Moving belt covers from a pallet and loading them on a cart to move them down in the tunnel. While lifting the cover EE felt a pop in EE's left elbow and couldn't move EE's arm. Was taken off work by doctor on 04/03/2026.

While roof bolting operator's pusher steel cross threaded in the drill chuck. Operator attempted to dislodge the drill steel, the drill steel was bent, the steel kicked back striking operator in the hardhat causing a laceration. Operator received stitches.
During examination of the Slope Hoist rope, a distortion was found on the rope at the 220 foot mark behind the Brakeman Car. Upon further investigation a faulty rope roller was found at the Slope Turnout at the 220 foot area off the mine bottom.

The crosshead for the Single Drum Lining Hoist became caught on top of a concrete pipe bracket that extended from the shaft liner 8.5" at 388' as the hook continued to be lowered down to a depth of 500'. The crew moved the work deck back to center the crosshead fell off the concrete pipe bracket and fell down to the headache ball causing 6' of damage to lower end of the hoist rope.
While employee was cutting a piece of belting on the roof bolter that was attached to the ATRS arm, with their right hand using a belt knife, the knife slipped and cut their left forearm causing them to receive stitches.

While shoveling on the slope belt, EE saw a rock bounce off the top belt and land on the belt rail. EE tried to pick it up, but it was heavier than EE thought. The rock fell towards the bottom belt and smashed EE's left ring finger and pinky finger on the top roller frame.

EE was drilling a hole in the top when the steel clogged. EE hit the steel with a 1' wrench. The wrench popped back up hitting EE in the mouth, splitting EE's lip as well as knocked out 2 of EE's teeth.
EE was standing on the shield. When the shield went into auto pull it came in to contact with EE's right foot, pushing EE's foot into the pan line. Was taken off work on 02/20/2025.
When removing the scraper, the frame that had been unbolted fell down, hitting EE's right upper leg and right knee cap. EE was taken off work on 02/19/2025.
Walking back from #2 entry at crosscut #42 to the longwall face, they slipped in a wet product from the pumpable cribs, twisting left ankle.
While reclaiming cables and hoses from the roof for the panline, they had their left hand under the cables to support them. The force of the falling cables and hoses slammed their left hand down against the panline.

Helping another crew member load props in a scoop bucket, EE threw EE's end of the prop in the bucket and it kicked back, smashing EE's ring finger between the bucket and the prop, causing EE to receive stitches.
While working on a conveyor chain under a shuttle car, when getting up off the ground, hard hat fell off and they tried to catch it, causing them to lose balance and fall head first into the shuttle car, causing a laceration which required staples.
While pulling a 72" bottom frame from the slope track through the compartment mandoor EE strained EE's lower back. Was taken off work on 10/17/2024
A rock came off the drum of the shearer and hit them in the face, causing them to receive stitches to their chin.
Assisting belt crew during a belt extension on the D3HG8 section. While installing a bottom roller the bottom roller bracket was not sitting flush enough with belt rail to install the "J" bolt. EE kicked the bottom roller bracket with right leg trying to get bracket in place and felt a pop in left knee.
Employee and another crew member were hanging miner cable in the #5 entry. When they lifted the cable to hang it, employee pulled something in their left shoulder. Continued to work until 7/18/2024 When employee was taken off work.
While working on the North side of the slope belt at Arch 737, EE stepped on mandoor, the door shifted causing EE to fall through. EE landed below on the slope track, lacerating left arm causing EE to receive stitches.
