Employee and 3 others were loading 4"" plastic water line on flat car. Water line was raised over the standards on the flat car and slipped out of the hands of one of the other men. When the end of the water line hit the bottom it mashed his finger between the water line and the standard on the flat car causing a laceration and fracture to the end of his finger on his right hand.

A roof fall occurred 2 breaks out by spad #4059. EE discovered the fall at 2:00am in the primary escape way. The fall measured 25' x 25' x 12' to 15' thick.

Employee was knocking a broke bit out of ripper head when he struck his thumb with a sledge hammer. The hammer slipped out of his hand causing it to strike his thumb resulting in 5 stitches

Employee was in the process of loading rock dust into trinkle duster. He cut one of the bags open to dump into the duster. When he started to lift the bag to dump it, he felt a sharp pain in his lower back.

Employee stepped off shuttle car landing on miner cable and twisted his right ankle.
Employee was stepping down from center of drill to drill station and stepped on a rock and twisted his right knee. He continued to work up until 9/5/12.
Employee was bolting top. He drilled through draw rock causing it to break into two sections. It fell and struck him on the left hand cutting his index finger resulting in stitches.
Employee had changed out a monitor package on the South section power center and was checking the breaker to see if it would set up. When he put the breaker in, it blew up causing burns to his right hand and a fracture to the end of his right ring finger. Injury was not life threatening but had a potential to be.
Employee was putting cover back over hydraulic hoses underneath the bat wing of the miner. The bat wing was being supported by a slate bar which was held by another employee. The bar slipped causing the bat wing to fall on his left hand. Resulted in stitches and a fracture to his fingers.
A roof fall occurred in the intersection in the #2 heading out-by spad 5149. The fall measured approx. 30' long, 20' wide and 5 to 5.5' high.
Employee was injured when a rock dislodged from the mine roof striking him and knocking him to the ground. When employee fell he reached out his arms trying to catch himself. When he hit the ground his right arm received a laceration which required stitches.
Employee was beginning to start the hole for inside bolt on next to last row when an undetected piece of draw rock fell striking the end of his right little finger causing a fracture and laceration requiring stitches.
Employee was in the process of picking up a shovel that had fallen off of the side of the miner, when a rock dislodged from the mine roof 24"" x 14"" x 6"" striking him on the back of his right hand. Employee stated that he had visually looked at the area and location of the miner to make sure it was alright to enter. He observed nothing. Resulted in stitches.
Employee pulled rib and it landed on his right foot.

Employee was drilling hole for bolt when rock turned loose and struck his right arm resulting in 5 stitches.
Employee was setting the bolter up for the first row in 7 Right. He was at the controls on the operator's side when the rib turned loose and the rock fell on its edge catching his right foot. The rock was 69"" long, 24"" wide at the widest point and 6"" at the narrow end. It was 11.5"" thick at the thickest point and 6"" at the thinest. This resulted in multiple fractures in his foot.
Employee was bolting in the #2 heading when a draw rock fell. He was trying to move the rock himself and in doing so twisted his right knee.
Employee backed the miner out of place and bumped rib with the miner pushing corner of turning rock loose over top of the coal. The rock struck him on the right side resulting in a broken foot.
Employee was performing maintenance on a Lo-Trac when he backed the back tires over an embankment causing the power trac to go over which resulted in the peddle catching employees ankle and foot resulting in strain.
Roof fall on the 002 section # 3 heading while getting first lift of the pillar block. Roof fall came from the pillar line, directly behind catching the #1 MRS as it was set up for roof protection. The approx. size of the fall was Length-30' width- 20' height- 5.5'. MRS system worked to catch falling roof and was retrieved after order was terminated to allow removal of MRS.
While sitting in the mantrip at air lock doors falling material struck employee on the lip resulting in a .5 inch laceration to the outer skin of the lip. Resulted in stitches seven (7).

A fall occurred on the # 7 belt 1 break inby spad 7977 of the B Panel of 2 Southwest Mains that was 5' thick , 16' wide, and 55' long. Area was dangered off until an approved cleanup plan could be acquired through MSHA. Area was cleaned and re-bolted.
Employee was drilling hole on the third row of bolts when material fell that was 36"" Long by 11"" wide 1"" to 0"" thick hitting him on right forearm requiring stitches.
Driver entered bed of truck with step ladder to prep. bed for hauling. While exiting bed slipped from ladder falling to ground approx. 4 feet.
Roof Fall occured at the airshaft mains area of the mine in the No. 5 intake entry at crosscut No. 9 spad No. 4167. The roof fall is approximately 12 ft in height, by 18 ft wide, by 22 ft in length is throughout the entire intersection.
Investigation indicates employee was entering Roof Bolter tram deck when he inadvertently lowered canopy onto body, resulting in multiple fractures.
Moving over to put up bolt and caught foot between two pieces of rock. Strain to knee.
A rock fell from around 6' and struck employee on left shoulder and hand causing a laceration on his hand requiring 3 stitches.
Employee was lifting thumper to place on the power center, when his foot slipped, causing him to twist his back.
A non-injury roof fall occurred on the 002 MMU during pillar recovery covering the Joy continuous miner. The rock fell from the cut and did not pull permanent roof supports. The fall measured 20' X 20' X 4'. A 103K order was modified and terminated to allow normal mining operations to continue after the miner was retrieved.
A non-injury roof fall occurred during pillar recovery on the 002 section, #4 block, 70' inby S.S. 7697, entrapping the mining machine. The rock measured 15' W X 18' L X 4.5' thick and fell in the cut. It did not fall above anchorage. MSHA issued a 103k order which was modified to retrieve the maching machine and resume normal mining operations.

