Struck by falling object

The separation of the rib/arch struck the employee

Accident type, without injuries

While lifting the longwall crusher into the cage using the overhead crane, the crane faulted. Had to get equipment to replace the fuse to continue loading and hoisting the crusher.

Absorption of radiations, caustics, toxic and noxious substances

An employee was conducting an inspection of the burner in DR-8, Argo Kiln. The employee was wearing a Tyvek dust suit, N95 dust mask, gloves, a hard hat, and safety glasses. As they were exiting the kiln, a small piece of dried soda ash struck their hard hat, and dust from the soda ash made contact with their left eye.

Fall to the walkway or working surface

Employee activated the spring return on the lubrication hoses and became tangled in the hose and fell.

Fall onto or against objects

An employee was standing on a 17 inch two step working platform performing maintenance work. As EE applied force to the box end wrench, the wrench slipped causing EE to lose balance and fall. In falling, EE's right knee contacted a ceramic liner, resulting in a fractured knee cap.

Struck by falling object

The employee was climbing down a scaffold ladder. Employee was tied off to an overhead SRL while descending a ladder. Their personal srl got caught on the scaffold gate. While descending the ladder, they noticed the caught psrl hook. As they looked up at it, the hook freed itself and retracted as designed. In that retracting process, the hook hit our employee in the mouth.

Struck by falling object

A Mine Production employee was in the process of bolting at 8 BMR. Employee noticed a bad top in the entry and exited the cab of the bolter to bar down the hazard. As employee barred down, the arch behind the employee came loose and fell and struck the employee from behind.

Caught in, under or between running or meshing objects

A Mine Production employee was operating bolter drilling a 14 foot drill hole when the steel became stuck. EE placed EE's left hand on the drill steel to apply outward force to free the steel and reached for what EE thought was the lowering lever and inadvertently activated the rotation lever. In doing so EEs hand rotated with the drill steel causing a dislocated left pinky finger.

The underground Utility Crew was taking structure down from a panel. As they were bringing in the forklift EE noticed a rail was close to the forklift tire and was going to run over it. EE bent down to move it out of the way, but the tire of the forklift ran over the end of the rail picking up the other side, hitting the employee in the lip.

Fall onto or against objects

On February 11, 2025, a mine maintenance employee was performing routine maintenance on a borer miner. While stepping onto the bore miner rotor bit block, EE's foot slipped, resulting in a fall and a laceration to EE's left calf from contact with the borer miner bit. The injury required sutures.

Fall to the walkway or working surface

An employee was standing on the FCT ramp for a belt move and slipped on the fines. Once the employee slipped on the fines, they felt pain in right knee. MRI was completed 1/27/2025 and was determined to be reportable on 2/7/2025.

While changing bits on a boar miner the bit wrench slipped smashing their left thumb between it and the tooling plate requiring four sutures.

Over-exertion in lifting objects

Employee was lifting a heavy drill into place when they felt a pull in their lower stomach and groin. The employee was evaluated by a physician and diagnosed with an inguinal hernia. Employee is scheduled for surgery to repair it on 11/27/2024.

Struck by falling object

An employee was removing a cable spool when the cable spool above it fell striking EE in the left arm, fracturing the ulna bone.

Accident type, without injuries

Experienced a skip loading issue on #1 production hoist. This resulted in muck (ore) being dropped in the shaft and subsequently damaging a lower deck and catwalk. This material then proceeded to transfer over into the service cage cheeseweight area.

Contact with hot objects or substances

The CZ-3 level indicator was incorrect and was blocked and salt-encrusted. Valves were opened during attempts to flush the sight glass with condensate, the sight glass ruptured. The employee was sprayed with hot condensate and crystallizer liquor, resulting in 2nd-degree burns.

Caught in, under or between a moving and a stationary object

Helped a forklift operator position a 6"" wide/300 lb. flange I-beam onto a steel rack, placing theirleft hand underneath the I-beam and pushed with right hand while the forklift operator withdrew the forks. As the forks were removed, the I-beam rolled and pinched their right middle finger between it and a 4-inch I-beam positioned below it causing a laceration needing sutures.

Over-exertion (Not Elsewhere Classified)

Employee working overtime was manually carrying a 6-foot-wide bottom belt roller, weighing approximately 70-80 lbs., on EE's shoulder. Upon reaching EE's destination, EE began to set the belt roller down when EE felt lower back muscles tense up and spasm.

Over-exertion in lifting objects

Installing 14 roof bolts in the 2W Mains, employee experienced pain in shoulder as they lifted the cable bolt to insert it into the roof bolter chuck. EE is advised to rest it on days off, re-evaluate 6th when they receive a full work release. EE notifies Solvay of an MRI scheduled on 20th, 22nd they are seeking Ortho for acute tearing and surgery is scheduled for June 6th, 2024.

Fall from scaffolds, walkways, platforms

Employee was assisting in putting a piece of culvert back into place on BC-57. The cover was lifted over the handrail, set on the deck and then lifted to move into place. The employee positioned self to assist placement. In the process of pushing the cover into place EE leaned against a handrail. The handrail gave way and the employee fell approximately 10' to the ground.

Fall to the walkway or working surface

A mine production crew pulling excess miner cable back to the power center had an employee watching the cable being pulled so they could stop them when the new cut location was reached. When finished, the employee stepped backward, stumbled and fell. In an attempt to catch themselves, they extended their arms backward while falling from ground level and injured right wrist.

Over-exertion in pulling or pushing objects

Employee was preparing a maintenance kitting pallet and moving material on top of the bottom pallet to stack another pallet on top. Positioning self in a manner to obtain proper leverage to slide a crusher anvil, then applying force to move the crusher anvil, EE felt pain in the lower back. Seeking medical treatment on March 12, EE was not released to work until March 18.

