Grabbed hood and put feet on bumper of Kenworth truck by himself to try to lift hood. As he pulled he felt pain in lower to mid left side of stomach. Taken to ER and CT was done. Showed negative and doctor said he had pulled a muscle.

Chute was plugged with large blocks of coal at crusher. A slate bar was being utilized with a sledge hammer to break the material free. Sledge hammer glanced off of bar being used and struck victim in right forearm resulting in cracked ulna bone.

Turned ankle while stepping down. Refused to go to ER. The incident occurred under previous ownership Arch Coal. Pine Branch took over the properties on March 4th. On April 16th a citation was issued to Pine Branch for failing to report. This report is being completed to avoid further action being taken against Pine Branch after discussions with MSHA.

We have experienced a non injury unintentional roof fall outby the inactive section. The roof fall occurred on 2 South Main in the #6 entry (intake) in an intersection at cross cut #14. laminated shale gave way from deteriorating. The approximate size of the fall is 5 feet thick, 35 feet wide and 20 feet long.

This mine has experienced a non-injury un-intentional roof fall outby the working section. The fall occurred on the intake of Six North which leads to 001 section. The fall is approximately 9 feet thick, 60 feet long and 20 feet wide. This area was initially supported with 60 inch fully grouted rods and 8 foot cable bolts. Laminated shale gave way from deterioration.

The above mine has experienced a non-injury unintentional roof fall in the outby areas of the mine. The roof fall occurred on the first left panel on nine South Mains which is in non-producing status. The fall dimensions were approximately 38 feet long, 20 feet wide and 7 feet thick. The roof strata was shale and initially supported with fully grouted resin bolts.

The injured sprained his right knee when he slipped while pulling a piece of ventilation brattice cloth out of mud. The employee worked until 6/14/2012 when this injury become a reportable injury. Further medical examinations revealed a tear in his right knee that has to be surgically repaired. Lost time began on (6/14/2012).

Flint Ridge Mine #2 has experienced a non-injury unintentional roof fall approximately 2000 feet outby the 002 section in the return of 4 Main in #1 entry. The fall was approximately 8 feet thick, 20 feet long and 50 feet wide in an intersection. The area was initially supported with 60 inch fully grouted rods. Laminated shale gave way due to deterioration.

EE was operating a 785 Caterpillar when a fire erupted. Flames were blocking the escape ladder so the employee jumped from the catwalk a distance of about 14 feet.

Employee was operating an 988 Cat Loader when he backed into a 16G Cat Grader that had entered the pit area. EE continued to work and went to the DR. on 06/18/2012 after receiving a jar which resulted in reoccurring back pain and was placed off work pending an evaluation by a specialist. First day off work was 06/19/2012.

Employee was assisting roof bolter operators conduct a methane test by the use of an extendable probe when a piece of mine roof fell from between 2 steel channels,(that were supported with 60" fully grouted rods), striking his right foot, causing a crushing injury to the tip of his big and third toe. The piece of mine roof did not appear to be loose on initial examination.

The employee was lifting a de-watering pump into a supply truck when he felt a sharp pain in his right shoulder. The employee continued to work until further medical examinations revealed a tear in his right shoulder that must be surgically repaired. This injury become a lost time injury on 02/06/2012.

The injured was changing a bearing on a 2 person rubber tired man trip in the mine shop on the surface. The injured was beating the bearing into the housing with a header board and a hammer when the board bounced up and caught his right index finder between the hammer handle and the board. The laceration required 4 stitches.

The injured was riding a rubber tired mantrip on his way to his work site when the mantrip dropped into a hole in the roadway on 4 South. The mantrip bounced causing the injured to bounce up and down in his seat dislocating the tip of his tail bone.

The fall was approximately 10 feet thick, 70 feet wide and 20 feet long. This area was initially supported with 60" fully grouted rods.

The fall dimensions are approximately 30 feet long, 40 feet wide and 4 to 6 feet thick with laminated shale.

The fall is app. 20' long by 20' wide and 5' thick. Laminated shale had deteriorated. The area was initially supported with 48" fully grouted rods.

Employee was using a pry bar to remove a pin from a lift jack on a D-11 Caterpillar Dozer when the bar slipped catching his finger between the bar and the lift jack. Employee was placed on restricted duty for approximately 4 weeks on 03/04/2011.

Laminated shale gave way which was supported with 60 inch fully grouted rods.

The piece of rock fell from between permanent roof support and the coal rib.

Employee had just finshed routine maintenance on a drill and as he was walking away from drill he slipped on a berm of dirt. When he reacted and tried to catch himself he devloped a pain in the left side of his ribs.EE finished his shift and went to the Dr. on 09/09/2010 and was placed on light duty work for five days.

EE stated that as he was dismounting a D-11 Cat Dozer his knee locked up and made a popping sound. EE finished shift then went to his family DR where X-Rays were Negative. EE returned to Dr. and an MRI revealed some torn cartlidge. EE's first day off work will be 07/05/10 as the mines were shut down from 06/26/10 thru 07/05/10 due to vacation.
