From accounts of person involved in accident, EE was preparing to tram Feeder onto belt tail during 3 East belt move when power cable for Feeder was dropped. It appears it fell across tram controls undetected. When EE activated start control switch, Feeder Hydraulic Tram Circuit engaged causing it to tram toward him, striking him & pinning between Feeder & coal rib.

2 recordedEE traveled to get backsight in order to advance survey station in the #5 entry of the 2nd Butt Right off of South Mains section. The backsight was outby a run-thru check curtain. He was struck by a loaded shuttle car as it traveled from the #6 face to the feeder through the run-thru check, and incurred fatal injuries. He was found by the shuttle car operator on his return trip.

Roof started to work #1 Entry 11204, then started to work #2, #3 Entry Outby at 4:35 AM. Fall occurred 11173 #3 Entry for 140' on the 7200 high voltage. Area is cribbed and abandoned.
IE was working with guard on feeder to replace shear pin, guard shifted and pinched IE's right index finger between guard and frame of feeder. IE was wearing metacarpal gloves at time of incident.
During the reaming of a bolt hole to allow a cable bolt to fit, the drill steel fell from the roof striking IE left middle finger.
IE stated that while walking to catch up with a co-worker, who was carrying a hammer, he stepped into an irregularity in the bottom causing him to stumble forward hitting into the hammer being carried by his co-worker. As he fell, the hammer struck his lip causing a laceration requiring 3 sutures.
IE was hanging power cable for belt drive when he felt a pinching sensation in his lower left abdomen. IE worked full duty up until 06/04/14, date of surgery.
Roof Fall happened at 6 East #4 Panel (Intersections & Crosscut #3 to #4 Entry). Size of Roof Fall: 100' Long x 19' Wide x 6' High. Ventilation was not disrupted. Belt structure and High Voltage cable involved.
After drilling hole for roof bolt & putting up channel, realized hole needed enlarged to accept bolt. As he started larger drill steel thru channel into initial hole, had his hand on channel to assist getting steel thru hole in channel, when drill steel contacted channel causing his finger to be pinched between channel & roof causing laceration & fracture to right index finger.
Employee is a belt man and has been for 11 years. ee has been experiencing swelling & pain in left knee from repetitive crawling and kneeling. EE has worked full duty up until 03/12/14, when doctor put on modified duty. No modified duty available. Last day worked full duty 03/11/14. Occupational Illness.

While trimming 36"" section belt prior to making a splice, injured worker using a hawk bill knife was cutting through the belt when the knife caught the board. Injured repositioned himself to get leverage. While applying pressure the knife skipped out of the board and across the belt contacting his right knee causing a laceration requiring 5 stitches.
IE was tramming miner, he was walking in a right to left direction. The miner was stopped and IE stepped into some uneven bottom and felt a popping sensation in his lower back.
Crew has just mined 15' out of #5 Entry off 20x10 x-cuts when roof fell in cut, then intersection started working. They backed the miner up and a few minutes later the intersection fell in.
IE was driving toward section belt on Johnson Man-Trip. Riding w/right hand resting on warning bell & driving w/left hand on solid steering wheel. Man-trip hit uneven bottom causing operator to grip bell w/right hand. The bell bowl spun pinching his finger between the bell and the clapper causing a FX. Continuing to work full duty. Surgery is scheduled for 07/09/13.
IE was traveling into mine, checking belt condition when he hit uneven bottom contitions causing steering wheel to turn twisting right hand.
Mine Maintenance Chief was working a come-along to pull pipe together when he felt a pain in his right shoulder. Initially diagnosed as a right shoulder sprain and he was released to regular duty. A recent MRI shows anterior labral tear of his right shoulder requiring surgery to repair. He become a LTA on March 1, 2013.

Employee was drilling roof, 2nd steel in roof had 3rd steel in right hand to get ready to put in roof resting on right leg when a piece of coal came off of canopy & struck employees right middle finger. Note: This is being reported late as report received from Indiana ER did not state that a suture was inserted.
Employee was helping move MRS, he was 40' back watching the cable and the MRS operator was 25' away from MRS when he pressurized it against the mine roof. The bolt end broke off and became like a projectile striking employee on the lenses of his left eye glasses breaking the left lenses and striking the left eye.
Employee went to swing canopy out to install a bolt and did not realize it was against roof and it let go and it hit him on the left wrist.
Employee was moving a large rock (4'square 4"" thick) when rock came down on right pinky finger, smashing end of finger and removed fingernail and split end of finger. He was wearing gloves.
Employee was hit with a pice of rock on left hand when putting drill steel into chuck, rock hinged off ATRS and fell onto chuck.

