EE was getting a crib off of the head drive when a rock fell out from between the shields, striking EE's left hand.

In the process of loading the last clean up car in the #3 entry on the left side of the cut. They positioned the miner cable on the rib and started loading the shuttle car. As employee was loading the car, a rock 40" long, 8" thick and 8" wide fell from the roof at the left rib line and cantilevered back striking them in the head and back area.

A small rock fell between shield #68 and #69 hitting EE in the left thumb, causing a laceration in which required stitches.

While shoveling on LW shields, EE put a hand on cable tray to steady self as shields were being moved. Rocks fell out between the shields and landed on top of EE's right hand.

Making the first cut of 2-L when EE was struck by a rock in the back of the head and neck. EE was seen at the Hospital and was released to work. EE continued working then was seen by EE's personal doctor due to having vision problems and was then taken off work beginning 9/17/2018 with the diagnosis of a concussion.

EE was supervising personnel drilling holes at an angle above the LW pillar for Jennchem glue insertion to help stabilize the strata in advance of the LW face. EE was struck and covered by rock that fell between the permanent roof supports and broke through the wire mesh. This employee received a broken ankle, pelvis and ribs and remains off work.

Was down on knees digging a hole to set prop, a piece of material struck them in the back of the head. This incident became a Lost Time injury on 10/26/2017 due to becoming unable to perform essential job functions assigned to them.

Employee was filling up duster at fan. Employee had right hand sitting on top of the dust hopper on the fan when a small piece of loose rock came off of corner between roof and rib striking employee's hand, causing employee to receive stitches

After finishing bolting the 1st turn of the left handed crosscut, employee was loading the bolter tray with roof bolts. As employee bent over to grab the bolts, a rock fell out between the pins of the bolted crosscut. Striking employee in the right shoulder area knocking employee forward.
