Employee was assisting roof bolter operators conduct a methane test by the use of an extendable probe when a piece of mine roof fell from between 2 steel channels,(that were supported with 60" fully grouted rods), striking his right foot, causing a crushing injury to the tip of his big and third toe. The piece of mine roof did not appear to be loose on initial examination.

Showing all 4While operating a Continuous Mining Machine on MMU 001-0 section in #8 right x-cut (punch thru), EE was struck on top of his shoulders with a piece of mine roof that appeared to be secured to the mine roof, became loose while mining #8 x-cut into #9 entry. The piece of rock fell from between permanent roof support and the coal rib.
Roof Bolter Operator was in the #7 entry and was in the process of installing a roof bolt when his drill steel hung up. When he reached over to pull the steel out of the hole, a piece of draw rock fell from the roof and struck him on the right hand.
The miner operator was cutting coal in the #5 entry and was getting ready to start his clean-up run when a piece of draw rock fell from between the roof bolts hitting him on the head and left shoulder. He was standing three rows of bolts from the unsupported top. Employee did not start missing work until 3/27/07.