Employee had just completed setting Rail Jack for Tail. It was just about tight when employee got up, he placed his left hand on Tail when the Jack left go falling, striking his left hand between the Jack and Tail Piece.

Employee was driving the Johnson #5 man trip when he turned to go under overcast the tire caught bottom, causing steering wheel to turn, twisting right arm to right and down. Employee had both hands on the steering wheel.
Tramming Joy CM downgrade when the miner began free-wheeling down the grade. The employee, positioned outby the miner, got his feet caught by the outby strands (loops) of the miner cable causing him to fall and be dragged along the bottom for a short distance.
Employee was walking around the roof bolter to get a plate for a bolt when he hit the left side of his face on a channel sticking out of back of bolter.

Employees were heading outside from the 4 West Section and was proceeding to go through a run-through when EE put his leg up in the air to push the run-through up and he caught his left leg on a roof bolt. EE complained that his left knee was hurting.

Employee putting 1st bolt in row(outside bolt). He started hole, noticed loose rock,let it fall,drilled hole,created a ledge 10""x10"" when he put up a roof channel with bolt in hole, bolter head out of reach,added bolt wrench,he pushed up bolt channel hit offset created by rock that fell out & wrench slipped off bolt striking him in safety glasses, nose & forehead area.
Employee started to drill a hole when a piece of rock fell out of the roof and hit the canopy, then struck employee in the left arm.
Employee walked and crawled to back of roof bolter, when he turned to go around the bolter he felt pain in right knee. Employee has worked full duty since 11/30/2010. At 01/18/11 doctor's appointment he was instructed to work modified duty with surgery to be on 01/24/11.
Employee was trying to remove stuck drill insert and had hit the top off insert with a hammer. A piece of insert struck employee on right leg between leg and crotch.
Employee was opening a 4"" shut-off valve, when valve came off it struck employee above his right eye causing laceration.

While preparing to load bolts, EE approached the scoop. As the scoop man shut the scoop off, he did not alert anyone that he was lowering the bucket. EE had put himself in a bad location.
While the examiner was running the Rt Return he found a fall 70' long in 0 Entry of South Line Room 3 XCut Outby Station # 8533. The Fall was above the bolts. The area had been bolted with 6.5 3 piece bolts with T-3 Channel and 10' Cable Bolts. Both agencies were notified, the Fall is in ouby area and did not impede travel and/or air.
Employee was pulling shuttle car cables and got peeble lime on his left wrist and right hand causing a chemical burn.
Employee was crawling on hands and knees while doing ""Run Through Check"" and cut his left hand on sharp piece of rock.
EE experienced discomfort in upper leg after shift on 3/9/10. Went to chiropractor on 3/15, 3/17 and 3/19. On 3/21/10, EE went to ER and was administered muscle relaxer, painkillers and anit-imflammatories. Scrip from ER stated no work until 3/29/10.
Employee was roof bolting in the North 3 Parallel Section when a piece of rock fell out striking the bolter head and landed on employee's right knee.
Employee was doing ""Run Thru Check"". The scoop was parked on inby side of check, when doing the check he hit battery end of scoop causing him to strike his lower lip on the steering wheel of his Little Mac mantrip causing superficicial laceration.
Employee was tramming throught the XCut 3-2 when he tried to get under a damaged channel when the bent portion of the channel hit him on the right hand lacerating his right small finger.
While installing roof bolt, employee twisted his right knee. He has worked full duty until 1/18/2011 when he re-aggravated the injury. Employee is working modified duty effective 1/25/2011 per doctor's orders.

