Contact with heat

EE was working with another to complete non-hazardous drum pickup in direct sun. Started at noon until 5pm. EE was leaving site, had to stop at exit gate where he threw up several times. He felt better and went down the road where he began to have numbness in his extremities and shortness of breath 911 was called and he was taken to the ERT and admitted overnight and released.

Employee was drilling 60"" hole to install roof bolt when a loose piece of roof fell, striking his left palm.

Employee ALLEGES the mantrip went dead at the top of hill. It rolled back, he pushed the brake pedal down but it busted. He then set the park brake but it didn't work. The rear end of the mantrip went into the rib. The employee complained his lower back was hurt. We DON'T believe the ALLEGED accident occurred due to defective equipment but rather due to the brake lines being cut!

Were retreat mining, had an inexperienced miner operator and was not cleaning the cuts as he was mining. The rock fell on the miner head holding the head on the bottom. The miner could not pull out on it's own due to the fact that the miner had spun the loose coal out from under the cats and the machine saddle bagged on loose coal.

Employee's left foot was pinned between ride and rib when tram stuck in.

On 8/9/2011 the 3rd shift fireboss discovered a fall at 7:25 AM. The fall is located between spad #120 and 121 between the #6 and #7 entries. The fall measured approximately 18 ft wide x 40 ft long x 6-7 ft high. This area was bolted with 5 ft fully grouted bolts.

Employee states he got rock dust in his eye while freeing up the rock buster. He also states he was wearing his safety glasses.

Employee was in mantrip when it reached the top of the hill and the drive shaft yoke broke. Mantrip started backwards. As the wheels sped up, it jumped and cut into the rib causing it to stop suddenly.

Employee was operating shuttlecar when he reached out of the canopy to move a CO monitor line. He caught his right index finger between the shuttlecar canopy and the mine roof.

An unintentional fall was discovered on the 1 Northwest panel one break inby spad #526 in the #6 entry. this fall was discovered on a weekly air way examination. The entry width was 21' wide timbered down to 17'4"". The fall measured 20' wide X approximately 60' long X 8' high. the top was constructed of laminated slate and fell to the rider seam.

On 12/20/2011 an unintentional roof fall was discovered in the #4 entry of the #1 Southwest Panel between 49 and 50 break of the scoop road. On 04/19/2011 this area was showing some signs of top activity and we started adding extra support but did not get it completed. We were going to finish it on 04/20/2011. The fall measured approximately 70' long x 18' wide x 9' high.

At approx. 10:50 the evening section foreman called out to report a fall in the #3 entry of th e3 S.W. 2 left panel (east Section) on the pillar section. The fall measured approx. 18' W. X 30'+ H. X 80' L. There were 2 MRS's and 1 14-15 CM Miner.

Shuttle car cable pulled tight and at an angle. Employee tried to pick it up and hang it and a pain shot through left shoulder.

An unintentional roof fall occurred on the #3 Southwest Panel at #1 break in the #1 entry approximately 20 ft outby spad #1129. The fall measured approximately 80 ft x 20 ft x 7 ft high. Area was timbered and dangered off. This fall was discovered during a weekly air way examination.

An unintentional fall occurred on the second bench of the high wall between the return and portal entries. The piece of rock that fell measured approximately 25 ft long x 14 ft wide x 6 ft thick. In the fall two man trip barns and one 2 man personal carrier were damaged.

At 5:15 the CO detector at #4 belt head went off, the belt was shut down. Two workers were close by & responded to the area. Discovered smoke, called outside & reported findings. All men were evacuated via the intake. After smoke was cleared & further investigating, a rock was found in the tail with coal causing a spillage & the belt to run off in tail piece causing heat & smoke.

The employee was in the mantrip when it was going up the hill at BK #16. The mantrip made it 3/4 of the way when the drive line came out causing the mantrip to go backwards down the hill and stopping suddenly after striking rib.

A continuation of a fall that was reported on 8/25/2008 was discovered at 11:30AM on 12/17/10. The fall was located between spad #133 and #134 between #2 and #3 entries on the #1 Southwest Panel at break 19. The fall measured approximately 18' wide x 60' long x 7'-8' high.

Employee was carrying an Idler Bearing for a shuttlecar out to set it down on the mantrip. As he set it down, he felt a sharp pain in his lower back.

Employee was taking mantrip off charge when it started to roll and the power cord was pulled out of power center. Employee tried to stop it by grabbing the cord with his hand, jerking his arm and shoulder.

Employee was installing his second section of drill steel in the drill pot when a piece of rock fell from the mine roof. The rock struck his right hand & smashed his right middle finger between the rock and the drill pot of the roof bolter.

A roof fall occured in the intake at break #68 between #8 & 9 entry at the mouth of the 1 Northwest section. The roof fall was 60 ft long, 18 ft wide and 8 ft high. Re-routed the intake, breakered the roof fall off. The roof fall will not be clesaned up.

Employee was walking beside belt, where it was low. He tripped on a rock and fell into belt which drug him 30' down beltline. Began missing work 9/14/09, return to work 9/16/09, then out again on 9/18/09 and has returned on 9/23/09.

Employee was walking from 4 head to the road way through a break through when a rock approx. 4' round and 4"" thick struck his right shoulder.

MSHA inspector was making a routine inspection and came upon 2 falls in the old 1st right headings one break inby spad # 657, one in intersection of #3 entry and one in intersection of #5 entry. The fall in #5 measured approx. 20 x 50 x 8. The fall in # 3 measured approx. 20 x 30 x 8.

Employee was leaving roof bolter to go get on mantrip. He walked through the #2 intersection when he rolled his ankle, twisting it. The employee worked 1/2 of his shift on 2/25/09.

An unplanned roof fall was encountered on inspection in the primary escapeway on first right section. Location No. 5 entry inby spad No. 603. The fall measures 18 ft wide by 40 ft long by 5 feet thick. This fall came partially out into the intersection of the primary escapeway.

While operating bolter in #4 heading of 1st right section, employee was drilling a hole in roof to install roof bolt when he applied too much thrust with less rotation. This caused the drill steel to bend striking his right hand. He was holding onto steel with hand at this time. He had no deformity to arm or hand but did have swelling.

EMPLOYEE WAS PULLING A PIECE OF DRILL STEEL OUT OF THE TOP BECAUSE IT WAS STUCK. HIS HAND SLIPPED AND STRUCK THE CANOPY.
