EE WAS LOCATED ON THE OFF SIDE OF THE BELT TAKE-UP GETTING READY TO PULL BELT SLACK IN WHEN A PIECE OF COAL RIB BROKE LOOSE & FELL OUT STRIKING HIM ON THE RIGHT ANKLE & FOOT. THE MATERIAL MEAS URED 5' X 14" X 18".

SECTION FOREMAN WAS ASSISTING CO-WORKER IN VENTILATING SECTION WHEN A PIECE OF WORK FELL FROM THE MINE ROOF AND STRUCK HIM ON HEAD AND KNOCKED HIM TO THE GROUND.

KETTLE BOTTOM FELL FROM BETWEEN BOLTS STRUCK HEAD AND SHOULDER(RIGHT). -RESTRICTED DUTY.
