Struck by flying object

The investigation is not complete, but as of now it appears a miner was fatally injured when material that traveled through a borehole struck him after a blast was initiated. The victim was on the 1290 level ramp waiting for a blast to be initiated. The concussion from the explosion traveled through a 3-inch borehole, blowing small rock and debris onto the victim.

Got on the forklift to do a pre-check. Finished the pre-check. Door was latched open to the body of the forklift and when EE reached back to pull the door close EE heard a pop in EE's bicep.

Struck by falling object

two employees were putting a floor jack onto a flat bed one was lifting the other was on the truck lifting with a strap when they got it on the bed and went to set it down the jack shifted and the handle hit the employee on the truck in the head on the ear giving them a cut.

An employee sustained a toe fracture while working underground during diamond core drilling operations. The helper was in the process of lifting a core barrel into the drill head vertically when core barrel fell onto EE's foot.

Mechanic was climbing down ladder of a under ground haul truck when EE stepped off the ladder on to the ground EE's knee twisted. EE had a small tear in EE's meniscus. EE worked full duty until the date of EE's surgery in August. it took a a while to schedule because it was out of state and EE had to lose weight before the surgery.

On 12-6-24 Employee lifted a jack leg into the back of a pickup truck when felt a light pinch in groin. Employee got home 12-8-24 and felt some pain in groin and went to the ER and has an inguinal hernia. Employee will need to see a surgeon to see if surgery is needed.

While hand scaling, a rock rolled onto the employees ankle causing irritation.X-rays indicate no fracture but n MRI is being ordered.

Miner was descending out of the cab of a jumbo and when EE stepped onto the ground EE hyperextended EE's knee resulting in a torn ACL and meniscus. Miner was able to work until EE's pre-op appointment on 10/10/2024

Lifting ejector bed cylinder into place with prybar when the prybar slipped out and the cylinder fell, crushing EE's right middle finger between the cylinder and the frame of the machine EE was working on. Caused laceration and fracture of distal phalanx of the finger.

While changing a flat tire on pickup truck, employee injured back while lifting the flat tire.

A piece of falling equipment caused an employee to swiftly move out of the way. As the employee started to run, EE stepped on an uneven ground surface and fell. The initial x-rays reveal a fracture to the left heel.

The employee was lifting a chain anchor and twisted to put it on the barge. At that time EE had no pain. Later that night EE felt left lower back was a little tight. When EE woke up Thursday 3-28-24, EE's back was tight so EE went to the ER and the doctor said it was a severe back muscle strain. The doctor said it was a chronic back problem.

Miner was on a tractor and stepped over to sit on the seat and felt pain in left knee on 2-1-24. Still bothered miner and got it checked 4/6 release full duty got checked in home state 7-17 no work release until miner gets knee scoped. LTI will start 7-17-24

When an underground mechanic's foot slipped off the step as they were getting off a haul truck, they caught themselves using both hands and their other foot. They felt their right bicep pop as a result of doing this. They might have a torn ligament. Until they see an orthopedic physician the next week, they were put on light duty.

1-11-2024 An employee was scaling in the 2125, A small rock around 1"" rolled down the scaling bar and struck the employee's hand bouncing off and hitting EE's tooth causing a small chip. The employee stated it was a small rock and EE did not think about dropping the bar.

While leaving AAP's work area back toward the main road down the switchbacks the Operator lost Control of the Vehicle and struck the berm. The impact cause chest injury due to seat belt

The employee working underground fell off the emulsion tank. they were sent to the ER and they have some fractured ribs and a head laceration. we are in the process of doing a full investigation to figure out what happened and how to prevent this from happening again.

the employee was riding in a bus after the shift when EE turned to look to the side EE felt pain in EE's lower back. the employee said EE's back had been sore for a while but had no pain until the bus incident. Soft tissue strain in the lower back.

the employee was walking in the shop when tripped and stuck Left hand out to catch themselves on a box. Their hand caught a screw on the box and cut their hand. resulting in 7 stitches to the left hand.