Employees were pulling a feed side conveyor chain into place with a chain fall hoist, the hook shaft broke and pulled out of the knuckle ring assembly. When this occurred, the employee was struck in the elbow causing a one-half-inch laceration which required two (2) sutures.

Struck by falling object

While two employees were attempting to move an angle iron brace one of the workers had picked it up but dropped because they lost their grip before the second employee could grab it. This caused the angle iron to drop about 16 inches the second EEs left foot.

Struck by flying object

Employee was grinding on some metal and claimed a foreign body from material, flew past employee's face shield and safety glasses entering the left eye.

Caught in, under or between a moving and a stationary object

At approximately 9:25 am on Friday the 29th of September 2023 while conducting media loading activities on the Heat Recovery Chamber-6A, at the Vulcan Project site, the employee forced between the pallet of media and support brace inside the HRC. The employee sustained soft tissue injury and minor fractures to the pelvic area.

Fall from machine

Exiting feet first from a boiler EE had been working on, through an opening at the traveling grates, EE's feet were not on the outer ledge, so EE slid off the traveling grates, through an opening between the front ledge and the traveling grate that was twelve (12) inches wide, falling in the ash hopper approximately nine (9) feet below and suffered an ankle injury.

On August 27th at 10:10 P.M. the #1 service cage tripped electrically. The cage did not get back up and running till 12:10 A.M. on August 28th.

Over-exertion in lifting objects

Employee secured a section of the miner cable with both hands and proceeded to lift the cable upwards to place on the cable hook, then felt a pop in their right shoulder and sharp pain causing them to drop the cable. Received a full work release from the clinic but opted to have surgery after MRI results which became a lost time injury starting 8/15/23.

Fall onto or against objects

An employee was working on a roof replacing metal roofing. EE got to the end of the SRL and it pulled EE back. Causing EE to trip. EE tried to catch self with left arm.

Struck by rolling or sliding object

Attempting to line up the pan line, an employee stepped up on a relay bar and placed hand on top of the spill plate to get a better view. As EE was looking towards the headgate, a large rock slid down the pan line and slid across EE's left hand, pinching it between the rock and the spill plate causing a laceration.

Using a battery-powered chainsaw to cut out foam stopping. Cutting upward so EE placed hand underneath and when EE did this, left ring finger entered the clean-out area of the chainsaw, resulting in EE's finger contacting the chain causing a laceration. Had on a cut-resistant glove which got pulled off, wadded up in the chain causing the brake to be applied to stop.

Contact with hot objects or substances

Employee was working on un-salting a production line. EE was bleeding steam off the line, isolating the steam, taking the hose off to drain and open the line. In doing so, EE turned and hit EE's head, spinning EE around and EE's hand went under the hot condensate.

An employee was walking down the underflow tunnel of TA-110 when their hard hat struck a light fixture.

Accident type, without injuries

Notified of a smoldering coal pile in the coal storage building. Shift Supervisor and Foreman secured the area, smoldering coal was extinguished and monitored for the rest of the shift.

Contact with hot objects or substances

Employee was attempting to clear the suction of an underflow pump that had plugged. The plug cleared and then they slipped while exiting the area and fell to their knees in the hot liquor on the ground, receiving burns to legs and arms.

Struck by flying object

Employee was emptying the S-300 hopper in the grits pile when a wind gust blew dust into EE's face and right eye. EE's eye was still irritated the next day and EE was taken to seek medical attention for an abrasion to EE's eye, which EE received prescription eye drops for.

Over-exertion in lifting objects

Employee rolled up area rug weighs 26 pounds in prep of mopping floor. When placing rug onto a cart, experienced pain in the L arm from shoulder to elbow. MRI's on shoulder and elbow show chronic degenerative disease, frayed labrum, torn rotator cuff, bicep tendon tear. Accepted as work related on 10/19 following medical review.

Over-exertion (Not Elsewhere Classified)

Employee received surgery 11/10 for torn rotator cuff after a couple months of pain that was not improving. MRI revealed full thickness perforation of supraspinatus tendon and concomital tear; supra- and infra-spinatus tendinopathy, partial and full thickness tears and bursal surface fraying. Degenerative arthritis in AC joint, bursopathy, and mild nonspecific synovitis.

Over-exertion (Not Elsewhere Classified)

Employee was walking down a flight of stairs on Cooling Tower 3 (CT-3). While descending the stairs, utilizing the handrail, EE stumbled, and caught self in a twisting motion, while applying weight to EE's left leg and knee. An MRI was completed indicating a torn meniscus, a fresh tear, as well as some other historical injuries. Surgery was performed.

A field operator noted a fire on CE-15. All soda ash employees helped to extinguish the fire. The fire was extinguished at 2:25am. No one was injured, only equipment damage occurred.

Struck against stationary object

An employee was confirming that a valve was shut, requiring EE to duck under a set of pipes. EE ducked back under the set of pipes to leave the area, walking bent over, EE struck the back of head/neck on a wash down water drop causing a laceration behind the right ear. EE was taken to the company physician for evaluation and received three stitches to close the wound.

On May 24, 2022, a fall of ground was reported in the 2 South 3 East return entry at 18xc. The unplanned roof fall was above the anchorage zone in an ""inactive area"" where roof bolts were utilized.

Struck against stationary object

A surface mechanic suffered a laceration to left middle finger, while picking up a volute liner which had a sharp edge, cutting through EE's gloves, requiring three sutures.

Fall to the walkway or working surface

Cleaning the kitchen floor with a mop and bucket, some of the soapy water had slushed out of the bucket. As EE was mopping the floor, EE slipped and fell on left side and shoulder.

