Employee was putting in collar pipe at the face in the 2195. Employee was spooning the hole out when a rock fell from the face about 10 feet up and struck them in the back.

Employee was jack legging in our Julain mine. A baseball size rock fell between the wire from the back and hit the employee's left forearm cutting it. Employee received 7 stitches and got a full release.

On 26 May at approximately 6:30 AM a miner was struck on the left side of EE's face while scaling. Pieces of the falling debris made it into EE's eye causing a scratch on the surface of the eye. The on-site medic, under medical direction, treated the scratch with prescription anti-biotics. The employee was wearing safety glasses at the time of the incident.

While scaling, miner's left thumb was struck by small rock causing a slight fracture injury to end of thumb.

While scaling, a rock fell from the back striking miner's right hand. Miner was evaluated at the medical station and placed on light duty.

EE was rock bolting the roof when a piece of rock fell and struck him on upper right arm. Rock size approx. 2" x 4".

WHILE THE EMPLOYEE WAS ROCK BOLTING A SMALL ROCK BURST OCCURED IN THE BACK CAUSING A SMALL ROCK TO STRIKE HIM

The employee was in the process of rockbolting the back in a stop when a rock came down and struck him on the arm causing a small laceration.

EMPLOYEE WAS COLLARING A HOLE IN THE BACK WITH A JACK LEG WHEN A ROCK FELL FROM THE BACK (2"X6"X6") STRIKING HIM ON THE NOSE

The employee was drilling the back with a jackleg in a stope when a rock came down from the corner stirking him on the arm.

He was fixing a stuck skip when a falling rock struck him in the head.

WHILE IN THE PROCESS OF SCALING/BARRING DOWN THE EE WAS AND STRUCK INT HE R-ARM WITH A ROCK APPROX 4"X4".

AFTER A FRESH BLAST FROM A RAISE UP ROUND THE EE WAS BEGINNING THE TASK OF SCALING DOWN WHEN A ROCK FROM THE HANGING WALL FELL AND STRUCK HIM IN THE LEG.

THE EE WAS DRILLING THE BACK IN A RAISE UP ROUNDWHEN A ROCK CAME DOWN AND STRUCK HIM IN THE HEAD/ HARD HAT. EE WENT TO THE DOCTOR ON 6/26/02.

THE EMPLOYEE WAS BEGINNING THE TASK OF SCALING AFTER A RAISE UP ROUND WHEN SOME SMALL ROCKS CAME DOWN FROM THE BACK STRIKING HIM ON THE HEAD.

EE WAS LOWERING A LOCOMOTIVE ONTO THE TRACK WITHA COME-ALONG WHEN A ROCK FELL OUT OF THE BACK STRIKING HIM ON THE LEFT HAND.

EE WAS STRUCK ON THE LEFT FOOT BY A ROCK THAT FELL OF THE HANGING WALL WHILE REPAIRING SLUSHER CABLE.

EMPLOYEE WAS AT THE FACE CHANGING BITS WHEN A ROCK FELL STRIKING HIM ON THE LEFT LEG ABOVE THE KNEE. EMPLOYEE RECEIVED STITCHES.