Driving a forklift underground on hit a few bumps and got a sore back and no pain that day. on 5-7-23 their back started hurting so they got it checked out at the ER. The doctor let them finish the shift and see a ortho if not better in 2 weeks. on 5-31-23 they got results and got an no work release.

Teammate was attempting to throw a pickup tire mounted on a wheel, approximately 75lbs. onto a flatbed truck. The tire/rim struck the side of the bed forcing the tire back to the ground. The teammate attempted to stop the motion of the tire and push it onto the flatbed when EE felt the pain in their left arm.

While replacing burned out lighting, 2 employees used the drill mast as a means of working from heights. The drill was used in the similitude of a fireman's ladder to raise and lower the employee to reach the lighting mentioned above. This resulted in the employee's foot being caught in between the drill mast and the pedestal resulting in a crushing injury of the left foot.

A driller's assistant was attempting to load an empty core tube into the rod string when the rod string fell and caught EE's hand between the rod string and the hole in the decking. The event resulted in a crushing of the right hand inflicting trauma to all 4 primary fingers excluding the thumb.

The injured miner was trying to dislodge a 10' drill rod that had difficulty unthreading. During the process the worker tried to use vibration to assist with the unthreading and began banging the rod on the rod roller. In the process the worker's finger was caught between the 10' rod and the roller which resulted in a smashing of the ring finger tip on the left hand.

My knee popped upon exiting the vehicle while servicing equipment on 4-30-22. The employee just wanted to get it documented at this time and went back to work Knee was sore. 4-25-23 same injury 1 year later no mechanism of injury this time, went to the doctor and employee will need Surgery to replace old ACL hardware and install new hardware.

Employees were installing split set bolts and the helper put the next bolt in the next hole and put it in to the wrong hole. It fell out of the hole striking the jackleg operator in the left lower leg. Employee had to get 11 stitches.

EE felt the knee was a little tight. EE reported it on EE's last day of shift. After days off and a full shift, EE wanted to get it checked out. The doctor wanted to scope it because the MRI did not show anything. When doctor did this, doctor repaired EE's meniscus and ACL on 7-5-22. This was an old injury and not work-related. There was no mechanism of injury.

An employee was walking underground and twisted their ankle on a rock 3-15-22. Still had pain so went back to the doctor and got minor surgery on 8-3-22. EE will be on light duty and will be re-evaluated 8-15-22.

Employee was putting in collar pipe at the face in the 2195. Employee was spooning the hole out when a rock fell from the face about 10 feet up and struck them in the back.

Employee was moving a pallet jack on the portal pad when EE slipped and fell. EE had some light pain in lower back at this time. This was not a reportable at the time of injury. EE still felt pain on 4-18 and decided to get a cortisone shot and was feeling sick so just wanted to go home to recover. EE got a note from doctor.

The employee was getting a mine rescue physical when the doctor found a Pre-existing Hernia. The employee had no pain and did not have a clue employee had a hernia. The employee 4 months later decided employee wanted to have it repaired, employee did not want to take any chance of it getting worse.

While mucking the 1400 mpc round out the bucket of UL20 mucker caught the rib and stopped the mucker instantly jarring the operators neck and lower back. The employee went to the ER the day they reported it they wanted EE to be released by an orthoepic, the worker comp case manager could not get EE an appointment until 7-21-21 with an orthoepic.

Employee was jack legging in our Julain mine. A baseball size rock fell between the wire from the back and hit the employee's left forearm cutting it. Employee received 7 stitches and got a full release.

Employee was changing hose in the back of a fuel pick up truck, when the fitting broke lose EE lost EE's balance and stumbled back falling out of the back of the truck. EE's pelvis has a light fracture.

Driller was measuring drill hole and tape measure got stuck, when retracting it and broke when it broke free it cut glove and finger. Driller had to get 6 stitches.