Employee had appeared to have had a massive heart attack. The preliminary report from the Indiana Coroner's Office states that it appears to be non-trauma related. So far the investigation seems to indicate EE died of natural causes.
Employee was riding out of the mine when the battery powered mantrip went over a hill and hit a hole in the roadway and caused his right leg to bounce out of the mantrip and be pinched between the trip and a crib.
Roof fall was discovered by th Air Coarse Examiner in Old 4 East x #2 between 1 - 2 room outby 1 xcut & between 2-3 room at the 1st xcut & oubty 1 x cut. Area was bolted with 6.5 bolts 3 piece & 8' cable bolts. The fall was above the bolt ankorage. The area is being secured by cribbing & timber and abandoned.
The outby mechanic was doing his permissibility on LC #8, when he went to location he found a roof on top of the 7200 cable. He went & got foreman & knocked power. Area approx. 100' long from STA #8302. Area was bolted with 4.5 1 piece bolt. The area has been cribbed off and 7200 re-routed. This fall was in South Main Section. The height at the fall was approx 5'.
Pre-shift examiner was preshifting mine for Tues AM in 4 West Section. He stopped at pump along Intake in #7 Entry, he smelled fresh resin. He went ouby 5 xcut in #7 Entry where he found a roof fall in the Inter. 4 xcut outby STA at 8152. This had been bolted with 4.5 2 piece bolts & 8' cable bolts. Area is cribbed off & abandoned. Fall measured approx 35' long 70""x 8"" high in Int.
Face of #4 Entry in the South Main Section at 1950 outby to 18+57. There was 8' cable bolts in the inter. 19+23. The fall was about 8' high just above ankorage zone. The fall measured approx. 100' long. According the the report from prep-foreman, the roof started working in the inter. 19+23 and the face of #4. They went for cribs & approx 15 minutes later it was on the bottom.
Roof Fall in 4 East occurred above the bolt ankorage in the Intersection of #4 Entry at 4+ 18 in the Last Open Crosscut. The fall was approximately 80' in length and 8' high. This happened between shifts and was found at 12:10 AM by the 3rd shift prep foreman.
Employee had pulled miner into cut and turned his head, striking side of head against end of channel causing a one inch laceration above his left ear.
XCut 3-4 at 5+37 at Intersection in #3 started working approximately 9:00 AM. Employee called to inform me and said that they were cribbing & timbering. On the way in I contacted the outside and by 9:30 AM I was informed that the above area had caved in on the LD Box.
Employee was bolting in 4 West section and had his left hand on drill head, steels fell out of roof striking employee on left hand.
While bolting in the 4 West Section employee stopped drilling and lowered head with steels in roof to check suction, steel fell out and struck employee on left middle finger.
Employee was striking rock with a hammer when a piece of metal broke off puncturing his right arm. At time of injury employee was pounding one hammer against another hammer.
Employee was washing off Joy miner when he got foreign object in his left eye.
10' of intersection at station 6989 on 10' radias from the solid. The area was bolted with 6.5' bolts and T-S Channel 10' cable bolts. The area is cribbed up.
Employee claims as he was getting up to move from one location to another he felt pain in his left hip. Employee continued working regular duty from date of incident until 08/07/09 when he was instructed by his doctor not to work underground. He worked outside modified duty 08/07/09 to 08/13/09 and returned to full duty on 08/14/09.
On 06/05/09 employee was lifting & loading belt structure into scoop bucket when he felt pull in lower left abdomen. Employee continued working full duty until 07/28/09 when physican instructed him to work ""light duty"". Employee is scheduled for hernia surgery 08/04/09.
While roof bolting employee put left hand on drill head, steel fell out striking employee on left ring finger.
Employee was installing a roof bolt (position channel), when he looked up, the bolt fell and struck him in the mouth.