Struck by falling object

The miner operator stopped mining, swung the tail toward the right rib and exited the cab. It is unclear why EE exited the cab of the miner. EE was a couple feet from the operator's compartment when the arch fell, striking EE in the head/neck/shoulder/upper back area. The injured employee was provided first aid in the mine and taken to the Memorial Hospital of Sweetwater County.

Over-exertion (Not Elsewhere Classified)

Performing an inspection inside of a tank, coming out of the tank from the side opening, feet first and had pressure to EE's biceps pulling self through the opening, lost grip with EE's right hand, used EE's left hand and arm to catch self and immediately felt pain and burning sensation in EE's left bicep.

An EE was jack hammering the Debris when it broke free. IE's hand was on the stack wall when the debris impacted EE's fingers

Caught in, under or between a moving and a stationary object

Removed a counter weight guide rail on a bucket elevator to work on a bearing behind it and when re-installing it, the lower portion slid off of the base and started to fall downward. As it fell, the injured employee grabbed the rail so it would not fall towards EE. This resulted in EE's left index finger being pinched between a stop block and the counter weight frame.

Contact with hot objects or substances

Assisting with a horizontal pressure leaf filter, being filled with hot water, in preparation for an acid wash, on the deck to confirm when the filter was full. The carriage bolt assembly that holds the filter head, to the filter shell, failed and sprayed the employee with hot water. Employee was treated for 1st and 2nd degree burns and given a full work release on 08/02/21.

July 18, our #1 shaft Service hoist went down at over temp approx. 6:00PM. The AC units in the control room had clogged filters. We cleaned the filters, were able to cool down the control room and the VFD's were reset by 830 PM, NO damage to the hoist or hoisting equipment, a COURTESY CALL made to MSHA Hotline. This did not affect entry/egress from the mine.

An employee was walking and stepped on a rock and rolled right ankle but continued working through the shift. Medical treatment was sought and an MRI was obtained on July 20th which determined that they had ligament issues in the ankle then employee was placed on restricted duty July 22nd.

Engineer was climbing the steps on a locomotive when hips and back began to hurt. Later claimed to have numbness in hands and legs.

Struck against stationary object

Employee struck head on a roof support cable bolt. The suspension in EE's hard hat broke and EE received a laceration, to the scalp, likely from the rivet for the suspension.

The mine experienced greater than 1% methane at the top of the exhaust shaft. This was due to methane liberation within the longwall section while mining through a cross cut and the top not caving as it should following a cut. This led to an evacuation of the mine and changes to the ventilation system to reduce the levels to below 1%.

Track laborer was using a track wrench on a joint bar bolt. EE slipped and twisted left ankle.

During a general hoist inspection On January 12, 2021, 2 broken wires in a single strand were identified on the head rope for the #1 service hoist. Two broken wires in a strand meets retirement criteria and the hoist was taken out of service to change the rope. The rope is a 6x25 flattened strand fiber core hoist rope.

Accident type, without injuries

A failure occurred in the tail rope system for the ore skips on the #2 friction hoist. It's suspected that a swivel bound, allowing the tail rope to unwind/part, creating a failure point on the rope about 150-175' below the north skip. No hoist damage occurred, this is a production hoist, not a personnel hoist. Investigation is ongoing.

Accident type, without injuries

This is being submitted at MSHA's insistence to terminate Citation No 9475521. A Sept storm caused an overflow of the Total Containment Pond, resulting in water-trona solution to spill into the desert. The water-trona solution is not hazardous to persons but required Solvay to contact Wyoming DEQ and BLM (Landowner) to comply with Solvay's environmental permit as it left the site.

Struck by flying object

Employee was adjusting fly ash system when a piece of material blew into EE's eye. EE was checked by a physician and received a prescription antibiotic ointment for precautionary purposes. This injury report was overlooked during furloughs, etc. amidst the COVID crisis.

Caught in, under or between a moving and a stationary object

Installing a monkey face with a hilti drill and pinched EE's finger between the roof and the monkey face causing a laceration that required sutures to close.

Over-exertion (Not Elsewhere Classified)

Employee was getting on the manlift. EE's foot got caught/snagged on the landing floor as EE was stepping while the lift lowered. As EE was shutting down the lift, EE's knee twisted. As a result, EE injured the knee which has required modified duty and physical therapy as of 4/28/20.

A bore miner was parked in an area under roof support to replace a shear pin. The roof was checked visually and appeared to be in good condition. A 5""x10""x1-3"" thick piece of the roof, in front of the bore miner and inby roof bolts, separated, broke apart and fell. A piece of the material hinged, swung toward the miner, and hit EE.

The mine had elevated levels of methane which called for an evacuation. MSHA 800 number was called out of courtesy even though it was not required under part 50. Because of the call we are submitting this 7000-1.

Employee was onsite doing drilling and suffered a nonfatal heart attack. Medical treatment was started and employee was transported to the hospital.

Struck by flying object

Employee was installing roof bolts. The steel plugged, the employee struck the steel with the drill wrench. Upon striking the steel, the socket end of the wrench broke causing the socket to ricochet off the mast and struck the employee in the thumb.

Employee notified management about soreness in elbow in March, EE is a surface mechanic and continued to work until September when EE notified us that the elbow was sore following work on a screw conveyor. EE had an MRI which indicated mild triceps tendinosis. EE was prescribed physical therapy making this reportable.

#1 Service Hoist experienced a fault on a motor used to cool the main hoist motor. This was a result of a faulty contactor. The hoist was not in operation at the time and did not affect mine personnel in any way. There were always two means of egress maintained out of the mine while the repairs took place.

Team member was lining a rail switch when team member felt a pop in left chest/rib area. Team member continued to work and finished shift. On 8/5/2019 team member went to a doctor where xrays were performed and a steroid was prescribed to help with swelling. No lost time, no work restrictions were placed.

