he cut his finger between drill steel and bolt chuck.

While operating operator's side of bolter, drill head went up higher than the canopy and got wedged onto canopy. Employee used left hand to separate head and canopy causing hand laceration.

Thsi area hdg has a roof fall 20-30' inby this fall which has fallen in has a60. we pooted rib prior to fall since fall has installed 10' cable bolt in this area fall approx. 20' & 17' wide cover 150'. fall is 4' high.

EE moved miner from X 8-9 to #8 inter. He stopped the miner to hook up rope to miner cable handler to drag miner slack to face of #8. When he bent down to get rope, felt miner up against him, causing cable handler to puncture his R. thigh. EE was not tramming the miner. Miner was checked & left-side IVS changed. Also, small roll in bottom observed in #8 hdg.