I.P. was installing Insulated Metal Panels when I.P. lost their footing. As I.P. fell they grabbed on to the edge of the Panel that was going to be installed. The grabbing motion at the edge / corner of the panel tore I.P.'s glove and therefore cutting the right thumb inside the glove.

Employee was walking down the road by the pound at Comet water treatment plant carrying a spool of rope. As EE was walking EE slipped on some ice falling and twisting ankle. There was gravel on the road and EE was wearing ice cleats.

Miner slipped on Kabota tractor platform while mounting the vehicle. When the miners foot slipped it got caught between the platform and engine.

On 19 August at approximately 11:00 AM an underground mechanic sustained an injury to abdomen when a suspended load fell and contacted EE. The employee was in process of lifting a Kubota tractor axle with a crane when the load fell and struck EE. The load fell approximately 18 inches and weighed 150 lbs.

Fall from machine

EE was ascending the ladder to the grizzly feeder at the crusher. EE boots were muddy and ee lost footing on the rungs of the ladder and fell backwards about 5 feet to the floor below, landing on tail bone.

On 5 January at ~10:00 AM an underground haul truck operator twisted EE's ankle when stepping off an underground tractor. When the incident occurred the operator was moving the tractor that was located in a bay where EE wanted to stage EE's haul truck. When EE was climbing off the tractor EE stepped on uneven ground twisting EE's left ankle.

On 2 October at ~5:00 PM, a miner was struck in the arm by falling debris resulting in a small laceration on the left forearm. When the incident occurred the miner was leaning a loading pole against the rib. The rock fell off the rib from approximately 8 feet high.

On 19 August at 5:30 PM a warehouse employee cut finger while opening a 55 gallon drum. At the time if the injury the employee was using a crescent wrench to pry the lid off a 55 gallon drum. The outer edge of the lid caused a laceration to right index, middle and pointer fingers.

On 19 August at ~2:00 PM a miner cut finger while splitting stick powder. At the time of the injury the miner was preparing stick powder for loading blast holes in a stope shot. The miner was using a razor knife to split the powder when EE cut the tip of the left little finger causing a laceration requiring sutures.

On 26 May at approximately 6:30 AM a miner was struck on the left side of EE's face while scaling. Pieces of the falling debris made it into EE's eye causing a scratch on the surface of the eye. The on-site medic, under medical direction, treated the scratch with prescription anti-biotics. The employee was wearing safety glasses at the time of the incident.

An underground miner sustained a laceration to his left index finger while removing a split set bolt from a drill hole. The injured miner hit the bolt plate with the mine axe causing the monster plate to turn. When the monster plate turned, it came into contact with the miners left index finger causing a laceration.

On 21 July at approx. 6:30 AM an underground loader operator experienced pain in his lower back radiating down the left leg. The employee first felt the pain after exiting his loader at the end of night shift on July 21st. The employee was evaluated by onsite medics and later seen by a doctor at the local emergency room. He was diagnosed with sciatica and lower back pain.

On 16 May at approximately 3:15 PM a surface ops worker was cleaning sediment containments with an excavator at the Comet Water Treatment Plant. The task required opening sediment bags to gain access with the excavator. While opening a sediment bag the hooked blade razor the employee was using came in contact with his left index finger causing a deep laceration.

The operator was heading underground and pulled into a passing bay. The operator heard a rattling noise behind his seat near the heater fan panel. The operator turned slightly to his left and reached behind him. When he reached behind him he stuck his left hand into an open hole where the vent cover was missing when his left middle finger made contact with fan blades.

As the drill crew was pumping a overshot down, a burr caught above the stuffing box and started sheering. The driller decided to cut out the bad spot. Driller stopped pumping and cut the wire line. He jammed the wire line with a pipe wrench and his thumb got stuck in the spot behind adjusting nut on the pipe wrench and the hole sucked the wrench against the rod trapping his thumb.