While mining crosscut 5-4 at 3 + 60 25' into the crosscut an uncharted 9 5/8 drill hole which contained H20 & CH4. The power was de-energized, reventilated and deluted below 2%. By 11:40AM power was restored and pumping VIA 2 8 hp pumps out 1-4"" discharge 1- "" discharge line.
Employee was driving Lil Mac Mantrip and hit rock on roadway causing steering wheel to spin out of hand and turn wheel knob hit employee on back of left hand.
Employee was setting ATRS, when he pushed ""on"" lever to set ATRS, his right hand slipped off control, he lost balance and fell hitting his right hand on tire or bolter or bottom.
Roof fall found by mine examiner during preshift in intersection in common entry 600-feet outby the faces of the active South Mains working section 100-feet outby spad 6619. Cave measured: 20? x 19.5? x 6? high. No evident roof defects. Primary roof support = 6 ? foot resin-assisted torque tension bolts (4?x 4? centers). Secondary support = 12-foot cable bolts (8? x 8? centers).
Employee was building & plastering wall on belt advance. He said he had pain in right arm above wrist and had swelling. This was after building cribs in 2 West Room. Note: He was released to modified duty, but only showed for work 1 day.
Employee was operating scoop and ran over block, causing scoop to raise and pinch employee against roof.
Employee was knocking CAT pin out when a piece of steel dislodged, striking employee on lower lip. Employee did not seek medical attention until 07/09/07.
Employee was working with a co-worker loading crib blocks into the scoop bucket. As the injured man was picking up a crib to stack it, the co-worker threw a crib that struck him on the right index finger. The result was a broken finger.
Employee was drilling a 14' hole for a 14' cable bolt. As he lowered the head, the drill steels stuck momentarily in the roof and then fell out of the roof pinching his left thumb between the column of drill steels and the top of the bolter head causing a laceration on each side of the thumb. Employee was wearing Meta-Carpal gloves.
Employee was bending torque tension roof bolts (20+) when he developed pain in his right wrist, he has discomfort when closing his hand. He worked from time of accident thru 06/09/07. At 06/11/07 doctor's appointment he was instructed not to return to work till after surgery on 06/26/07.
Roof Fall that started at the Face #6 Entry 3 West Main STA#6103 extended initially 125', over the next 2 1/2 shifts Fall travelled outby on diagonal direction approx. 600' to #2 Entry STA 6001. 7 pieces of equipment (2 Miners, 4 S Cars, 1 Feeder & a Section Ld Cntr)inby on the right side were retrieved after the area settled down & several areas were supported by cribbing.
Injured was transporting 20 loose 10' cable bolts on top of scoop tractor. After backing into crosscut, employee pulled scoop tractor back out onto runway. As he straightened out the scoop, one of the cable bolts sprung toward employee hitting him on his right ear, causing laceration.
Employee was positioning the plate on a cable bolt when he lost his balance and inadvertently pushed on the up lever pinching his right thumb between the plate and the roof.
Employee noticed the past few months both hands going numb when using them and have been progressivley getting worse. Employee has worked full time up until 06/19/07 when he had carpal tunnel surgery done on the right hand.
#4 Entry Roof Fall just inby 5867 Station. A shuttle car was caught x-cut 4-3 and 4 man Personel Carrier inby in #4.
Employee's title is beltman, duties include repetitive actions.
Employee was putting stack of roof plates (4 Bundles) on bolter. The stack fell over striking his left forearm causing laceration.
Employee was kneeling beside the operator side controls of the #5 Fletcher Bolter. As he tightened the 6 1/2 foot combination bolt, a piece of roof rock (1/2 size CSE SCSR) fell between canopy and bolter boom striking his left gloved hand injurying his index finger. His hand was resting on his thigh it was hit.
Employee was operating helper side on Long Airdox Twin Boom Roof Bolter. As he was drilling a hole to install a rib bolt, a piece of roof rock fell striking employee on his left gloved hand causing a laceration to the skin between the thumb and index finger.
While running scoop, operator ran over a rock and the scoop went up and came down hard. He then felt pain in his neck and left arm.
Employee was dropping the head of bolter and had his leg in the wrong place, head of bolter struck him on his right thigh.