Accident type, without injuries

#1 Bore Miner mined into a small void of gas and water. With immediate ventilation changes, gasses were dispelled and the mining process proceeded within 15 minutes. The MSHA hotline was notified due to Solvay Chemicals receiving a citation (9348706) the prior week for not immediately reporting the same issue as a rock outburst.

A mechanic's hand was injured by a fan sheave and pinched by the V-belt mechanism resulting in a laceration and a fracture of the hand.

Struck by flying object

An employee was struck by a shard of metal while replacing the bits on the bore miner.

Employee was attempting to unroll a tape wrap on an extension cord bundle. The employee removed gloves to unroll the tape. After failing to unroll the tape the employee pulled out personal pocket knife in an attempt to cut the tape. During cutting a cut occured to EE's left hand between the thumb and index finger.

Struck by falling object

NAIS crew was hydroblast cleaning the inside of tank. EE wanted to shotgun a different area of the tank but there was a slab of material approx. 30-40 lbs where EE wanted to stand. EE tried to pick up the material to move it out of the way. EE lost control of grip and the material fell on EE's right hand. EE was able to get hand out from underneath the material and exited the tank.

During routine maintenance on longwall equipment, a methane release was detected by monitoring equipment. Area was immediately evacuated and ventilation controls were adjusted to dilute the liberated gas. Gas continued to be emitted so as a precaution the mine was evacuated. MSHA was notified as courtesy through the 1-800 #.

The injured person was using pliers to pull zip ties to secure electrical wiring. The pliers slipped off the zip tie and hit the employee in the mouth breaking a tooth and chipping another tooth. The injured employee return to work the next scheduled shift and has not lost any time for the incident.

Struck by falling object

A piece of a cone shaped plate was being lifted into place. The plate started to tip over and the supervisor said to let it fall because it was still attached to the chain fall. While the supervisor was talking, EE reached out to grab the chain and cut left middle finger on top of the plate. EE had not yet put gloves on.

Caught in, under or between two or more moving objects

A mine crew was welding sections of Drisco pipe together for use by Mine Tailings. In the process of aligning two sections of pipe in the welding machine assembly, the employee's hand was pinched between the sections of pipe. This resulted in a non-displaced fracture to the first knuckle joint of the right index finger.

Team member received a papercut to their right index finger on 7-5-18. Washed the cut out with soap and water and put latex glove over it. 7-12-18 Team member went to dr for antibiotics for infection to the papercut. 7-17-18 EE was taken to hospital for care and to prevent septic shock.

Caught in, under or between a moving and a stationary object

A mine production ee was changing miner bits. During the removal process a bit became stuck and as the ee pulled addition force to free the bit, the bit wrench slipped causing the bit wrench handle to strike another bit, pinching the ee finger between the bit wrench handle and a miner bit, resulting in a fracture to the ee right index finger.

Over-exertion in pulling or pushing objects

Employee was hauling a load of roof bolts and resin to a production panel. The bolting resin stack had worked out of position in transit. The employee attempted to manually re-adjust the bolting resin, as ee was pushing the resin back into the proper position, ee felt a ""pop"" and discomfort in lower right leg. EE was placed on restricted duty beginning 4/11/18.

Worker was repairing a conveyor belt. The worker was cutting the belt close to an area where there was a mechanical splice in the belt. The knife blade caught on the mechanical splice, the worker lost control of the knife blade and sustained a laceration to EE's right knee. Medical treatment in the form of 4 Stitches was required. The worker was released to return to work.

Struck by flying object

A Mine Production crew was repositioning trim chain onto a sprocket, trying to rotate it by striking it with a hammer, so employee was kneeling in front. A fragment of metal separated from the sprocket and struck the employee in the upper leg. An attempt to retrieve it was done on 2/21;ee received two stitches and was placed on restriction to allow the injury to heal.

EE was walking between the A track and Runner switch at the Landing at Solvay when EE slipped on a piece of ice falling on buttocks. EE felt a pain in lower neck/mid back area and a tingling in left hand. EE was taken to the ER where EE was advised a follow up with EE's primary physician, restricted to light duty and discharged.

Struck by flying object

An employee using a metal hammer, connecting a pin into the links on a conveyor chain sustained a puncture wound that was caused by a piece of metal that had come from the pin as it was struck by the hammer, in EE's thigh. After, EE became aware of this, a few days later, EE was taken for medical evaluation where it was determined that the metal fragment should be removed.

Struck by flying object

A Mine employee was positioning a relay bar being installed on the longwall set-up face and supported by a come along (chain style). While positioning the relay bar, the come along chain broke and struck the employee in the face. The employee sustained a laceration wound on the chin that required stitches and a tooth was also damaged. The employee has been released to work.

Contact with electrical current

Incorrect components in the VF Drive led to overheating of a transformer and a normally open contractor. Overheating of contractor led to the welding of the internal contacts, resulting in abnormal current flow to area where incident occurred.

Over-exertion in wielding or throwing objects

A hoist mechanic removing a check valve nut using a 4lb hammer and a pin hammer, when ee felt a pull in arm. The employee was treated on site, before being taken to the local hospital, and was released back to work pending further evaluation. Upon medical recommendation on October 26, 2017, employee elected surgery, scheduled for October 30, 2017.

An employee was setting up the first piece of decking on a new overcast. As the crew set the side walls, the first piece of decking rotated and struck the employee. Their left little finger was pinched between the decking and floor. The employee was treated on site, taken to the local hospital, and the wound required stitches to close. The employee was released back to work.

A Mine Maintenance employee, that was wearing cut resistant gloves, was repositioning a sprocket the employee was welding. In the process of moving the part, finger was pinched between the sprocket and another tool. The employee was treated on site before being taken to the local hospital where the wound was closed with twelve stitches. The employee was released back to work.

