IE was setting up to start loading with the CM. IE had adjusted IE's tubing for ventilation & as IE walked outby towards the end of the CM a rock hinged off of the last row of roof bolts & knocked the tubing down. The tubing knocked IE down & the rock flipped over onto IE's left ankle which caused the injury.

THE INJURED WAS CHECKING BELT STRUCTURE IN A BELT CHANNEL. A ROCK 30" BY 23" BY 7" FELL FROM BETWEEN THE ROWS OF ROOF BOLTS AND STRUCK INJURED ON HIS SHOULDER AND HEAD.