While delivering H-Beams to Section, employee slid beams from the corner of rib to center them up and avoid trailing cables. He then felt burning sensation in his right shoulder. Employee has worked since incident and is off effective today, February 23, 2007 for surgery on his rotor cuff.
Employee was installing a roof channel. The channel was sitting on the canopies. As the employee was installing the bolt, the channel slipped and ran down the bolt, striking the employee's left hand, lacerating the tip of his left thumb. Employee was wearing company meta-carpal gloves at the time of incident.
Employee was roof bolting and while adding a drill steel, he hit the up lever and caught his right thumb between the steels.
Empoyee was moving shuttle car tires. As he was attempting to move one of them, it started to roll away. When it started to roll toward the employee, the employee tried to get out of it's way and divert it at the same time. He experienced a burning sensation and felt a ""pop"" in the right groin area.
Employee was making a splice. When he was hammering the splice together a piece of the splice tool fragmented, striking the employee in the right knee.
Employee was bending over to check wheel unit bolts on a shuttle car when he experienced lower back pain that radiated to both legs.
Employee's job duty has been Roof Bolter since March, 2006. He has been diagnosed with Bilateral Carpal Syndrome due to work.
Employee was pulling tow bar off supply trailer that is used to drag pallet off, when he experienced pull in lower back.
The third shift had got to the Section approximately 12:00AM and there was rock down in #3 Entry. Also, at the Feeder the Prep Foreman sent the scoop man for cribs and the area at Feeder was the first place they worked and #3 fell in by the time they got to it.
The 2 Rt crew had just finished taking a cut in #9 Rm and move to #10, when the cut that had just been mined started to work. It fell in then the intersection just outby started to working, it also fell approximately 60' Long 7' height 18'wide.
Drill still stuck in the roof and employee swung drill head and caught left hand between steels and canopy (fractured left hand).
Employee was transporting trash to supply hole with scoop when he hit a bump on the tramway causing his head to jam into the underside of canopy.
Employee was greasing the screen belt drive when he caught his coveralls on the scrapper bar causing him to twist his lower back. He slipped and fell against the railing.
XCut 4-3 mined to the depth of 20', no break above 3'-4' where encounterd an area that was supported with 7' Installs and 12'cable.

Employee was installing a point anchor bolt. When he put the top and bottom together the coupler was near the roof; he had his left hand on coupler and hit the up pressure lever, jamming his left index finger in the hole.
Employee was cutting bands on a bundle of T5 channels and cut left wrist on metal band.
Employee was pulling cable off bolter.He felt ""pop"" in his right shoulder.
Employee notified Supervisor that his left hip has been bothering him more and more.
Bolter was bolting the#5 Entry inby Xcut Station 4 + 84. They were on their 4th row in the Face of #5 of a 20' cut. The bolTters went to the back of machine then heard the roof working over the bolter. Then they started to crib around bolter, the roof fell at Face #5 and into Xcut 5-6.
Employee was pulling mining cable to make better roadways when he experienced sharp pain in lower back.
Employee was installing a 6' double lock bolt, when he tried to push bolt into roof he felt pain in mid back.

Employee worked night shift of 6/16/05. When he came out of mine he complained of soreness in his right knee. Repetitive crawling and kneeling is done on a daily basis in his job duties.
Roof Fall #1 Entry 1 Right Section Outby Station #2946 2 XCuts
Employee was installing sail boats with co-worker and they were not lining up. They tried to adjust sail boats and EE lost his balance and the sail boat fell on his hand.
Employee was carrying a bundle of bolts alongside bolter, complained of pain in his lower back.
Area drilled OK on the mining cycle, no indication of any problems. Roof started working at 12:30-12:45, by 1:00 it was caved.
Employee was trying to get a pallet out of garbage tub while standing in tub. He was jerking on pallet when it became dislodged causing employee to fall back against dumpster striking his back.
1 Right #3 Entrty 22 to 1 Intersection caved in, cleats in roof with calcite, slick and slide roof just above the bolts, caved just above the bolts.
1 Xcut Inby Station #753 Roof Fall 3 Left Return. There was not any affect on mine ventilation or obstructed traveway. Area is abandoned and will be fenced off.
Employee was lifting lids to service battery. The lids were jammed together. When he tried jerking the lid free he pulled his right shoulder.
EE was installing last row of bolts in 3-2 Xcut, conditions were low. Drilled with 1st steel, needed more height to change steel. Held steel in roof with left hand, slid head on bottom. Smashed left hand between canopy and steel.
At end of 3rd shift, crew parked their mantrip by the garbage bin & were cleaning off mantrip. Standard 3rd shift procedure. Scoop operator followed crew out-stopped-unloaded his pallets & was in the process of pulling up to straighten out & back the duster into the charger, when he was almost straight the scoop started sliding & slipped into the MAC 8 that was unloading garbage.
At end of 3rd shift, crew parked their mantrip by the garbage bin & were cleaning off mantrip. Standard 3rd shift procedure. Scoop operator followed crew out-stopped-unloaded his pallets & was in the process of pulling up to straighten out & back the duster into the charger, when he was almost straight the scoop started sliding & slipped into the MAC 8 that was unloading garbage.
Employee was troube shooting shuttle car cable, using a screw driver to pry the leads part. He was using a razor knife, cutting toward the screw driver-the knife slipped- cutting his right ring finger. The laceration required 3 stitches.