A non-injury roof fall occured during pillar recovery on the active 003MMU section, 5rt. pillar block, 100' inby S.S.# 7416, covering and entrapping the continuous mining machine. MSHA was notified and issued a 103(k) order which was modified to retrieve the mining machine. The 103(k) order was terminated at 19:21 hours to allow normal mining operations to resume.
Employee was getting roof bolts off of drill when he stepped in a hole hurting his lower back. (Started losing time on 6/8/09)
Employee tripped over a timber that was covered and hidden by rock dust, injuring his knee. Employee had surgery due to this accident on 5/15/09, during his previously scheduled vacation.
Possible alleged ignition on the 003 MMU in the #5 Face, 130' inby spad 7190. Miner man thought he seen a flash, while cutting coal for 2nd car. Foreman who was also there reported that he didn't see anything. They shut the miner down and then continued to examine the miner and area, and found no sign of ignition.

Roof fall was discovered 100 ft outby spad M407 in a crosscut in a return airway, #2 mains, 11,600 ft from the face, area was developed in 1992.
While assisting another roof bolter, a rock fell and bounced off the canopy striking him on the right forearm causing a deep puncture wound.
A peice of steel slipped and caught his right middle finger between the steel and frame of the miner, requiring stitches.
A roof fall was discovered on 7/22/2008 at 4:05 pm in #6 entry on S W Mains outby S.S. 4600 aprox. 3,000 feet outby the working sections. The fall was approx. 18 ft wide, 10 ft long and 0' to 7' feet thick. No equiptment, persons, nor ventilation was effected. This area was supported with 6 foot resin bolts fully grouted and 12 foot cable bolts. The fall will not be cleaned up.
He was making a splice, holding the cable in one hand, got tape out of his pocket, pulled tape with his teeth and broke a tooth. No lost time.
Diesel Man Bus engine solenoid would not pull in enough for the engine to start. The engine cover (hood) was raised to push solenoid in by hand. The vibration of the engine starting caused the hood to fall striking his left hand and right middle finger resulting in a contusion of the left wrist and a Fx distal tuft right 3rd finger/laceration.
On 6/14/2013 we were notified by worker's compensation that he was awarded CWP benefits. Employee last worked on 3/31/2008 and is no longer employed with the company. The accident date of 3/31/2008 is to accommodate online reporting per MSHA (Denver) instructions.
Employee was placing drill steel in the chuck on pinner head. A piece of rock slid down the drill steel catching his thumb between the rock and the pinner head. Went to doctor on 1/8/08. Started losing time on 1-9-08.

Employee was loading a belt take up unit on a flat car-threw a timber upon take up to block with-timber struck roller and roller came down-striking employee. went to dr on 10/19/07 Returned to work on 10/22/07.
EE was struck by a rock falling out between bolts while moving miner cable & water line. Went to DR 10/5/07 started losing time 10/07/07.
EE was drilling bolt hole-head on pinner stopped up dropped head reached over to check suction on head. Drill steel fell out of hole, striking him on finger. Went to dr on 6/21/07-ret'd 6/22/07.
Employee was operating shuttle car, hit hole in roadway. The jarring jammed his teeth together, chipping a tooth.
Employee was standing on wooden pallet, installing a piece of tin on side of bldg.-stepped backwards, falling, landing on his right shoulder & right hip. Laceration of right forearm. Went to Dr on 6/8/07. Week-end off, not scheduled returned 6/11/07.
Employee was moving a piece of rock off the side of the continuous miner. Another piece of rock fell off striking him on side of Rt. foot. Went to Dr. on 3-13-07 (started his 4 off days on 3-14-07) Started losing time on 3-18-07.
Employee was using a wrench to tighten up a water line fitting-wrench slipped striking his in the mouth-chipping 1 front tooth and loosening another front tooth. Went to dentist on 3/5/07. regular days off 3/5,3/6,3/7 return reg job 3/8/07.

Employee was swinging drill boom over to put up rib bolt, catching finger between canopy jack housing and boom. Went to doctor on 12-09-06. Employee on 4 on 4 0ff schedule. Returned to work on 12-13-06 10:00 PM.
Employee was scaling top with a slate bar-small piece of rock fell-striking him on top of left wrist.
Employee suffered a fatal heart attack while underground; he was not involved in any type of work when the attack occurred. An investigation was conducted by MSHA and it was determined that there were no contributing work-related factors involved and that the death was the result of natural causes as determined by medical professionals.
Employee was operating a Johnson Stinger (4-wheeler) personnel carrier beside the belt line. A tie road end broke on the Stinger. Employee tried to control the Stinger then bumped his arm and head against the belt rail and roller. He went to doctor when injury occurred; continued to work. Went back to doctor 1/17/07 and started losing time 1/21/07.

Air shaft mains as on retreat mining cycle when draw rock & rock fell from gob area on the ripper head of the continuous mining machine-pushing the ripper head down in the bottom-had to use the retriever to pull miner out-no injuries & no equipment damage.
Employer was going to check water line. Uni-hauler came around the corner of block of coal & back end of hauler struck him on the outside of his knee. Went to the Dr. on 7/15/06, started losing time on 7/16/06.
EE was operating the continuous miner, making a clean-up run on the right side of entry (curtain side). He was stooped down and a piece of rock slid off rib, striking him on lower left side of back. Went to Dr. on 3-8-06. Went back to Dr. on 3-13-06.
EE WAS OPERATING A 4-WHEELER (STINGER) PERSONNEL CARRIER. THE FRONT WHEEL DROPPED INTO A HOLE IN THE HAUL WAY, CAUSING THE STEERING WHEEL KNOB TO STRIKE HIM ON THE INDEX FINGER OF HIS LEFT HAND (LACERATION - STITCHES).