Accident type, without injuries

Portion of the side wall (rib) had fallen due to water erosion by a nearby sump impairing two stoppings at the bottom of shaft #3. In order to effect repairs in this area, power and ventilation were shut down after mine employees were evacuated. Repairs were effective, power and ventilation were restored within two hours.

An employee received a laceration while tightening a bolt with a hammer wrench that slipped, pinching employee's finger between the wrench and breather pipe cap. The employee was treated on site before being taken to the local hospital where the wound was closed with seven stitches. The employee was released back to work without restriction.

After receiving the results of a non-destructive rope test, in which, the tail rope on the service cage exceeded retirement criteria, the rope had strength loss of greater than 10%, the #1 shaft service hoist was taken out of service until new rope is installed.

Individual was tightening a T hand on an iron hand attached to a forklift with a wrench. The wrench slipped causing the individual to strike hand on the forklift causing a laceration.

While removing a valve, a mechanic accidentally bumped a 24 inch pry bar, which fell through a grating opening, striking the pipe below, then falling at an angle, striking an employee working approximately 15' to the side of the overhead work, resulting in a non-displaced curvilinear fracture of the scapula.

An employee returned to the Steamplant Control Room to collect paperwork for employee's next job task. The employee went to the kitchen area to get a drink where employee collapsed. Aid was rendered immediately and on-site emergency personnel were summoned. Employee was transferred to the local hospital unfortunately all efforts failed to save employee's life.

Removing a table and vice from a drill platform. Removing the top T bolt and was starting to remove the bottom T bolt, when the vice rotated and pinched the employee's finger between the vice and the bolt. Requiring nine stitches to close the wound to the tip of right middle finger. An x-ray showed no fracture. The employee was released on restricted duty.

The employee was performing a visual inspection of the area; walked to the back of the mining machine to un-hook the water hose and retrieve the remote for the continuous haulage. As ee was walking back to the bolter station, a roof slab fell hitting the mining machine, the load out tail of the miner and the employee.

Over-exertion in lifting objects

Working to connect piping in mine, EE used EE's shoulder to support the pipe while lowering the pipe hanger. Felt a pop in EE's right shoulder. Reported on 9/11/16, taken to be evaluated and given a full release on 9/13/16. 9/15/16, the employee requested an MRI and was given a second full release. 10/6/16 the employee was seen by the company doctor and placed on restricted duty.

Contact with hot objects or substances

Working in an area with accumulations of crystal & hot liquor, felt a sharp pain in left heel, exited the area and found a small blister because of a tear in the back of rubber boots. The burn was minor and EE worked the rest of the shift, next shift and went on scheduled days off. On 9/16 the total area had grown in size, went to the doctor and was placed on restricted duty.

An employee was operating the #6 BMR when a rock approximately 2 inches in diameter released from the mining face and deflected off the cutting arm striking the employee in the mouth. The impact caused a chip to the employee's tooth and a scratch to their lip. The employee was treated and released to full duty.

The employee was adding 25' of vent tube to a high velocity fan when dust was dislodged from the tube which was blown into eye from the side around safety glasses. Employee's eye continued to be irritated so employee notified supervisor on 8/25/2016 and was seen by an Optometrist. Employee was diagnosed with an eye infection and given antibiotic drops.

A Mine Utility crew was moving a shuttle car with a Wagner loader, attached with a chain. Two - 8 ½"" hydraulic hoses were hooked up for the steering of the shuttle car. The chain became un-hooked, which tensioned the hydraulic hoses, until they broke. A hydraulic hose whipped back towards the operator of the Wagner striking EE in the chest & neck. EE received three stitches.

A Miner Operator was walking between the rib and the bore miner, when a section of roof (approximately 6""x6"") separated from the mine roof and fell striking the tail section of the miner. In doing so, a fragmented piece 18""x6"" struck the employee on the side of their head, shoulder and arm. This resulted in the employee sustaining a laceration to their right triceps.

An employee sustained a distal right radial splinter fracture of the left wrist. EE was outside dumping the ball mill dumpster, slipped in the mud, losing their balance, struck arm on the side of the dumpster while trying to catch self from falling. Following a full medical evaluation with the company doctor on April 4th it has been determined that surgery will be required.

Slipped opening ranger door and sustained pressure and pain in left shoulder. Employee had an MRI on 1/5/16, reviewed by company physician, inconclusive. A 2nd opinion with an orthopedic surgeon, 2nd MRI was prescribed with contrast 1/19. Jan 20, company learned that the employee had suffered a dislocated shoulder resulting in a tear of the labrum that will require surgery.

An employee sustained burns to the left side of his body. The dryer feed belt had gone down, causing an accumulation of 210 degree crystal and liquid. The employee was standing in front of the chute washing the material off of the belt. A blockage higher in the chute broke free and ejected hot material where he was standing, he tried to turn away but it contacted his left side.

Removing a pipe suspended with a come-a-long, lowering to a forklift. Employee was holding the pipe to steady it, the pipe shifted, hyper-extending his arm and tore his right bicep.

December 14th, the employee was still experiencing pain, at which time an MRI is prescribed, MRI on 15th and on 16th the MRI results indicate a partial tear of the MCL.

While grinding 6 inch angle iron to fit to an I-beam, an employee was on a step ladder with a foot on the I-beam. While losing his balance, attempting to grab something so he wouldn't fall, the grinder slipped hitting the back of his right hand causing 2 lacerations. He received 4 sutures and returned to work.

Troubleshooting SC-42 screw, after removing the lid, the employee was lowering it to the deck and caught his finger between the bracket and the lid causing a tuft fracture of the left ring finger.