While driving the mantrip, employee hit a piece of rock with front tires causing the steering wheel to spin around hitting him on the left wrist.
While building a return wall, employee stacked a few blocks closer to the wall. While tightening up the top of the wall he felt a sharp pain in his left side, this pain radiated into his back and leg.
While employee was working on the belt he crossed over the belt, got caught in the cables and fell off the belt. The belt was not Running.
EE'S JOB IS SCOOP OPERATOR. HE HAS HAD SWELLING & PAIN IN HIS LEFT KNEE FROM BENDING & CRAWLING.
While loading supplies on the roof bolter, employee twisted to grab another handful of bolts when he felt his knee ""pop"". He fell down grabbing his knee in pain; when knee was checked there was no swelling or visable signs of injury.
Employee was checking wheel unit bolts on a shuttle car when he bumped and twisted his left arm.
At the end of 2nd shift an air connection was made between #3 and #4 Room; there were no signs of any visible roof problems and according the the Roof Bolters everything drilled good! At approx. 3:00 AM the mechanic's heard the roof crack - but did no other working. At approx. 5:00 AM they heard another crack - roof started working and caved.
EE was getting back into the High Lift - there are 2 hand grabs to help you get into the High Lift - one is easily accessible - the other is close to the door-according to EE as he was reaching to get the handle that was close to the door, his foot slipped and he missed grabbing the handle, he fell backwards on the ground. *Employee quit 8/5/2004.*
A roof fall occurred in the 1 Right Section MMU-001. The fall occurred in the #1 Entry of 1 Right Section. The fall began just inby Station #1902 and extended inby approximately 150'. The area was bolted with 4' point anchor resin bolts. Fall occurred above the anchorage of the bolts. No men or equipment were involved. Ventilation was not interrupted.
When mining the xcut 5 to 6 in 1 R/T, the Roof Bolters detected a 1 foot break - they added extra 5' bolts, when the miner started the xcut 6 to 7, knowing that there was a break at 1 foot only took a 15' lift when the lift was completed approximately 2"" of rock fell, roof started working and caved approximately 6' high - 18' wide - 28' long.
While mining Rooms 22,23, 24, 25 - we were in our last set of xcuts, a xcut was turned 23 to 22 Room - when testing the roof, it was drummy and a short cut was taken 15' - when the bolter came into bolt, they heard the roof cracking and working. Area did fall in a approx. 12:15 p.m.
After a long weekend employees were in the process of loading bolter supplies. Injured grabbed a handful of 6' bolts and when he twisted to put them on the bolter he felt a sharp pain in his back.
ACCORDING TO TECH. SUPPORT, WE ARE IN A CHANNEL OF BAD ROOF, WITH JOINTS, CALCITE, MAKING THE ROOF VERY SUSCEPTIBLE TO FALLS.
WHILE TIMBERING AN AREA INBY THE FALL WHERE THE ROOF SHOWED SIGNS OF BEING BROKEN UP, THE AREA THAT FELL STARTED TO WORK. THE MEN TIMBERING WENT TO GET THE FOREMAN. WHEN THEY GOT BACK THE AREA HAD CAVED. THE AREA WAS BOLTED WITH 6' BOLTS & CABLE BOLTS INBY AND OUTBY THE FALL. THE AREA THAT FELL WAS BOLTED WITH 4' DOUBLE LOCK BOLTS.
#7 ENTRY OF 2 WEST-#5 ENTRY OF 2 RIGHT-THIS AREA CAVED AT 3 PM ON 4-3-04. THERE WERE NO VISIBLE SIGNS OF ANY CLETES OR CUTTERS-AREA WAS BOLTED WITH 4 BOLTS: THERE WERE NO ROOF PROBLEMS WHEN THIS AREA WAS BOLTED ON 2ND SHIFT 4-2-04.
EE WAS IN THE PROCESS OF LOWERING THE DRILL HEAD ON THE BOLTER, AS HE WAS REPOSITIONING HIMSELF TO START BOLTING HE SLIPPED ON A PILE OF COAL, CAUSING HIS RT. THIGH TO SLIP UNDER THE DRILL BO OM AS IT WAS BEING LOWERED.
THIS IS A CONTINUATION OF THE ROOF FALL THAT OCCURED 2/20/04 AT 12:30 PM.
WHILE MINING IN #7 ENTRY THE MINER OPERATOR HEARD THE ROOF CRACKING A FEW TIMES. THE FOREMAN THEN CHECKED THE AREA & FOUND THE ROOF TO BE SOLID-TEST TAKEN: WHILE THE FOREMAN WAS PRESENT THE RO OF STARTED CRACKING AGAIN. THE MINER WAS MOVED TO #5 ENTRY, THIS IS WHEN THE XCUT FROM #5 TO #7 STARTED WORKING & CAVED.
ON 1/31/04 THE XCUTS FROM 3 TO 5 OF 2 WEST STARTED WORKING - TIMBERED AREAS OFF OVER THE WEEKEND: REBOLTED INTERSECTION IN #4 & #3 WITH 9'BOLTS & CABLE BOLTS, CRIBBED INTERSECTION. AT 11:50 PM MONDAY JUST THE XCUT CAVED IN, THERE WERE NO CHANGES IN #4 XCUT 3 TO 4 - ALSO - 2/6/04 A CONTINUATION OF THIS CAVE, IT CAVED IN XCUT 6 TO 7 AND #7.
WHILE RIDING ON A PEROSNNEL CARRIER THE INJURED HIT HIS HEAD ON A ROOF BOLT.
EE was carrying roof bolts and developed a hernia.

