Employee was operating scoop when a piece of draw rock approximately 16" in diameter & 1/2" thick struck him on the side of his head causing his head to strike the frame of the scoop deck.
Employee was installing a roof bolt when a piece of rock fell and hit the employees left hand. This caused his pointer finger and little finger to be pinched between the rock and drill pot which caused a laceration that required sutures. This is to report medical treatment only NO days were missed.
While operating a Roof Ranger II from the operator side installing the last row of bolts in the number #7 entry a piece of draw rock fell striking the employee on his right hand. This caused a laceration above his right little finger that required sutures. This is to report medical treatment only NO days were missed.
While advancing the roof bolter up for the next row of roof bolts a piece of draw rock fell striking EE on the left little finger. This caused a fracture of the second metacarpal joint on the left little finger that required medical treatment. This is to report medical treatment only NO days were missed.
While bolting in the #4 entry ee attempted to put a bolt in a newly drilled hole when a piece of draw rock fell out. The rock hit ee right hand pinching it against the drill pot. This caused two lacerations that required sutures. This is to report medical treatment only No days were missed.
Roof bolter was installing a roof bolt on the rib side and as he put two pieces of steel together a piece of draw rock fell from the mine roof and struck his right middle finger lacerating it. The laceration required (4) sutures. This is to report medical treatment only.
Roof bolter operator was bolting in #2 entry on the off side of the machine, when a piece of draw rock 5.5"L X 18"W X 3.5" thick fell from the top and struck the drill head and then struck him on the right forearm. He required (9) sutures. This is to report medical treatment only.
Operator installed outside roof bolt and began to swing head in. A piece of rock 18"L X 8" W X 2" thick fell and struck his right hand which he had placed near the drill pot. He sustained a simple fracture to his ring and little finger of his right hand. Medical treatment was to splint (tape them together). This is to report medical treatment only.
As the roof bolter operator lowered his canopy to swing over and set another bolt cap coal measuring approx. 5" wide x7" thick X 22" long fell and struck him in the lower back.
Injured stated that he was removing draw rock (from roof) with hammer when "wrong" rock fell out. Operating roof bolter operator's side waiting for other guy to get finished. EE states that he bent over and the rock fell and hit back of head causing contusions.
After moving the bolter up in a cut, the operator reached out past the canopy and laid his right hand near the head to help himself get in postion to bolt. A rock fell out and struck the top of his right hand lacerating his middle finger. He received 5 sutures. He was not wearing metacarpal gloves. This is to report medical treatment only.