Employee was stepping off railcar ladder. He missed the bottom step with his right leg his left leg was caught on the side sill grab iron and he hyper extended his knee. On October 20th he received an MRI. On October 29th MRI results showed a complete tearing of his ACL and MCL. Employee is off work until ACL an MCL are repaired.

A mechanic was replacing a ""T"" section of pipe, which to align was suspended from two points. One of the points was a section of 3"" x 3"" x 3/8"" angle iron supported by structural I-beams. While maneuvering the horizontal pipe, the section of angle iron shifted from between the I-beams allowing the angle iron to fall and strike the mechanic on the hand.

An unplanned roof fall above the anchorage zone occurred in the 11 East 7 South tailings disposal panel at 27XC within the number 2 entry. This area was previously mined with no active extraction of ore at the time of the event.

EE and another ee were instructed to clean out EL-11 pit. They cleaned the grate covering the pit, then decided to remove the grating to better access the pit. EE was holding the grating and let it slip out of his left hand resulting in a tendon tear in his right arm at the bicep.

Hoist trailing cable damage disabled cage controls and communication and was taken out of operation by the hoistman. The hoist was returned to operation by use of two-way radios allowing communication to the hoistman from the cage who was able to hoist in manual control.

Due to a product spill by a delivery truck driver, an employee sustained a laceration to the posterior region of his scalp, requiring 14 staples to close, due to a fall. He did not recognize that flocculent had been spilled on the ground creating a slipping hazard. As he proceeded across the spilled area he slipped striking his head on the concrete.

An employee was removing a shuttle car tire to work on the steering. He removed all the lug nuts and separated the wheel from the hub, 4 to 6 inches. Picking up the tire with both hands, the rim and tire slid back toward the hub and his finger was smashed between the tire and a wheel stud.

An operator was locking out liquor in a valve that had no handle, the operator used a pipe wrench to open the valve. When the operator pulled hard on the pipe wrench, he felt pain in his right shoulder. After further evaluation and treatment on 1/30/15 the employee was placed on restricted duty.

A piece of loose chain was wedged between a flight and the AFC pan line causing a potential heat source, which in turn, ignited methane releasing from the mine floor.

An unplanned ignition of methane gas occurred at compressor #27 located at the surface mine gas recovery station. A leak occurred within the compressor station when a 3/8"" stainless line failed causing a release of methane gas.

Employee injured on 8/18 with no reportabilty. On 9/10, after a follow-up medical visit, the employee was directed not to return to work until further healing takes place. Decoupling a hose from a guar line, shut the ball valve, bled the pressure, opened the pump box drain valve, start decoupling hose, was sprayed with residual condensate left in the line in the abdomen.

Struck against stationary object

Due to the body positioning and the location chosen for footing an employee fell forward cutting his right forearm requiring 6 stitches. When his arm landed on the sheet metal.

Absorption of radiations, caustics, toxic and noxious substances

Opening a condensate flush valve on a tank already in overflow, the hot liquor inundated the overflow of the tank causing it to spill over the top of the tank contacting the employee on his left side from the top of his shoulder down his left arm, chest and back.

Hydrochloric Acid was accidentally released into a trench filled with Soda Ash Liquor. A cloud began to form in the immediate area of the tank of diluted Hydrochloric Acid Fumes. There were no injuries or illnesses, however as a precautionary measure we evacuated the immediate area.

Using a shovel to remove rocks the employee felt a sharp pain in his abdominal area. His supervisor was notified. July 3rd, he stated that a hernia had developed and was diagnosed with having sustained an abdominal hernia. July 14, the surgical report was reviewed and confirmed that the injury sustained was linked to the employees work activities on June 26th.

The employee accessed a stationary railcar to release the hand brake. While applying pressure to the hand brake she felt a pain in her right forearm. She was diagnosed with having inflamed tendons in her forearm and released to work. Upon subsequent follow-up with a specialist on January 14, 2013 a cast was placed on the arm which now constitutes a medical treatment case

While operating roof bolter #15, the employee experienced intermittent problems with the centralizers closing on their own. During the troubleshooting process the electricians asked the employee to move the boom of the roof bolter. While moving the boom a drill steel broke free striking the employee in the forearm.

The #1 surface hoist was taken down to repair a loose brake fitting at 5:00 a.m. The fitting was repaired and the hoist was brought back on line at 6:00 a.m.

The #1 Service Hoist tripped power on a ground fault. The hoistman attempted to reset the fault and the motor would not reset. Upon investigation it was found that moisture had entered the ABB Drive motor housing. The motor was dried and power was reset.

A Roof Bolter Operator had a drill steel stuck in the drill chuck and was attempting to pull it out with a chain that was tied off to the centralizer arms. Once the drill steel was in rotating motion the chain spun loose and struck the employee in the face and nose.

The employee was attempting to free up an adjustment piece on a 48"" pipe wrench. A piece of metal broke off when the pipe wrench was hit with a hammer causing the piece of metal to penetrate the skin of the employee's arm.

The employee was palletize 48"" structure. As the employee was lifting the rollers he twisted his right knee and felt a pop.

The employee sustained an injury to his right hand while performing the task of changing out a lifting cylinder on the continuous haulage unit. The employee's hand was caught between the lifting cylinder bracket and the evaluated roadway re-railer.

An employee underwent an MRI to determine the nature of the injury associated with his right shoulder. On 2/27 the MRI was reviewed and he was diagnosed with a partial tear to the rotator cuff. He continued to perform his assigned job and is awaiting a 3/14 appointment with a surgeon to determine if surgery is needed repair the damage to his shoulder.

A mine production employee working in 7 Bore Miner panel sustained a sprained ankle when he stepped off the auxiliary fan skid back onto the mine floor. The employee had stepped up onto the skid to reach a miner cable that was secured from the mine roof by a J-hook. The employee severely rolled his ankle thus was unable to report to his normal job duties.

