Showing all 25Ee dislodged a piece of draw rock as he was installing a ventilation curtain. The rock measured 1'x5"x1". As he drove a nail into the curtain, the rock came out on an angle and struck him in the head, requiring 4 stitches.
EE stated that he was roof bolting when a piece of rock fell from the roof and struck his side and ankle on the left side. EE was x-rayed and x-rays are negative.
EE was swinging the head of the roof bolter machine to the next position. He had his right hand on the top of the roof bolter when a piece of rock fell striking his little finger causing crushing injuries. He was treated at Harlan hospital and told to come back 6/21/06 for amputation.

EE stated that he was roof bolting when a piece of draw rock fell and struck him on the right hand. He was xrayed and showed no fracture.
Employee stated that he was in a previously supported area spotting a roof bolt when a piece of drawrock approx 1' x 3' x 4" thick fell and struck him in the back. He was transported to the hospital where he was x-rayed and diagnosed with a bruised back.
EE was reaching for roof bolt plates when a small piece of rock fell & struck the middle finger of his left hand. He was x-rayed & did not have any broken bones, nor stitches. He was to return to work on the next shift. He did not return to work. Efforts were made to contact him & were unsuccessful. He has now contacted us & is alleging a back injury from this incident.
Employee was roof-bolting when a piece of draw rock fell from the roof and slid down his back. He refused medical treatment and missed work the next two days. He returned on 5/5/05.
EMPLOYEE STATED HE WAS ROOF BOLTING WHEN A PIECE OF ROCK 2'X 3'X 6" FELL & STRUCK HIM ON THE RT. LEG. EE WAS TRANSPORTED TO THE HOSPITAL WHERE IT WAS DETERMINED THAT HE HAD SUFFERED A BROKEN LEG.
EE STATED THAT HE WAS PUTTING SUPPLIES ON THE ROOF BOLTER WHEN A PIECE OF DRAW ROCK FELL AND STRUCK HIM BEHIND THE RIGHT EAR. THE INJURY CAUSED A LACERATION BEHIND THE EAR REQUIRING 13 STITCHES. EE MISSED WORK ON 11-19 AND 11-20 THEN RETURNED TO WORK ON 11-22-04.
EMPLOYEE HAD COMPLETED ROOF BOLTING OPERATIONS AND WAS IN THE LAST OPEN X-CUT WAITING TO GO TO ANOTHER PLACE WHEN HE SAT DOWN ON THE TOP OF THE ROOF BOLTER. A PIECE OF ROCK MEASURING 18" X 24" X 3" FELL FROM THE ROOF AND STRUCK HIS RIGHT SHOULDER.
Ee stated that he was outby the last open x-cut when a piece of draw rock approx 2 feet x 2 feet x 1/8 inch thick struck him on the back. He continued to work until 6-28-04 when he consulted a Dr.

The rock struck him on the right arm. Ee was transported to the hospital where it was determined that he had a fractured arm.

EE WAS SHOVELING THE BELT LINE WHEN A PIECE OF ROCK ABOUT 1" X 7" FELL AND STRUCK HIM IN THE BACK. HE WAS TRANSPORTED TO THE HOSPITAL WERE XRAYSWERE NEGATIVE.
EE STATED THAT HE REMOVED HIS HARD HAT TO REATTACH HIS LIGHT CORD, WHEN A PIECE OF ROCK APPROX 1' IN DIAMETER FELL FROM THE ROOF AND STRUCK HIM IN THE HEAD CAUSING A LACERATION REQUIRING 7 STI TCHES.
EMPLOYEE WAS PUTTING A BREAKER SWITCH IN AT THE POWER CENTER WHEN A PIECE OF ROCK FELL & STRUCK HIM ON THE TOP OF THE HEAD.
EE STATED THAT HE WAS CHECKING A HOSE THAT WAS LEAKING WHEN A PIECE OF DRAW ROCK FELL AND STRUCK HIM ON THE LEFT HAND CAUSING A LACERATION REFQUIRING 13 STITCHES & FRACTURED FINGER
EE STATED THAT HE WAS WALKING THROUGH AN INTERSECTION WHEN A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HIM ON THE BACK. EE STAYED UNDERGROUND UNTIL THE END OF HIS SHIFT. EE WAS EXAMINED AT T HE HOSPITAL AND ALL XRAYS WERE NEGATIVE.
