Showing all 26Employee was injured when a rock dislodged from the mine roof striking him and knocking him to the ground. When employee fell he reached out his arms trying to catch himself. When he hit the ground his right arm received a laceration which required stitches.
Employee was beginning to start the hole for inside bolt on next to last row when an undetected piece of draw rock fell striking the end of his right little finger causing a fracture and laceration requiring stitches.
Employee was in the process of picking up a shovel that had fallen off of the side of the miner, when a rock dislodged from the mine roof 24" x 14" x 6" striking him on the back of his right hand. Employee stated that he had visually looked at the area and location of the miner to make sure it was alright to enter. He observed nothing. Resulted in stitches.
Employee was drilling hole for bolt when rock turned loose and struck his right arm resulting in 5 stitches.

Injured was bolting top when a rock fell from the roof striking him on his left shoulder and back. EE continued working until 2/5/08 at which time he started missing work.
Employee was drilling roof bolt hole-rash & rock fell from roof striking him on back of left leg. (Rock & rash slid of the canopy)

Employee was setting jack against top - rock fell from top catching his finger between rock & jack. Went to doctor on 9-26-07. Started restricted duty on 9-27-07.
Employee was preparing to put bolt in hole that had been drilled. A piece of rock fell and struck him on nose. Went to Dr. on 3-26-07 (simple fractur of nose & 3 stitches to left side of nose). Returned to work on 3/27/07.

Employee was walking down beside roof bolter machine. A piece of draw rock fell striking him on the arm. He went to doctor on 6/10/05. Returned to work on 6/13 after his scheduled 3 days off.
EMPLOYEE WAS MARKING OFF BOLT HOLE. A PIECE OF ROCK FELL, STRIKING HIM ON TOP OF HIS HARD HAT, PUSHING IT FORWARD DOWN ON HIS SAFETY GLASSES AND CAUSING A HAIRLINE FRACTURE OF THE NOSE. THIS EMPLOYEE WORKS A 4 ON AND 4 OFF SCHEDULE. THE 20TH WAS HIS FIFTH DAY AND HE WILL BE OFF UNTIL 4/24/04.