An employee was operating a roof bolter in the 11 East 11 South 3 BMR section, he was extending to place the bolting resin in the hole the mine roof fell and a rock struck the employee on the right shoulder. The employee was initially given a full release to work. On 2/7 he was diagnosed with a torn rotator cuff, and been placed on restricted duty on 2/9 awaiting surgery.

On 11/13 the employee was manually handling belt structure. On 11/14 he stated that his left shoulder was bothering him for some time to the point that he could not raise his arm, perform physical work activities and the pain would reside during days off. An MRI on 11/15 shown an acute tear to the left rotator cuff.

A crew was working on BF-54 to determine proper bag house operation. The effected employee removed the port hole (inspection hole) on top of SC- 54 to view the screw conveyor and determine if they had suction on BF- 54. As his co-workers closed the main door on the bag house the screw conveyor and inspection port hole gained suction and pulled his finger into the screw conveyor.

While climbing on the back of the roadway to get a rock out of the chute, the employee stepped on one of the wheels on the roadway and fell. He tried to catch himself and sustained a sprained to his left shoulder and a small laceration on his left forearm. Employee worked until obtaining an MRI on 4/20/2012 showing a tear in his shoulder. He had surgery on 4/23/2012.

At 1:00am a hydraulic brake pump tripped power due to a loss of hydraulic fluid. As a result the #1 service hoist was inoperable for a period of time until another hydraulic brake pump was brought on line. The hydraulic pump was replaced and brought back into service.

An employee walked up into the room and was standing next to the conveyor boom behind the cab of the miner. He was positioned partially under the roof bolts along the right side of the entry. As he turned to leave, a section of the roof separated up to the bolts and a portion struck the employee causing a laceration on the right side of his head which required four sutures.

At 3:41pm the #1 hydraulic brake pump tripped power due to a loss of hydraulic fluid. As a result, the #1 service hoist was inoperable for a period of time until the #2 hydraulic brake pump was brought on line. A fitting on the #1 hydraulic pump had failed causing a loss of brake pressure. The hydraulic fitting was repaired and brought back into service.

An employee was using a forklift to move a bore miner tooling plate. After having transported the load, he was attempting to reposition the tooling plate so it could be placed on the mine floor when the load shifted resulting in the employees finger being pinched between the mine rib and the plate. The employee sustained a fracture to the end of the finger.

Two employees were in the process of closing a raw water block valve to TA-56. As both employees were working together to close the valve, one employee felt pain in his left shoulder. The employee went and sought medical attention with an Orthopedic Surgeon who prescribed an MRI. The MRI indicated that an the employee had an acute tear to his rotator cuff.

A summer student was using a utility knife to cut vinyl siding from a wall. He was standing with the utility knife in his right hand. and his left hand and forearm resting against the side of the wall. While applying downward pressure on the knife, it came out of the cut and the employee then jabbed himself in the left forearm, sustaining a laceration which required 3 sutures.

While carrying wooden planks down a set of stairs, an employee lost her footing and fell on the stairs. As a result, the employee felt pain in the lower portion of her back. She continued to work until 6/30. On 7/1 the employee underwent an MRI. 7/6 the employee was directed to stay off work until further tests can be carried out which are scheduled on Aug 6.

EE sent his co-worker downstairs and cont'd. to vacume alone. The vacume hose came loose and the vacume suction pulled EE's arm into the flex hose.EE was unable to pull the vacume breaker , his co-worker saw what was happening and pulled the vacume breaker. EE has compartment syndrome to left arm.

A mine electrician sustained burns to his right forearm and right side of his face when he closed a 400 amp breaker and a phase to phase occurred causing an arc flash. The investigation found evidence that water had entered the breaker housing creating a common path between phases.

An employee was in the process of donning his respirator when trona fragments entered his right eye. He was taken to the ER and referred to an opthamologist on 1/31/11 who determined he had a slight infection and a scratch to the cornea.

Fall from ladders

An employee was positioned on the 2nd rung of a 4ft ladder applying foam to the outside of a stopping lost her balance and fell. On 2/23 an orthopedic surgeon diagnosed her with having herniated disk and place her on restricted duty. Further medical reviews have indicated that the fall and the employees job position could have reaggravated a pre-existing back injury.

Two mine employees were in the process of removing a metal cover plate located over a valve bank on a stamler/feeder. While both employees were lifting the cover, one end slipped resulting in the injured employee's left hand being pinched between the cover & the stamler. On 01/07 he was diagnosed with having sustained a fracture to the tip of his middle finger on his left hand.

An employee and was reaching with his right hand to clear a rock that was located in the bretby tray. It appears that a rock was propelled from the head gate drum striking the employee in the left hand and chest. The employee was diagnosed with having sustained a severe strain to the wrist and lacerations to the thumb. On January 3rd, he was placed on restricted duty for 10 days.

An employee was changing the injector pump on #11 forklift in the shop. He was utilizing a long handled 1/2"" drive ratchet to turn over the motor on the forklift to align the timing marks. He was standing on the balls of his feet, leaning into the forklift when he felt pain in his left calf muscle. He sustained a tear to the upper calf muscle and was placed on restrictive duty.

The employee was utilizing a rock hammer to tap on a slip-fit packing gland located on a shuttle car tram motor, when a piece of metal broke off the hammer and embedded in the employees chin. The employee sought medical attention and the piece of metal was removed and the wound closed with sutures. The employee has been placed on restricted duty until the sutures are removed.

A production operator sustained burns to his right arm, face and abdomen due to steam being released when opening the top access door on Crystalizer CZ-451. When the employee opened the door steam at a temperature of 148 degrees F was released from the crystalizer which came in contact with the employee.