INJURED WAS SITTING ON MANTRIP VEHICLE WITH HIS HAND OVER THE SIDE OF VEHICLE. WHEN VEHICLE STARTED TO MOVE THE INJURED'S HAND WAS BETWEEN THE GROUND AND VEHICLE.
AFTER EVERYTHING WAS BOLTED, TWO WORKERS WENT BACK TO INSTALL BELT HANGERS. THEY SET THE BOLTER UP TO INSTALL THE TIGHT SIDE BELT HANGER - WHILE CO-WORKER WAS INSTALLING THE HANGER, EE WENT IN BETWEEN THE RIGHT SIDE DRILL BOOM AND THE ""T"" BAR. WHILE LOOKING FOR THE NEXT BELT HANGER MARK - HE PUT HIS HAND ON THE DRILL BOOM. HIS 2 FINGERS WERE CAUGHT 2 MOVING PARTS & CUT OFF.
ROOF FALL WAS FOUND IN #7 ENTRY, SURVEY STATION 1693 AT 18+50. IT WAS FOUND ON THE SUNDAY NIGHT PRE-SHIFT EXAM. ON SAT. 8/22 A SLIGHT PSI BREAK WAS DISCOVERED IN XCUT 6 TO 7 AT 18+50.
A ROOF FALL OCCURRED IN THE INTERSECTION JUST INBY SPAD 1551 OF THE 6 LEFT SECTION: NO VISIABLE SIGNS OF CUTTERS OR CLAY BEINS. AREA WAS BOLTED WITH 42"" #6 REBAR TENSION REBAR BOLTS. INITIAL C AVE WAS APX 5'HIGH, 18'WIDE, 18'LONG. AT 6:30 AM CAVE EXTENDED OUTBY APX 100' - 18'WIDE, 7'HIGH.
WHILE MAKING THE SUNDAY MINE RUN EE FOUND THE CAVE IN 1 WEST SECTION 42+10 IT CAVED FROM #2 TO #7 APX 8' OF ROCK FELL ENTRY WIDTH 19 TO 20' 42"" TENSION REBAR BOLTS - 6 ACROSS - A BOOM HOLE WAS CUT FROM 4 TO 2.
WHILE SERVICING THE MINER, EE REMOVED THE RUB RAIL FROM THE MINER TO HOSE AROUND THE SCRUBBER MOTOR. WHEN REPLACING THE RUB RAIL HE FELT SOMETHING PULL IN HIS LOWER BACK.