Employee was operating a snow mobile traveling to retrieve water samples, as employee headed off road which was blocked by a large snow drift. Employee was forced to take an alternate travel. When the machine traveled the berm the snow mobile slide off the berm and rolled down hillside.

Once we were notified of the incident he was sent to the medic who did not notice any swelling, redness or tenderness. EE was operating a loader and got out to do some work on the ground. EE got off the loader and was working on the ground when he stepped backward tripped on a rock landing on his back. Resulting in cracked ribs.

Employee was jack legging in order to anchor the feed frame to the face. Employee was trying to kick the plate on the rock bolt with his left foot to align it. When employees foot came in contact with the plate there was a pop in his back which contributed to the injury.

EE was stacking powder boxes for the crew and stacked them too high and the boxes fell on EE who stacked boxes in the powder mag. EE Strain her back.

EE allegedly was climbing off a haul when he slipped, claiming he had hurt his shoulder from fall off the truck. EE left work to the ER and the PA said he had to be on restricted duty until he could be diagnosed by a specialist. EE was diagnosed on 5/27/11 for a slight tear in left shoulder by the Specialist.

EE was working on the rear light of a pickup, when the pickup rolled over the wheel chocks and pinned him between the rear bumper and the front bumper of another vehicle. EE fractured his pelvis.

EE was working on another task which required only half face respirator. The ee decided to enter an separate area and help with an operation that the company required a full face respirator. The crew was loading a bag full of dry cement mix into a hopper, as the bag was opened the employee got dust on his face and dust fell into his eyes when he removed his goggles.

While scaling, a rock fell from the back striking miner's right hand. Miner was evaluated at the medical station and placed on light duty.

While working, a water plant operator was kneeling when a coiled 6"" flex hose struck his hard hat from a distance of approx. 2-3 feet because of co-workers working above handling the line. Operator states the impact knocked his hard hat off. Operator went for medical evaluation where he was placed on light duty for minor upper back pain.

Employee was shotcreting a fan bulkhead, when a piece of steel plugged the nozzle causing pressure to build up in the hose which blew the nozzle off. When the nozzle came off the emloyee's wrist was bent back causing a minor sprain with some small abrasions.

Employee walking down to sump and stepped in a hole next to a tree. As he fell forward and tried to catch himsef, he struck the tree stump with the index finger of his right hand resulting in a broken finger.

Injured employee was observing a concrete pour. A forklift was moving a pallet of sandbags, and as the operator began to back up, one of the sandbags started to fall from the pallet. The injured employee and another employee attempted to reposition the sandbag on the pallet, and he felt immediate pain in the lumbar region of his back.

The employee was obtaining training on how to change out a bag filter at the water treatment plant. During the task the employee was removing his hand from the bag canister when the lid fell on his right hand, striking his right ""pinky"" finger.

The employee was drilling a hole between two aluminum tanks when the drill bit became caught on the aluminum. The energy was transferred into the drill handle causing his hand to be slammed into an edge of the aluminum tank. He sustained a laceration to the top right hand requiring six sutures. After treatment he was returned to full work duty.

Slip/fall on icy road; at approx 6:30pm, EE left the dining area and was walking to the STP to start her night shift for that evening. As EE was crossing the road, she slipped and fell on the ground. She realized she had injured her ankle and she immediately reported to the medic.

The employee was loading a small trash pump into the bed of a light vehicle when his finger was caught in a pinch point between the small pump and the truck bed. The employee sustained a laceration to the left forefinger which required six sutures. After treatment onsite the employee was returned to full duty work.

While installing resin bolts with 4 foot sticking out to accommodate the installation of an overhead crane, the employee got his hand in a pinch point and received a 1.5 inch laceration between the thumb and first finger. The wound was closed with steri-strips and was diagnosed to have a possible infection.

Employee was sitting in the seat of an Ingersol Rand extendable forklift with her left hand on the ROPS mounted hand hold, when the wind blew the door shut and smashed her left index finger. One bone in the finger was fractured and a laceration was caused that required sutures.
