I.E. stated a rock fell from in between 2 shiels hitting employee on EE's right middle finger. Employee transported self to ER and received 4 stitches and X-Ray showed a fracture.
I.E. stated rocks fell from roof, first hit in the hard hat knocking it off and then hitting I.E. in the head after hard hat was knocked off
IE was troubleshooting #3 LW shield from #4 shield. While bumping functions a rock was dislodged from between the two shields and struck IE in the left side neck as IE was watching for a blown hose. This resulted in IE receiving 12 stitches to left side neck.
IE was working on HG-8. IE was in entry #1 at crosscut #91. Right before the accident, the IE was standing next to the miner man talking, with back against the rib. IE was struck by two pieces of shale. The first piece hit the hard hat knocking it to the ground, second piece struck the IE on top of head resulting in five staples. Rock measured 8" long, 4" wide, 0-3" thick.
A rock fell while setting the ATRS, lowering the ATRS to get the rock off the screen more rock fell and a piece slid down the ATRS boom into the walk way.
Employee knocked a stopping at 102 x-cut on HG6. After knocking the stopping, employee was scaling some block that was hung on the roof and a 4 inch block fell 8.5 feet striking employee on right foot fracturing the middle toe.

The IE was assisting with hanging hog panel on the coal rib. The IE reached for a bar to help hold the hog panel up and a piece of rock hit EE in the left knee.

The IE stated to the operator IE was struck by a rock that fell from between the shields and struck hand. Stated rock was 6"x 8"x 2" and IE's hand was on the cable tray.
I.E. was hanging a vent tube up after it had been knocked down. A piece of rock measuring 4" X 5" X 1" thick hit EE in the mouth causing EE to have 4 stitches in the upper lip.
Employee was roof bolting and had pulled roof mesh onto the ATRS. Employee was holding on to it to stabilize the mesh when a rock 2' X 2" X 2" thick hit employee on the back of the hand.
Employee was walking back to tool boxes in the travelroad after being at the face. A piece of shale fell through the mesh measuring 3"X3"X1" thick hit EE above the right eyebrow causing a 1" cut that had to have stitches put in it.
EE was standing behind the operator while operator was setting the ATRS. The ATRS was almost to the top when a rock fell out hitting the ATRS and sliding down towards the canopy. The rock struck the canopy and bounced off striking the EE on the top of right hand causing a laceration that resulted in 6 stitches. The top was 14' high and the rock was 20"x14"x3" about 20 pounds.
I.E. was placing wire on the ATRS. While he was positioning the wire a rock fell out striking I.E. on the right hand. Which resulted in crushing his right pinky.
Employee was cleaning rocks off of the tail drive covers when a rock fell out between the shields and smashed his right middle finger.
Employee was making the faces on the section in x-cut left in the number 2 entry at x-cut 26. He walked up on the left side of the roof bolter and checked the top where the ATRS was located, and he was struck by a piece of rock, it bent his back down, and he injured his lower back.
Employee was roof bolting and installing the angle brackets and a piece of rock fell out of the roof and struck him on the right forearm. He went to the hospital and received 4 stitches in his right forearm.
Employee was running a drill and a rock fell out and hit him on top of the right hand and cut his hand between the ring finger and the knuckle. He went to the hospital and received 2 stitches and returned to work.
Employee was installing the wire mesh to the roof and rocks fell out smashing his fingers in the screen wire and the ATRS on the roof bolter. He went to the hospital and received 5 stitches in his right hand on top of the knuckle of his pinky finger.
Employee was hanging curtain on the unit and a sharp rock fell and hit him in the left cheek of his face. This happened in the #2 entry of the Headgate #2 Unit. He went to the hospital and received 9 stitches on his face and returned to work.
Employee was putting up screen wire and a rock slid out of the screen wire and hit him in the upper left lip area. He was taken to the hospital and received 4 stitches.
Employee bent over to pick something up of the side of the roadway when a sharp small rock fell and hit his right hand, it cut his right ring finger. He was transported to the hospital and received stitches in his right ring finger and returned to the mine. This happened in the #1 entry on the Headgate Unit. 001 Unit.
Employee was hanging roof bolter cable across the entry of #2 entry of the Tailgate Unit at x-cut #90, a piece of rock fell from the screen wire and struck him on the back of the neck at the hair line and made a laceration down the back of the neck. He was taken to emergency room and received stitches in the back of the neck and returned to work.
Employee was putting up angle brackets on the tailgate using the single boom roof bolting machine. When putting the bolt into the hole a piece of rock slid out and hit the employee over the left eye. We took him to the doctors office and he received 15 stitches and he returned to work.