While rotating the dryer to remove a spring, the employee stopped the pipe threader but the dryer kept spinning. The employee went to check to see why it was spinning on its own and the handle of the pipe threader struck him on the right side of his hard hat causing a laceration to his brow. The employee received sutures to close the wound and was given a full work release.

While installing the scavenger belt under #2 shaft apron feeder, they were running the conveyor while measuring to fabricate the guards. The employee stepped back and then stepped forward the same time a flight came around and caught his right boot and drug it under the drive shaft. He sustained a contusion to his right foot and ankle. On 10/7/10 he started restricted duty.

The employee was attempting to remove the bolter cable down from the ""J""hooks which were suspended from the roof by a roof bolt plate. As the employee lifted up on the roof bolter cable, the ""S""hook which is attached to the ""J"" hook came free from the plate eye. This resulted in the ""J""hook falling and striking the employee on the bridge of the nose resulting in a fracture.

n SOS services temporary contract employee was working on cleaning and re-installing the spray nozzles on a turbo mister unit. While performig this task, the employee lost his balance and his forearm came in contact with one of the nozzles causing a laceration. The laceration to the forearm required seven (7) sutures and the employee was given a full release.

The employee was performing trouble shooting on the electrical ground monitoring system of a 480 volt receptacle located on a 500 KVA transformer. An arc flash occurred and the employee sustained 2nd and 3rd degree burns to his hands, face and arms.

EE was in the process of stripping the outer jacket from a 3 conductor 250 MCM motor feeder cable. He was using a utility knife to cut the outer jacket of the cable to strip it back to perform an electrical test before terminating it to the motor. During this process his utility knife slipped out of the outer jacket and cut the middle and index finger of employee.

Once completed, the employee was carrying the starter back to place it in the MCC cabinet when he felt pain in his lower abdomen. The employee was diagnosed with having sustained a hernia.

The trailing cable on the on the #1 man and material cage became caught on a curtain wall panel and was pulled in two. This resulted in loss of power, communication and conveyance of the hoist. One employee was on the cage returning to the shaft collar when the incident occurred. The hoist was returned to the collar within 45minutes.

An employee and a co-worker were pulling a 3'x4' sheet of metal plate. The employees had placed a chain through two holes in the plate and were dragging the plate across the mine floor. After letting go the employee turned to walk when he felt the pop in his knee. ON March 8th he was diagnosed with having a torn meniscus requiring surgical repair.

A longwall employee was walking along the shield line when he noticed that the shear had stopped cutting and proceeded to walk along the shear line to assist employees with the rock jam. A rock released from the tailgate drum striking the employee in the left hand causing a fracture and partial amputation to the middle finger and a laceration to the index finger.

Two employees were securing a head roller frame to the mine roof. One of was operating a roof bolter& the other ee was holding the glue resin in the bolt hole. The ee raised the bolt through frame & the bolt struck the other ee's finger between the frame & roof. He received 17 sutures to close the laceration on his left index finger.

An excursion of SO2 gas was released from the sulfite scrubber exhaust stack. The SO2 gas carried a distance of 350 feet causing three contractor employees who were assembling a product silo, to the east of the sulfite building to experience nose, eye and respiratory discomfort.

A transport line containing liquid sodium bisulfite solution separated at a joint causing a release of SO2 gas. Contractors working in the adjacent area of the S-300 plant were exposed to the SO2 gas. The area was evacuated and flow to the line was isolated. The released liquid sodium bisulfite was neutralized by applying soda ash and the area was secured and flagged off.

Caught in, under or between a moving and a stationary object

An employee sustained injuries to both his right and left hands while operating a Fletcher Roof Bolter placing bolt in a corner section. The drill steel became stuck and seperated from the extension chuck. The employee then grasped the drill steel with his right hand and began pulling it back into alignment with the chuck extension. His glove became entangled in the steel.

Over-exertion (Not Elsewhere Classified)

The employee was performing repairs on the longwall shear which required him to be on the top of the shear body. While climbing down off of the shear, he stepped down onto the Bretby tray. His right foot slipped causing him to lose his balance. He sustained stretched ligaments in the left ankle and will see a specialist on September 28th.

At 6:45 pm on Sept 18th, an ignition of gas occurred at the site of the gob vent bore hole number 2. Upon discovery, the gob vent bore hole pump and engine were taken off line. The incident is still under investigation to determine cause and corrective actions.

Struck against a moving object

An employee was operating a shuttle car, while tramming the employee traveled over a bump in the roadway resulting in a jarring motion to the employee's back. On Sept 18th, employee was taken for medical observation and diagnosed with a strained lower back muscle. On Sept 24th the employee was sent for further medical observation and placed on restricted duty as of Sept 25th.

At approximately 5:30 am on September 13th, an event occurred that triggered a release of methane. The areas of most significance were directly behind the shields and tailgate side of the longwall face and the stopping line that separated the longwall gob from the return air course (bleeder). Employees were evacuated from the mine.

A maintenance employee was re-installing a transmission in a jeep. The transmission was placed on a hydraulic lift jack and as the employee was moving the transmission into position he felt a sharp pain in his shoulder. At this time, it appears that the employee may have a torn rotator cuff although the MRI results will not be read until September 24th.

Over-exertion in pulling or pushing objects

Employee was pushing a jeep underground at the start of shift and later into the shift his back became sore. Upon further investigation and medical evaluation the injury to the back was deemed to be worked related as of September 9, 2009.

Caught in, under or between running or meshing objects

The employee was using a hydraulic metal shear to cut strips of stainless steel metal. He reached around the back side of the shear to grab the metal to slide it into position when the actuating lever to lower the shear was activated. His right thumb was underneath the cutting edge resulting in the thumb being amputated just below the first knuckle.
