After roof bolting machine was pulled up to install next row of bolts & after ATRS was set, IE swung his head out & placed drill steel into head in preparation to drill outside hole, when a piece of roof rock(18"x12"x1"-2" thick) fell striking canopy then striking his left hand causing laceration.

Employee was drilling roof, 2nd steel in roof had 3rd steel in right hand to get ready to put in roof resting on right leg when a piece of coal came off of canopy & struck employees right middle finger. Note: This is being reported late as report received from Indiana ER did not state that a suture was inserted.

Employee was hit with a pice of rock on left hand when putting drill steel into chuck, rock hinged off ATRS and fell onto chuck.

Employee was roof bolting when a rock fell from between bolts and struck him on the left lower leg.

Employee started to drill a hole when a piece of rock fell out of the roof and hit the canopy, then struck employee in the left arm.

While drilling to install a roof bolt a piece of roof rock 1-foot x 2-feet x 6 to 8 inches broke off striking him on the left side of his face and left forearm and right foot.

Employee was in the process of starting a new hole for the installation of a 4-foot resin bolt. As the employee placed the starter drill steel into the drill chuck and raised the steel up against the roof line, a piece of draw rock fell striking his right hand requiring sutures to close the wound.

Injured and co-worker were roof bolting a cut in the #3 entry when a piece of roof rock 39" long x 4" to 10" thick x 0" to 7" wide broke out between the ATRS and the Safety Post Jack hitting him on the back of the head pushing him face first into the bolting machine causing facial fractures.

Injured entering the mine to begin shift when a he was hit by small piece of laminated shale roof rock - causing a 1/2" laceration to his forehead requiring stitches.

Injured was drilling hole when a piece of roof rock broke off around the steel and fell striking his right wrist and dislocating a bone.

Employee was roof bolting in the North 3 Parallel Section when a piece of rock fell out striking the bolter head and landed on employee's right knee.

Injured was kneeling beside roof bolting machine bending roof bolts, when a piece of draw rock fell from the roof, striking injured on his right side.

While operating Continuous Miner from kneeling position, injured was struck on the side of his right foot by a piece of draw rock.

Employee was kneeling beside the operator side controls of the #5 Fletcher Bolter. As he tightened the 6 1/2 foot combination bolt, a piece of roof rock (1/2 size CSE SCSR) fell between canopy and bolter boom striking his left gloved hand injurying his index finger. His hand was resting on his thigh it was hit.

Employee was operating helper side on Long Airdox Twin Boom Roof Bolter. As he was drilling a hole to install a rib bolt, a piece of roof rock fell striking employee on his left gloved hand causing a laceration to the skin between the thumb and index finger.

Employee was shoveling coal along the rib when a piece of roof rock, 5" to 2" tapered by 28" wide by 52" long fell from between permanently supported roof bolts and struck the employee on his left shoulder and lower left leg

EE was bolting the roof, when he started a hole with a starter bit a rock chip came out of the roof and struck his left arm right above his elbow. The rock had sharp edges and weighed approximately 3 lbs.

Employee was installing a roof bolt when a slab of roof rock fell from between the last permanent row of bolts and the ATRS striking him on his back and right side ribs causing a contusion and pain.

Injured was setting a wooden post (5" x 6" x 60") under a "C" channel approximately 40 feet outby the face area. As the employee reached behind the post and started to move it under the channel, a piece of rock 1" x 6" x 12" fell off the top of the "C" channel striking the employee on his left wrist.
