Showing all 7A slab of mine roof fell striking him on his left side knocking him to the ground. The roof rock measured at its maximum distance 15 foot long x 40 inches wide x 8 inches thick. The mine roof looked normal and was about 7' 6" tall. The section of roof that fell out had a polished slickenside surface above it that was not visible after the area had been mined and roof bolted.
Injured was bolting in #9 entry of the number 1 unit when his left foot was struck by a piece of draw rock causing a laceration and a fracture on his left foot.
An employee was bolting in the #8 Entry of the XC-07 On the MMU-004, when a piece of rock fell out from the left side of the entry striking the employee. Employee was treated for a head wound and received stiches for his lacerations.
A piece of draw rock fell out approximately 2 feet by 3 feet by 8 inches thick and struck an employee on the left foot behind his metatarsal. Injured was seen by orthopedic doctor and was discovered that employee had a left closed fracture of the 3rd metatarsal.
Injured was putting in supplemental support, during installation a small piece of draw rock fell and struck injured left arm. Injured continued working his shift, went to doctor following day and had a fracture in his radial shaft.
Injured was roof bolting in the #6 entry of the 1st Main South, when a piece of slated fell out of the roof and struck injured in the mouth. The slate chipped injured's front teeth, making this a reportable incident.
