Employee was throwing roof bolter cable out of the way of the bolter when a rock fell and struck him on top of his head and left shoulder.
Employee was installing cable bolts in #3 Entry. He was drilling when the rock between bolts fell and struck his left foot.
Employee was drilling 60" hole to install roof bolt when a loose piece of roof fell, striking his left palm.

Employee was installing his second section of drill steel in the drill pot when a piece of rock fell from the mine roof. The rock struck his right hand & smashed his right middle finger between the rock and the drill pot of the roof bolter.

EMPLOYEE WAS ROCK DUSTING WHEN A PIECE OF SLATE MEASURING ABOUT 12" L X 5" W X 3" THICK FELL OUT FROM BETWEEN BOLTS STRIKING EMPLOYEE'S LEFT ELBOW.
A ROCK FELL BETWEEN BOLTS & PUSHED THE CANOPY DOWN INTO THE SHAFT CAUSING THE CANOPY TO HIT THE EMPLOYEE IN THE TOP OF HIS HEAD. ROCK THEN FELL OFF OF CANOPY & STRUCK EMPLOYEE.
Cleaning on #3 belt drive. Rock fell striking him on left shoulder.
EMPLOYEE WAS USING BELT ROPE TO STEADY HIMSELF AS HE WAS WALKING THROUGH WATER WHEN A ROCK FELL & CUT HIS RIGHT INDEX AND MIDDLE FINGERS.
