A Mine Production employee was in the process of bolting at 8 BMR. Employee noticed a bad top in the entry and exited the cab of the bolter to bar down the hazard. As employee barred down, the arch behind the employee came loose and fell and struck the employee from behind.
The miner operator stopped mining, swung the tail toward the right rib and exited the cab. It is unclear why EE exited the cab of the miner. EE was a couple feet from the operator's compartment when the arch fell, striking EE in the head/neck/shoulder/upper back area. The injured employee was provided first aid in the mine and taken to the Memorial Hospital of Sweetwater County.
A bore miner was parked in an area under roof support to replace a shear pin. The roof was checked visually and appeared to be in good condition. A 5"x10"x1-3" thick piece of the roof, in front of the bore miner and inby roof bolts, separated, broke apart and fell. A piece of the material hinged, swung toward the miner, and hit EE.

The employee was performing a visual inspection of the area; walked to the back of the mining machine to un-hook the water hose and retrieve the remote for the continuous haulage. As ee was walking back to the bolter station, a roof slab fell hitting the mining machine, the load out tail of the miner and the employee.
A Miner Operator was walking between the rib and the bore miner, when a section of roof (approximately 6"x6") separated from the mine roof and fell striking the tail section of the miner. In doing so, a fragmented piece 18"x6" struck the employee on the side of their head, shoulder and arm. This resulted in the employee sustaining a laceration to their right triceps.
While barring down roof, a piece of trona slid down a scaling bar striking his right hand lacerating his right index finger.
An employee was operating a roof bolter in the 11 East 11 South 3 BMR section, he was extending to place the bolting resin in the hole the mine roof fell and a rock struck the employee on the right shoulder. The employee was initially given a full release to work. On 2/7 he was diagnosed with a torn rotator cuff, and been placed on restricted duty on 2/9 awaiting surgery.
An employee walked up into the room and was standing next to the conveyor boom behind the cab of the miner. He was positioned partially under the roof bolts along the right side of the entry. As he turned to leave, a section of the roof separated up to the bolts and a portion struck the employee causing a laceration on the right side of his head which required four sutures.
An employee was struck by a rock which dislodged from the mine roof. Ventilation tubing was needing to be extended and the employee stepped up to the miner to retrieve a drill so a spad hole could be drilled to secure the ventilation tubing. As the employee stepped back away from the miner, the rock fell from the roof striking him in the head, neck and back.
On September 29th an employee was operating a roof bolter in 2 Bore Miner, when a piece of trona separated from the roof and fell, striking the overhead canopy. The piece of trona fractured into smaller pieces and a piece struck the employee in the left hand, causing a laceration. The wound required 10 sutures, thus making this a reportable injury.
While driving a forklift on 11 E towards 2S, a rock fell from the top and struck the employee on the shoulder and cheek. This caused a laceration on his cheek that required 16 stitches. He did receive a full work release.
On June 21st an employee was operating a roof bolter installing a bolt in a roof pan, a rock measuring 19"x13"x3" came loose, striking the employee in the left shoulder. The employee sustained bruising to the shoulder. This continued to cause him pain thru Sept. He had surgery on 10/22 and the injury was determined work related on 11/9/07.
Mine Production employee sustains lacerations to ear and head and fracture to ankle as a result of being struck by a piece of rock falling from the roof.
While drilling the fourth bolt hole the drill steel got plugged and the employee put his hand on the top of the steel to see if it was stuck. A rock fell from the top and stuck the employee's hand as it was on top of the bolt. He was taken to seek medical attention and has a fractured middle finger and received 10 stitches for the laceration on his left hand.
The employee stepped under the tail of the miner to check the next bolting pattern with his foreman. Before he made it back under a bolt, a rock 20'x3' fell from the top and struck the employee in his left shoulder. He was taken to seek medical attention and received stitches. He was released to full duty.
Employee was fire bossing, he went into #5 room to fire boss. Employee turned around to help with cable; a rock fell from between the bolts and struck the employee in the back of his head, shoulders, neck and right shoulder blade. He was taken to the emergency room and flown to LDS hospital in Salt Lake.
EMPLOYEE WAS HANGING THE REMOTE ON THE HAULAGE WHEN A ROCK 4'X 6'X 5' FELL & STRUCK HIM IN THE BACK, HEAD, LEG AND FOOT. IT KNOCKED THE EE INTO THE HAULAGE & BOLTER. HE IS SEEKING MEDICAL ATTN.
EE WAS WALKING OUT OF DRIFT WHEN A PIECE OF BACK FELL HITTING THE EE IN THE LOWER RIGHT LEG RESULTING IN A LACERATION REQUIRING SUTURES. GROUND CONTROL MEASURES WERE IN PLACE, I.E. HAD BEEN IN SPECTED, ROOF BOLTS.
WHILE HANDING ROOF BOLTS TO AN EE, A ROCK FELL FROMT HE ROOF STRIKING EE IN THE HAND. THIS RESULTED IN A CONTUSION TO TWO FINGERS.
A ROCK FELL AND STRUCK EMPLOYEE ON THE HAND, RESULTING IN A FRACTURE. EMPLOYEE WAS PLACED ON RESTRICTED DUTY ON 9/1/00.
DURING A ROOF FALL, EE WAS SHOWERED WITH ROCKS. HE WAS TRANSPORTATED TO HOSPITAL WHERE HE WAS TREATED AND RELEASED. UPON READING THE X-RAY, THE DR. DISCOVERED A HAIRLINE FRACTURE AND THE EE WA S PLACED ON LOST TIME EFFECTIVE 2-1-00.
