Showing all 3The EE was installing split set roof bolts with a jack leg drill. While driving the bolt into place the steel plate on the roof bolt knocked a rock out of the back. The rock hit the miner in the head, his hard hat was on his head pushing his face onto the jackleg drill, breaking his tooth.
The miner had just ate lunch, walking back to work area, a rock fell out of the back and struck the EE in the head. The EE was taken to the hospital on 8/15/10. The accident was not reported until 8/15/10 because the miner was observed underground and on 8/15/10 and did not know who he was. Was told at the hospital by his wife about the rock striking him.
