Struck by rolling or sliding object

Employee was cutting plates/ties loose from track, with a torch, to remove it from the mines. Once the track was cut loose it rolled over on the employee's feet. Bruising his left foot.

A scoop was cleaning a crosscut to permit travel down the beltline, the scoop bucket hooked into a piece of 3"" water line, causing the water line to swing around and strike the employee on the lower right leg.

Struck by flying object

Employee was lifting a belt roller with a slate bar while another employee was trying to adjust the height of the roller. The slate bar broke striking the employee in the groin area.

Accident type, without injuries

A roof fall occurred in the last open cross-cut, in the #5 entry, on the 003 section. The fall was approximately 25 feet long, 25 feet wide, and 6 feet thick. No persons or equipment involved.

Struck against stationary object

Employee was working a head drive when a water line busted. He turned away from the water line and began to run away when he ran into a guide roller that was positioned beside of the belt line.

EE was operating a Joy SC10 Shuttle Car when this accident happened. EE was getting off of his shuttle car when he stepped on the ground (bottom) the bottom was uneven, causing him to twist his knee.

Struck against a moving object

The coal truck driver was leaving the mine pit area which had more coal accumulated than normal and the pit area was tight. The loader man directed the truck driver on where to back in, loaded him and then told him to pull forward. He pulled forward approx. 25 feet and turned over on the right side.

Struck against stationary object

Employee was preparing to start drilling the hole for a roof bolt when he slipped and hit the rotation lever, causing him to twist his wrist.

Struck by falling object

As ee was standing in intersection of the last open crosscut, a piece of draw rock approximately 4'x6'x5"" fell, striking him on the back and left side.

Injured was hanging a roof bolter cable, as the roof bolter was tramming tension came into cable causing him to cut his finger on the insulated hanger, requiring 2 stitches.

Employee was operating the miner off the umbilical cord. As he placed it on the tail of the miner so he could tram to the next place, the cord became intangled in the conveyor chain and as he tried to free up the cord, his middle & ring fingers on his left hand were mashed by the conveyor flight.

Ee was loading bolts, plates glue and straps onto the back of a roof bolter when his left hand struck the sharp edge of a strap, causing a laceration that required 4 stitches.

While trimming conveyor belt at the tailpiece employee's razor knife slipped, cutting his lower thigh.

Fall to the walkway or working surface

Employee stated that he slipped and fell, striking his back, as he was moving the tie off for his shuttle car cable. This alleged accident occurred on 2/8/07 but was not reported until 2/12/07.

Fall onto or against objects

Employee stated he was crossing the tailpiece and fell, hitting the starting box.

Fall to the walkway or working surface

EE was conducting preshift exam, when he slipped and fell. The bottom was wet and slick. EE seeked medical attention and advised to wait and watch. Became a reportable accident when EE had surgery on 6/8/07.

Struck by falling object

Ee dislodged a piece of draw rock as he was installing a ventilation curtain. The rock measured 1'x5""x1"". As he drove a nail into the curtain, the rock came out on an angle and struck him in the head, requiring 4 stitches.

Ee was repositioning the roof bolter when he caught his hand between the rib and the canopy of the roof bolter.

EE stated that he was changing conveyor belt structure out when he felt pain in his lower back. Ee went to the E.R.Dr where it was determined that he had suffered a lower back strain.

Caught in, under or between a moving and a stationary object

EE was swinging the head of the roof bolter machine to the next position. He had his right hand on the top of the roof bolter when a piece of rock fell striking his little finger causing crushing injuries. He was treated at Harlan hospital and told to come back 6/21/06 for amputation.

Employee was using a cut off saw when the blade broke & struck him on the left forearm. He was transported to the hospital where he received 5 stitches. He did not lose any time and returned to regular duty.

EE was attempting to straighten a piece of roof bolter drill steel that was bent, by lowering the head of the bolt machine on it. The drill steel sprang out and struck EE in the face. EE was X-rayed and had received a fractured cheek bone and jaw.

Ee stated he was using a shovel to muck the belt when he shoved his shovel up his thumb struck the mounting bar of the scraper. Ee went to the Dr., xray and tests were negative however, ee was put on light duty. Ee has not missed any time from work but is still on light duty.

EE stated that he was rolling a piece of rock off the water line when he felt pain in his right shoulder and neck.

EE WAS OPERATING THE ROOF BOLTER WHEN HE HAD PLACED HIS LEFT HAND UPON THE CANOPY OF THE ATRS. HIS FINGER WAS PINCHED BETWEEN THE MINE ROOF AND THE ATRS. HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE RECEIVED 3 STITCHES. THE HAND WAS X-RAYED AND SHOWED NO FRACTURES.

Employee stated that he was roof bolting & caught his left index finger between the drill steel and the roof bolting wrench. He was transported to the hospital where it was determined that his finger was broken & excessive tissue required surgery to repair the damage.