A CAVE WAS FOUND IN OUR COMMON ENTRIES, 1 XCUT OUTBY SPAD #195 AREA IS INBY WHERE AN OTHER CAVE HAD FALLED ON 7/20/01. THE AREA THAT HAD FALLED WAS BOLTED WITH 6'POINT ANCHOR BOLTS.
A X CUT BETWEEN 36 AND 37 ROOM WAS JUST CUT THROUGH APX A 15' CUT. WHILE MOVING THE MINER, THE 15' CUT STARTED WORKING AND FELL IN ABOUT 2' TO 3' HIGH, AT THIS TIME THE ENTIRE X CUT 36 TO 37 S TARTED WORKING AND EVENTUALLY CAVED.
EE & FOREMAN WERE SITTING @ FEEDER DISCUSSING WHAT NEEDED TO BE DONE DURING SHIFT. #2 CAR PULLED ONTO THE FEEDER WHILE THEY WERE TALKING. EE GOT UP, WALKED PAST #2 CAR- OPPOSITE THE OPERATOR- GOING TOWARD HIS MANTRIP. #2 CAR PULLED OFF FEEDER & WAS MAKING A LEFT TURN WHEN THE BOOM OF THE CAR STRUCK EE KNOCKING HIM TO THE BOTTOM. SHUTTLE CAR OPERATOR DID NOT KNOW HE WAS THERE.
WHILE RUNNING SHUTTLE CAR HE HAD A DISCOMFORT INHIS CHEST. WHILE HELPING TO MOVE THE MINER HE BECAME SHORT OF BREATH. HE WAS THEN BROUGHT OUT AND TAKEN TO INDIANA HOSPITAL.

ROOF FALL IN THE #5 ENTRY OF WEST MAINS AT 4+70 APPROX 20' LONG, 10' WIDE AND ABOVE THE ANCHORAGE (USING 6' DOUBLE LOC BOLTS) WITH ROCK DOWN ALONG THE RIGHT RIB IN #5. THE AREA THAT FELL WAS M OSTLY ONE LARGE PIECE.
WHILE MOVING THE MINER, EE POSITIONED HIMSELF IN THE MIDDLE OF THE MACHINE, WHILE TRAMMING FORWARD, THE CABLE TENDING ARM CAUGHT HIM FROM BEHIND, WHEN THE ARM CAME IN CONTACT WITH EE, EE HIT T HE WRONG TROM LEAVER, WHICH PINCHED HIS LEG BETWEEN THE RIB AND CABLE TENDING ARM.
CAVED IN THE XCUT #1 TO #2 OF WEST MAINS, APX. 140' INBY STATION #125 OF NORTH MAINS ( THE CAVE IS 140' LEFT OF SPAD 125). THE CAVE IS APX. 50' LONG, 17' WIDE AND 8-9' HIGH. THE XCUT WAS BOLTE D WITH 5' TENSION REBAR BOLTS. THIS CAVE WAS FOUND WHEN MAKING A WEEKLY EXAM OF THE RETURN AIR COURSE.
A ROOF FALL OCCURRED AT APX 11 PM IN THE #2 ENTRY OF NORTH MAINS STATION #73(9+10). THE CAVE IS APX 30 LONG 20' WIDE 15-20 HIGH. #2 ENTRY WAS BOLTED WITH 6' DOUBLE LOCK BOLTS UP TO WITHIN 10 F T OF THE CAVE. THIS AREA DRILL GOOD WHEN THE REBOLTING WAS DONE. THIS AREA WAS BOLTED WITH 4' FULLY GROUTED BOLTS.
A ROOF FALL OCCURRED PRIOR TO THE 0600 PRE-SHIFT. THE FALL WAS FOUND AT 0628. THE LOCATION OF THE FALL WAS IN #1 ENTYR OF NORTH MAINS, STARTING AT SPAD #65 (8H0) GOING OUTBY FOR 60'. THE #1 EN TRY IS A RETURN ENTRY.
